Supply of equipment and medical equipment as part of the project "Reconstruction and modernizationo apparatus equipment of I, IV, VIII, IX, X, XI, XII Department of the Pediatric Center of the Central Clinical Hospital of the Medical University of Lodz. "Proceedings are conducted under the Subsidy Contract No. POIS.09.02.00-00-0105 / 17-00 / 305/2017/1494, Project no POIS.09.02.00-00-0105 / 17 "Reconstruction and modernization of the equipment of the Branch I, IV, VIII, IX, X, XI, XII Centersa Pediatric Central Clinical Hospital of the Medical University of Lodz "under the European Regional Development Fund as part of the Operational Program Infrastructure and Environment for 2014-2020.

Main Site: Independent Public Health Care Center Central Clinical Hospital of the Medical University of Lodz, Pomorska Street 251, Łodź University Pediatrics Center, Pankiewicza 16, POLAND.

Package No. 1, according to Annex No. 2 to the Terms of Reference, including:

• ENT table. The orderer will consider the Annex 8 requirement fulfilled if the Contractor submits that he is insured against civil liability in the scope of activities related to the subject of the order for individual packages:

The value of the bid bond for the entire offer of a given WyKonvents, is the sum of wades of the offered packages. The Ordering Party determines the amount of the deposit - in accordance with Chapter X of the ToR (Requirements for the bid bond).

According to the Regulation of the President of the Council of Ministers of 27.7.2016 on the types of documents that the contracting authority may request from the contractor in the procurement procedure, and forms, in which these documents can be submitted, inor confirmation of the lack of grounds for excluding the contractor from participation in the procurement procedure in the circumstances referred to in Article 24 sec. 1 point 12-23 and para. 5pts. 1 in connection with art. 25 ust. 1 point 3 of the Act, the orderer demands the following documents: Information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21, issued not earlier than 6 months-before the deadline for the submission of offers - certificate of the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxes, issued no earlier than 3 months before the deadline for submission of bids or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables with possible interestor fines, in particular obtained the law provided for by law, postponing or spreading overdue payments into installments or suspending the execution of the decision of the competent authority in full; The certificate of the relevant local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund, or other document confirming that the Contractor is not in arrears with payment-social security or health insurance premiums, issued no earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtaining the legally required exemption, postponement or payment in installments pthe plausibility or suspension in full of the implementation of the decision of the competent authority; A copy from the relevant register or central records and information about business activity, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 of the Act. - Contractor's statement on the lack of issuing a final court judgment or final administrative decision on defaulting payment with himtaxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming payment of such receivables with possible interest or fines or conclusion of a binding agreement on repayment of such claims - Contractor's statement on the lack of a ruling against him as a precautionary measure he is seeking a banfor public orders according to Annex 16 to the Terms of Reference. 2
