Extreme Lithotripsy Supply System

Acquisition of extracorporeal lithotripsy system according to the specification. Maximum contract value - 1 000 000 RON without VAT. The number of days until which clarifications can be requested before the closing date for submission of tenders / applications is 15 days. The term in which the Authority-The Contractor will respond in a clear and complete manner to all clarification requests / additional information: 10 days before the deadline for submitting bids, according to O.U.G. 107 / 2017.

Extracorporeal lithotripsy system, according to the specification

Amount: 1 piece - Declaration regarding non-compliance with the provisions of art. 164, 165, 167 from-Law 98/2016. The economic operator (leader, associate, third party, subcontractor) will fill in the appropriate requirement in the DUAE form. Fitting in art. 164, 165, 167 of Law 98/2016 on Public Procurement attracts the exclusion of the bidder from the procedure applied for awarding the public acquisition contract.

The supporting documents proving the fulfillment of the assumed bythe completion of the DUAE shall be submitted, at the request of the contracting authority, only by the tenderer ranked first in the intermediate rankings drawn up to complete the evaluation of the tenders.

Declaration on own responsibility for non-compliance with the provisions of art. 59 of Law 98/2016 on public procurement.

The economic operator (leader, associate, third party, subcontractor) will co-operatethe appropriate requirement in the DUAE form. The supporting documents proving the fulfillment of the undertaken by completing the DUAE are to be presented at the request of the contracting authority only by the tenderer ranked first in the intermediate rankings drawn up at the completion of the evaluation of the tenders.

Fiscal attestation certificate (including the payment of social contributionse) for legal persons, issued by the Ministry of Public Finance or its subordinate units, which have the status of budgetary creditor. The economic operator (leader, associate, third party, subcontractor) will fill in the appropriate requirement in the DUAE form. The updated supporting documents (tax certificates) demonstrating the fulfillment of the criteria will be presentedqualification, according to the information contained in the DUAE, which must attest to the lack of outstanding debts at the time of their filing.

The supporting documents proving the fulfillment of the assumed by completing the DUAE are to be presented, at the request of the contracting authority, only by the bidder ranked first place in the intermediate rankings completed at the evaluation completionarea of offers.

- Certificate regarding the payment of local taxes and taxes issued by the City Hall (Local Council). The economic operator (leader, associate, third party, subcontractor) will fill in the appropriate requirement in the DUAE form. Updated supporting documents (tax certificates) demonstrating the fulfillment of the qualification criteria, in accordance withwith the information contained in the DUAE, which must attest the lack of overdue debts at the time of their deposit. The documentary evidence that proves the fulfillment of the assumed by completing the DUAE is to be presented, at the request of the contracting authority, only by the bidder ranked first in the intermediate rankings drawn up at-completion of the evaluation of the offers,

- The criminal record of the economic operator and of the members of the management, management or supervisory body of the respective economic operator or of those who have the power of representation, decision or control within him, as it results from the certificate founder issued by the ONRC / constitutive act. The economic operator (leader, associate, tershadow, subcontractor) will fill in the appropriate requirement in the DUAE form. The supporting documents proving the fulfillment of the undertaken by completing the DUAE are to be presented, at the request of the contracting authority, only by the bidder ranked first in the intermediate rankings drawn up at the completion of the tender evaluation.Contracting entities, regarding the organization, carrying out and finalization of the award procedure are:
