Medical equipments.

1. The subject of this order includes the supply of medical equipment and supplies, delivery to the Employer's headquarters, assembly, employee training, and service. The subject of the order includes: - Package No. 1 - KTG with telemetry 1 item. - Package No. 2 - Delivery bed 3 pcs. - Package No. 3 - Gynecological chair-2 pcs. Package No. 4 - Video laryngoscope for difficult intubation 1 pc.

1) Delivery and assembly of medical equipment described in Annexes 5, 6, 7, 8 to the ToR,

2) Launching the installed equipment;

3) Practical training and theoretical doctors and other users indicated by the Employer in the field of camera operation and testing techniques.

4) Performing service work, reviewperiodic, ongoing maintenance in accordance with the manufacturer's recommendations, making repairs ensuring efficient and safe operation for the indicated period from the moment of signing the delivery-acceptance protocol for the performance of the order.

1. The Contractor is obliged to pay a deposit in the amount of: - Package I PLN 673.00 - Package II PLN 2, 059.00 PLN - Package III PLN 493.00 - Package IV PLN 110.00 - PLN 2.The bid bond can be paid in one or more of the following forms:

1) money,

2) bank guarantees or social-savings bank guarantees, however, the surety is always a cash guarantee,

3) bank guarantees,

4) insurance guarantees,

5) sureties granted by entities referred to in art. 6b par. 5 points 2 of the Act of November 9, 2000. on the establishment of the Polish Agency for Enterprise Development

3. The bid security should be submitted before the deadline for submitting bids.

According to the Specification of Essential Terms of Orders and the specimen of the contract - Annex to the ToR.

1) are not subject to exclusion from the proceedings pursuant to art. 24 paragraph 1 items 12 - 23 and art. 24 sec. 5 point 1 of the Public Procurement Law,

2) meet the requirements specified by the Employer in the followingscope:

2. The Awarding Entity will assess whether the Contractor is not subject to exclusion and meets the conditions for participation in the procedure based on a statement submitted together with the offer in the form of a standard form of a single European order document, hereinafter referred to as a single document prepared in accordance with the model constituting Annex 3 to the ToR. The contracting party informe that a single document can be downloaded from the https://www.uzp.gov.pl/aktualnosci/komunikat-dotyczacystowania-jednolitego-europejskiego-zokument-zamowienia and fill in places indicated by the Ordering Party in accordance with Annex 3.

3. The information contained in the uniform document is a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedingsu.

1. In order to demonstrate the absence of grounds for exclusion of the Contractor in the circumstances referred to in art. 24 sec. 1 of the Public Procurement Law, the Employer requests the following documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids.

2) certificate of the competent head of the officeconfirming that the Contractor is not in arrears with paying taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest - or fines, in peculiarities obtained the legally required exemption, deferral ofor spreading overdue payments into installments or suspending the entire authority from the execution of the decision.

3) certificate of the relevant branch of the Social Security or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions than 3 months before the lapsethe deadline for submission of bids or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of such receivables, including possible interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in full of the decision of the competent authority .

4) statements by the contractor:

a) o-failure to issue a final court judgment or final administrative decision against the payment of taxes, fees or social or health insurance premiums or - in the event of issuing such a judgment or decision - b) documents confirming payment of such claims together with any interest or fines, or conclusion of a binding agreement in the repayment of these-receivables, - c) lack of a ruling against him as a preventive measure and prohibition of applying for a public order,

d) not paying taxes and local fees, referred to in the Act of 12.1.1991 on taxes and local fees (Dz. U. of 2016 item 716)

5) Contractor's declaration of belonging or non-affiliation to the same capital group

2) for the purpose ofthat the offered deliveries meet the requirements specified by the Ordering Party, the Ordering Party requests a statement that the offered subject of the order is admitted to trading in Poland, in accordance with the Act of 20.5.2010 on medical devices (OJ of 2015, item. 876), constitutes a medical device within the meaning of the aforementioned Act (if applicable) and for task No. 4 technical cards / catalog, CerCE certificate / Declaration of Conformity. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to above, he shall submit documents in accordance with Chapter. IX B SIWZ.
