The subject of the orders is design and implementation in the formula "design, build and equip" reconstruction and renovation of the Operating Block and Delivery Block in order to adapt them to the applicable regulations together with the reconstruction and renovation of the existing technical and energy infrastructure together with the equipmenttechnological up.

Main Site: Independent Public Health Care Center, ul. Szpitalna 3, 88-200 Radziejow, POLAND. The subject of the order is the reconstruction and renovation of the Operational Block and the Delivery Block in order to adapt them to the applicable regulations together with the reconstruction and renovation of the existing technical and energy infrastructure, based on the functional and functional programAnnex No. 12 to the Terms of Reference and the technical specification of technological equipment constituting Annex 3 to the Terms of Reference, including: 1) medical gas installations (compressed air, vacuum, oxygen) as required, 2) hot and cold water installations together with internal ones hydrant installations, 3) internal power installations located in the SPZOZ buildingRadziej-w with the implementation of necessary electrical cable connections, 4) internal central heating installation, 5) sanitary sewage system, 6) gravity ventilation installation, 7) ventilation and air conditioning installations, 8) computer network installation and adaptation to the newly designed, 9) vertical all installations. When designing and implementing the above these installationsThe Contractor must take into account all works outside the scope of the functional and functional program (PFU), necessary for the proper functioning of the hospital after reconstruction. Planned works include the execution of a construction project of the Operational Block (BO) on the basis of the attached PFU concept, execution of a construction design of the Delivery Batch (BP) on the basis of the technol projectThe company BP is Annex No. 13 to the SIWZ possessed by the Employer and construction works related to the reconstruction and renovation of BO and BP with their equipment. The order in the scope of the implementation of the project documentation includes in particular: 1) preparation of a multi-branch concept of reconstruction of BO in reference to the PFU concept, 2) preparation of knowledgea lobular construction project with the necessary arrangements, 3) performance of all assessments, expert opinions (including technical condition assessment and fire protection expertise for the whole hospital building), all necessary opinions, agreements, consents and permits necessary for-implementation of construction works and handover of the facility for use, 5) preparation of executive designs, 6) preparation of technical specifications, bonuses and cost estimates, 7) performance of energy (electric / cooling / thermal) balance, water, sewage, air for mechanical ventilation and air conditioning for the entire hospital building. At each stage of the above mentioned activitiesThe Contractor is obliged to make arrangements with the Ordering Party. The Contractor will obtain all the agreements, opinions, documents and administrative decisions required in accordance with Polish law, necessary for the design, development, commissioning and commissioning. The subject of the orders includes delivery, installation, commissioning of medical devices, medical and sanitary equipment, medical furniturein accordance with Annex 3 to the ToR and Annexes of minimum technical parameters in accordance with Annexes 3.1 to 3.17 to the ToR. Before submitting the offer, the Contractor may visit the construction site and its surroundings in order to assess, at his own risk, cost and risk, all the factors necessary to prepare his reliable offer, including all necessary workpreparatory, basic and accompanying both to conduct construction and assembly works as well as to prepare the Project for obtaining a building permit. The Contractor shall train the Employer's personnel, perform equipment commissioning, exploitation tests and trial operation, in accordance with the Employer's requirements specified in PFU.

1. A Contractor who can apply for the award of a public ordery is not subject to exclusion from the proceedings due to circumstances mentioned in art. 24 sec. 1 item 12-23 and in art. 24 sec. 5 point 1 PzP.

2. In the case of Contractors jointly applying for the award of a contract, the fulfillment of the requirement regarding the lack of grounds for exclusion referred to in Chapter XIV.3 of the SETC should be. demonstrated by each of the Contractors jointly applying for the order.

3. List of declarations submitted for the preliminary confirmation that the Contractor is not subject to exclusion:

1) The Contractor shall send in electronic form with a qualified electronic signature valid as of the day of submitting bids - a declaration of non-subject to exclusion submitted in the form of a Single European Order Document (EAT). Szczegoły - chapter XIV SIWZ.

2) In the case of the jointapplying for orders by contractors, EAT shall be submitted by each of the contractors jointly applying for the order. These statements confirm the lack of grounds for exclusion to the extent that each of the contractors shows no grounds for exclusion.

3. The ordering party, prior to the order being awarded, will call the Contractor whose offer has been the highest rated, to submit in the designated, not shorter than 10 days, valid for the date of submission of statements or documents referred to in Chapter XIV SIWZ, confirming the circumstances referred to in art. 25 ust. 1 Pzp.

4. List of documents and statements submitted to confirm the circumstances referred to in art. 25 ust. 1 point 3 PPL does not contain grounds for exclusion. The ordering party, before granting the order, will call the Contractor whose offer has been onqualified to submit:

1) Information from the NQF in the scope specified in art. 24 sec. 1 item 13, 14 and 21 Pzp.

2) Certificates of the competent head of the US - detailed contains ch. XIV.

3) Certificates of the relevant ZUS organizational unit or the Agricultural Social Insurance Fund - detailed in ch. XIV.

4) Copy from the relevant register or from CEIDG.

5) Contractor's statements about the lack of issuing to him the valid oneon the verdict of the court or the final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming payment of these receivables together with any interest or fines or conclusion of a binding agreement regarding repayment of such receivables - content proposaland statements in the above scope constitute Annex No. 9 to the ToR.

6) Contractor's statements about the lack of a ruling against him as a precautionary measure to prohibit applying for public orders - proposal for the content of the statement in the above-mentioned scope is attached as Appendix 10 to the ToR.

7) Contractor's statements about not paying taxes and local fees, referred to in the Act of-12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) - the proposal for the content of the statement in the above-mentioned scope is attached as Appendix 10 to the ToR.

6. If the contractor has a registered office or place of residence outside the territory of the Republic of Poland, instead of the documents from ch. XIV, para. 2 SIWZ, consists of doc. from ch. XIV, para. 2.1 SIWZ.

8. Within 3 days-from the date on which the Ordering Party publishes information on the opening of tenders, the Contractor is obliged to provide a declaration of belonging or non-affiliation to the same capital group. The contractor must prove that he is insured againstcivil liability in the scope of conducted activities related to the subject of the order for a guarantee sum of at least PLN 1, 000, 000 (one million PLN).

1. Contractors joining the tender are obliged to pay a deposit of PLN 20, 000.00 in the following forms: in cash, bank guarantees or social-savings credit sureties, except that the surety of the cash register isa financial guarantee, bank guarantees, insurance guarantees or in the form of sureties granted by entities referred to in art. 6 b paragraph 5 points 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development.

2. A surety or guarantee must contain a commitment in accordance with art. 46 ust. 4a and para. 5 points 1 and 3 PPL. 4 The bid bond should be submitted before the deadline for submitting bids, whereby the payment of the bid bond in cash will be considered effective at the moment of crediting the bill of the ordering. the offer must be accompanied by a document confirming the deposit payment (does not apply to the form in cash).

6. In the case of payment of a deposit in another foIt is recommended that the original document should be attached to the offer in a separate envelope and a photocopy of the document, certified with the original by the Contractor, should be permanently attached to the offer. The ordering party paying back the deposit in the mode of art. 46 PZP will return the original document to the bid bond. In the tender documentation, a photocopy of the returned security deposit will be certified as true copy.

7. The bid bondshould be an attachment to the offer.

8. According to art. 22 a para. 1 PPL, the contractor may, in order to confirm compliance with the conditions of participation in the proceedings, in appropriate situations and in relation to a specific order or part thereof, rely on technical or professional capacities or financial or economic situation of other entities, regardless of the legal nature of connecting it with himlegal relationships.

9. The Contractor who relies on the abilities or situation of other entities must prove to the Employer that while performing the order he will have the necessary resources of these entities, in particular by presenting the obligation of these entities to give him the necessary resources to fulfill the order.

10. Contractors may jointly apply for a grant forpublic speaking on the terms set out in art. 23 PzP.

11. Contractors jointly applying for the award of the contract shall be obliged to appoint a representative to represent them in the proceedings for the award or representation in the proceedings and conclusion of the public procurement contract.
