1. The subject of the order is the delivery of disposable medical equipment for renal dialysis, hereinafter referred to as equipment. 2. The quantities and type of ordered equipment are described in the description of the subject of the order for each part of the proceedings separately. Order description-Annexes 1 and 2 to SIWZ. 3. The offer must include a full scope, consistent with the description of the subject of the order for the proceedings in question. Offer of equipment that does not meet the required parameters or lack of information required in the assortment and price form, as well as the lack of the above. The form for the part where the offer is submitted (attachments 1 and / or 2 to SIWZ) will resultto reject the offer. 3. The Ordering Party accepts the possibility of submitting partial tenders in the present proceedings, as follows: 1) Package No. 1 Diasafe plus hemodialysis filters, 2) Pack No. 2 Y extender for one-dialysis

1. The contractor may apply for the award of the contract, who: 1) are not excluded from the proceedings due to the circumstances mentioned in art. Article 24 1 pt. 12-23 of the Public Procurement Law (obligatory grounds for exclusion); 2) meet the conditions of participation in the proceedings, if they were odrafted by the contracting authority in the contract notice and the SIWZ. The contracting authority does not specify the conditions for participation in these proceedings. 2. Contractors may jointly apply for an order. 1) Contractors jointly applying for a contracting authority shall establish a representative to represent them in the proceedings for the award of or representation in proceedings and the conclusion of an agreement in thea public order. (2) The provisions relating to the contractor shall apply mutatis mutandis to performers seeking jointly the award of the contract. 3) If the offer of contractors jointly applying for the contract is selected, the contracting authority will demand prior to the conclusion of the agreement on public procurement, the contract regulating the cooperation of these contractors. 3. In order to pre-flushThe absence of grounds for exclusion of the contractor from participation in the tender, the contractor attaches the current: statement on non-suspension of the submission in the form of a Single European Procurement Document (hereinafter "JEDZ" or "unitary document"). A single document constitutes Attachment 4 to SIWZ. In the case of joint application for a contractor's contract, a single document is requiredwill each of the contractors jointly apply for the order. These statements confirm the fulfillment of conditions for participation in the procedure and no grounds for exclusion in the scope in which each of the contractors demonstrates meeting the conditions for participation in the proceedings and lack of grounds for exclusion. 4. In order to confirm the lack of grounds for exclusion of the contractor from participation in the procedure, the contractor shall submit an updateinformation from the National Criminal Register in the scope specified in art. Article 24 1 pt. 13, 14 and 21 of the Public Procurement Law issued not earlier than 6 months before the deadline for the submission of bids. In order to confirm by the contractor that the offered deliveries meet the requirements of the contracting authority, the contractor shall submit: 1) a description of the equipment offered in the form of a folder, technical sheet, catalog or leaflet. ORDERINGThe applicant requires that the above- Documents are prepared in the Polish language.
The ordering party is required to provide the original folder, technical sheet, catalog or leaflet, no handwritten notes, notes, etc. are allowed
2) free sample of offered equipment for package no. number of package and item number in package, the exact package must be given on the packageThe name and address of the contractor and the name of the procedure within which they are submitted with a note of "sample." Within the package containing the samples should be listed their exact list. The date on which the ordering party's posting of the information on the opening of the tenders in question is posted by the contracting authority-in art. 86 sec. 5 of the Public Procurement Law, the contractor is obliged to provide the contracting authority with a declaration of affiliation or non-affiliation with the same capital group referred to in art. Article 24 1 point 23 of the Public Procurement Law. With the submission of a statement, the contractor may provide evidence that links to another contractor do not lead to distortion of competition in the procurement procedure. FROMThe bidder requests the payment of the bid bond in the amount of: 1) in the scope of the package No. 1: 4 900 PLN
2) in the scope of package 2: 30 PLN. The contractor allows changes in the contract in the case of:
1) completion of the production / withdrawal of the offered equipment and commencement of production of equivalent equipment, ie with the same or better qualitative, physicochemical, operational and technical parameters-can not be used to increase the value of the contract, increase the unit price and these changes can not be disadvantageous to the ordering party, 2) change the catalog number of the equipment, 3) change the trade name of the equipment while preserving its parameters, 4) and services,
5) change of the minimum remuneration for work determined pursuant to art. 2 sec. 3-5-Act of 10 October 2002 on minimum wage for work,
6) changes in the rules of social insurance or health insurance or the rate of social security or health insurance premiums. If the changes referred to in subparagraphs 2 (4-6) above will affect the performance of the order object by the Contractor. An application for a change of contract is submitted to the Contracting AuthorityAt the same time, the Contractor shall submit to the Employer the justification for the change made and evidence supporting the effect of the above- changes to the costs of the Contractor in terms of performance of the subject matter of the contract. On this basis, the Contracting Party will assess the appropriateness of introducing a change in the agreement of the amount of the Contractor's remuneration for the performance of the subject matter of the contract and in this scope will present his position to the Contractor.
