Intraocular lenses.
1. The subject of the order is the supply of consumable accessories, compatible with the offered apparatus for phacoemulsification and front and rear vitrectomy with the lease of the device, delivery of intraocular lenses, intraocular preparations, dotorebkowych rings, heavy oil, fine-disposable equipment, microsurgery knives, reusable surgical instruments, iris hooks, viscoelastic sets, disposable operational drapes, according to the list of assortment and price, constituting Annex 2 to the ToR and required technical parameters for phacoemulsification apparatus and anterior vitrectomy and back, along with equipmentin Annex 3 to the ToR.
2. The Employer allows the submission of bids for parts: - Part 1 - delivery of consumable accessories compatible with the offered apparatus for phacoemulsification and front and posterior vitrectomy with the device lease Part 2 - delivery of intraocular one-piece and three-part IOLs Part No. 3 delivery of intraocular lenses-# 4 delivery of intraocular preparations
Lot No. 5 delivery of retortable rings in disposable applicator
Part No. 6 Heavy oil delivery
Lot No. 7 Delivery of small disposable equipment for ophthalmology
Part No. 8 Delivery of knives for eye microsurgery
Part No. 9 delivery of disposable vitrectomy instruments
Part No. 10 Delivery of silicone wristbands
Czpart no. 11 delivery of reusable surgical instruments
Part No. 12 supply of hooks
Part No. 13 Delivery of pupil dilators
Part No. 14 Delivery of viscoelastic kits (chondroitinate, hyaluronate)
Part No. 15 delivery of disposable operational drills
Additional requirements:
The Contractor is obliged to create a deposit within 7 daysor commission maintenance of all required sizes of lenses in part 2 and 3 in a quantity of 5 from dioptres from 20 to 26 and in quantities of 2 from dioptres under 20 and over 26 for the duration of the contract, in accordance with - 10 of the contract template -I.
Before the tender submission deadline, the Contractor is obliged to pay a bid bond in the amount of PLN 60, 810.00 (in words: sixty thousandeight hundred ten zlotys 00/100)
In the case of an offer for individual parts, a bid bond is in force in the amount of: - Part No. 1 - 28 300.00
