CPV Description: Periodical delivery of medical devices and consumables needed for the treatment of the patients of St. Petka Hospital, Vidin, in two separate positions - Subject of the present a public contract is divided into two distinct items: Medical devices and consumables to be deliveredare detailed in the technical specifications. According to Art. 30, para. 1 of the RPSP, the subject of each of the separate items includes nomenclature units. Participants may bid one, several or all nomenclature units included in the lot. The lack of an offer for delivery of any of the nomenclature units included in the lot for which a tender has been submitteda, there is no reason to remove it from participation in the procedure. The evaluation and ranking of the tenders of the participants is done by separate nomenclature units of the segregated position based on the award criterion "optimal price / quality ratio".

Main Site: Hospital Pharmacy and Medical Device Stock at Sveta Petka Hospital Vidin, Vidin, 1 Tsar Simeon Veliki Str19. Orthopedic implantable medical devices and consumables distributed in 5 groups of nomenclature units, as follows:

1. Group 1 "Total Hemostasis with Cementitious Fixation"; Group 2 "Total Hemostasis with Moore Unipolar Cementation; group № 3 "Total hip prosthesis with bipolar cement", group 4 "Medicine5 Group "Medical consumables osteosynthesis for upper limb"

The participants should hold a valid wholesale license for medical devices issued by the BDA in cases where they are traders. Such a document is not required when the participants are producers establishedand on the territory of the Republic of Bulgaria, according to Art. 77, para. In order to prove the compliance with the above requirement, the participant shall fill in the ECEDOP, Part IV "Selection Criteria", subsection "A: Fitness".

In order to prove the acquired professional capacity, the participant shall submit (at signing of the contract) the following document: a valid auctions permitwholesale of medical devices issued by BDA in the cases where they are traders. Such a document is not required when the participants are producers established on the territory of the Republic of Bulgaria, according to Art. 77, para. The Contracting Authority does not impose requirements for the economic and financial condition of the Participant and does not provide for the submission of any of the provisions of Art. 62 of the PPL documents for-proof of economic and financial standing.

The Contracting Authority shall necessarily remove from participation in a procedure for the award of a public procurement a participant, for when, before or during the procedure, the circumstances referred to in Art. 54, para. 1 of the Public Procurement Act. The Contracting Authority may remove from participation in a procedure for the award of a public procurement contract, when it has occurred before or onprocedure, the circumstances referred to in Art. 55, para. 1 of the Public Procurement Act - Other grounds for disqualification arising from national legislation:

Participants who are related parties within the meaning of - 1, items 13 and 14 of the additional provisions of the Public Offering of Securities Act, can not be independent participants in the same procedure. Companies registered in jurisdictions-with a preferential tax regime, and the persons controlled by them, it is forbidden to participate directly and / or indirectly in a public procurement procedure under the Public Procurement Act and the statutory instruments for its implementation, irrespective of the nature and value of the public procurement, including through a civil society / consortium in which a company registered in a preferential jurisdiction is registeredThe participant presents information, ie. declares the absence or presence of circumstances under Art. 54, para. 1 of the Public Procurement Act and under Art. 55, para. 1 of the Public Procurement Act, marking "yes" or "no" in the following parts of the EDEPD:

a) Part III, Section A "Grounds related to criminal convictions" - the participant should answer ", or with "yes", if it is, by providing information on convictions for the villagethe offenses provided for in Art. 54, para. 1, item 1 and item 2 of the PPL:

a.1) participation in a criminal organization - under art. 321 and 321a of the Criminal Code; a.2) corruption - under Art. 301-307 of the Criminal Code; a.3) fraud - under Art. 209-213 of the Criminal Code; a.4) terrorist offenses or crimes related to terrorist activities - under Art. 108a, para. 1 of the Criminal Code; a.5) money laundering or financing of terrorism - under Art. 253, 253a or-253b of the Criminal Code and under Art. 108a, para. 2 of the Criminal Code; a.6) child labor and other forms of trafficking in human beings - under Art. 192a or 159a-159d of the Criminal Code; (b) Part III, Section B, "Reasons for Paying Taxes or Social Security Contributions" - the participant shall respond by "no" if not sentenced or "yes" if is, by providing information about the circumstances under Article 54, paragraph 1, item 3 of the PPA related to a payment instrument(c) Part III, Section C "Reasons for Insolvency, Conflict of Interest or Violations" - the participant shall mark the answer "yes" or "no" by providing information on the circumstances under Article 54, paragraph 1, item 4-7 of the Public Procurement Act and Article 55, paragraph 1, item of the PPA, d) Part III, item D "Specific national grounds for exclusion" - the participant marksresponding "yes" or "no" by providing information on convictions for crimes under Art. 172, Art. 194-208, Art. 213a-217, Art. 219-252, Art. 254a-260 and Art. 352-353e of the Criminal Code, as well as for crimes analogous to those under Art. 54, para. 1, item 1 of the Public Procurement Act, in another Member State or a third party.

The participants are obliged to notify in writing the contracting authority within 3 days from the occurrence of a circumstance under Art. 54, para. 1 of the Public Procurement Act or from the knowledge that the participant is a related party with another participant in the same procedure.
