The subject matter of the contract is the delivery of automatic and semi-automatic devices including software equipmentand accessories under the rationalization and optimization of storage, tracking and movement of drugs and MSM projects at the central hospital and departmental depot, including the necessary building modifications associated with the delivery of equipment and a 12 month warranty service for the delivered equipment. The facilities are required by hospitals under the management of the public hospital Nemocnice akad. L. Derer, Sickca sv. Cyril and Methodius and Ruzinov Hospital. A detailed definition of the subject of the contract is given in Section B.1 Description of the Subject of the Contract and B.2 Commercial Terms of Delivery of the Subject of the Contract of Inquiry.

Facilities are required for hospitals in the administrationcontracting authority Nemocnica akad. L. Derera, Sv. Cyril and Methodius and Ruzinov Hospital. A detailed definition of the subject of the contract is given in Part B.1 Description of the Subject of the Contract and B.2 Commercial Terms and Conditions of Delivery of the Subject of the Contract of Inquiry. 1.2. 1 of the Public Procurement Act with the following documents:

a) pism. (a) documentedby issuing from the criminal record no more than three months after the expiry of the time limit for the submission of tenders. In connection with the acquisition of the Act no. 91/2016 Coll. on Criminal Liability of Legal Persons and on the Amendment and Addition of Certain Acts as of 1 July 2016, the Applicant shall also submit an extract from the Criminal Records Register of non-legal persons not older than 3 months; (b) substantiated by a health insurance certificatee and Social Insurance Agency not older than 3 months on expiry of the bid submission deadline,

c) pism. (c) evidenced by a local tax return not older than 3 months at the expiry of the time limit for the submission of tenders; (d) substantiated confirmation of the relevant keg not older than 3 months on the date of expiry of the time limit for the submission of tenders; (e) documented proof of entitlementsupply goods, carry out construction work or provide a service that is subject to the subject matter of the contract. (f) attested by an affidavit

If the applicant fails to submit the document referred to in point 1.2 above. font. a) is obliged to prove the conditions according to - 32 par. 1 pism. (a) a contract on public procurement, to provide the contracting authority with the data necessary to request the extract from the registerpenalties. If a group of suppliers submits an offer, it demonstrates that the conditions for participation in a public contract relating to a personal status within the meaning of Section 32 of the Public Procurement Act are met, for each group member separately. A member of a group shall only be entitled to supply the goods or to provide a service in relation to the part of the subject matter of the contract which they are to provide.

1.7. The contracting authority shall recognize equivalent registration or. Ta certificate of registration issued by the competent authority of another Member State to which the applicant proves compliance with the conditions of participation in the public procurement. The contracting authority also accepts other equivalent documents submitted by the tenderer. 1 pism. NB: In conjunction with the acquisition of the Act no. 91/2016 Coll. on Criminal Liability of Legal Persons and on Amendment and Addition of Certain Acts as of 1 July 2016; 1 pism. a) the provision of public procurement also proves this person by issuing a copy from the criminal record of a legal person not older than 3 months fromon the date of expiry of the time limit for the submission of tenders. The applicant may provisionally replace the documents proving the fulfillment of the conditions of participation determined by the public contracting entities with a single European document pursuant to Section 39 of the FTA. Information on the single European document is given in Section A.2 Demonstration of Compliance with Applicants' Conditions. All required documents and documents bailersubmit as a scan of originals or a certified copy.

3. The public procurer will be in communication with applicants or interested in proceeding in accordance with - 20 of the Public Procurement Act through the Josephine Communication Interface, this method of communication relates to any communication and filing between the public purchaser and the bidders. Subject to public procurementit is not a green public procurement, nor an innovation procurement, nor a procurement focusing on social aspects.
