Preparation and delivery of ready meals for patients of individual departments of the hospital

1. The subject of the order is the preparation and delivery of ready meals for patients of individual departments of the Voivodship Specialist Hospital named after St. Rafał in Czerwona Gorze. Detailed description of the orderof the document is included in Annex 5 of the ToR.

1. The subject of the order is the preparation and delivery of ready meals for patients of individual departments of the Voivodship Specialist Hospital named after St. Rafał in Czerwona Gorze. A detailed description of the subject of the contract is includedAnnex 5 of the ToR.

2. The scope of the contract includes:

2.1. 24-hour board for patients hospitalized at the Voivodship Specialist Hospital St. Rafał in Chęciny, street Czerwona Gora 10, by providing catering services in the field of preparation, based on the Contractor`s kitchen and delivery of ready meals at the designated time to the indicated places at the seat of the Ordering Partylying,

2.2. washing transport vessels (thermoses, thermo ports with replaceable cartridges and GN containers);

2.3. performance of the service in accordance with applicable laws and regulations in the field of nutrition, health and safety at work, as well as with other provisions related to the service provided, i.e. use and compliance in the production and distribution of Good hygiene, production and cater practicengowa and the principles of the HACCP system.

1. In order to pre-confirm that the Contractor is not subject to exclusion, the Contractor is obliged to attach to the offer a statement in the form of a single European order document (Annex 2 to the ToR).

2. The single European order document should be sent to the Employer in electronic form with a qualified electronic signature fromvia the Tendering Platform: https://czerwonagora.logintrade.net/rejestracja/ustawowe.html

3. In order to confirm that there are no grounds for excluding the Contractor from participation in the proceedings, the Contractor whose highest bid was submitted shall submit the following documents at the request of the Employer:

3.1. information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 mouthyou, issued not earlier than 6 months before the deadline for submitting bids or requests to participate in the procedure;

3.2. certificate of the competent head of the tax office confirming that the Contractor is not in arrears with taxes, issued not earlier than 3 months before the deadline for submitting bids or requests to participate in the proceedings, orb another document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority;

3.3 . certificate of the appropriate field unitorganization of the Social Insurance Institution or the Agricultural Social Insurance Fund (Kasa Rolniczego Ubezpieczenia Społecznego) or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or requests to participate in the proceedings, or another document confirming that Wykthe contractor has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferment or installment of overdue payments provided for by law or withholding in full the execution of the decision of the competent authority;

3.4. excerpt from the relevant register or from the Central Register and Information on Economic Activity, jif separate provisions require an entry in the register or records in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act.

4. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in item 3:

4.1. point 3.1 submits information from the relevant register or, in the absence of such a recordstru, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence, the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 points 13, 14 and 21;

4.2. points 3.2 to 3.4 submit a document or documents issued in the country in which the Contractor is based or hassce of residence, confirming accordingly that:

a. he is not in arrears with the payment of taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular he has obtained exemption, deferral or installment of overdue payments or withholding in totalimplementing the decision of the competent authority;

b. its liquidation was not opened or its bankruptcy was declared.

4. The Employer will assess the fulfillment of the conditions for participation in the procedure on the basis of the required documents and statements submitted by the Contractor in the offer and the information contained therein in accordance with the principle: meets / does not meet.

The Employer does not specify / set a specific condition in tin the range

1. Contractors who can apply for the contract:

1.1. are not subject to exclusion pursuant to art. 24 paragraph 1 point 12 23 of the PPL Act;

1.2. are not subject to exclusion pursuant to art. 24 paragraph 5 points 1 and 8 of the PPL Act

2. The procedure will be the procedure referred to in art. 24aa section 1 of the Public Procurement Law

3. The contractor is required to attach to the offer:

3.1. offer formhead;

3.2. EAT;

3.3. Current ISO 22000: 2006 or 22000: 2018 Certificate issued by an external certification body in order to evaluate the offer in the criterion of "Quality of service";

3.4. The Contractor`s statement confirming his experience in the field of round-the-clock feeding of patients of medicinal entities Annex 6 to the ToR, in to evaluate the offer in the "Experience" criterion;

3.5. commitment underthird litter (if applicable),

3.6. the documents listed in sections 3.1 to 3.5 should be submitted in electronic form with a qualified electronic signature via the Platform: https://czerwonagora.logintrade.net/rejestracja/ustawowe.html. In addition, the Contractor whose tender has been highest rated submits at the request of the Employer the following documents:

4.1. listzędzi, plant equipment or technical devices available to the Contractor to perform a public contract with information on the basis for the use of these resources, i.e. the Contractor must have at least one means of transport adapted to transport meals,

4.2. documents confirming the introduction of the HACCP system;

4.3. a document confirming the title of the kitchen or other documentt which gives the right to use the kitchen to prepare meals for min. 410 people a day (breakfast, lunch, dinner).

5.5.
