These proceedings are conducted onbased on art. 138n paragraph 1 in connection with art. 138g of the Public Procurement Law of 29.1.2004 (i.e. EUR 000 and the Regulations for the award of a public contract for social services and other special serviceslyes referred to in art. 138g paragraph 1 of the Act of 29.1.2004, Public Procurement Law.

This procedure is conducted in electronic form via the miniPortal https://miniportal.uzp.gov.pl/ referred to in Article 24aa of the PPL Act

The proceedings to which this document relateszone is the number: EZ.28.6.2020. Contractors are obliged to refer to the above-mentioned procedure number in all documents submitted to the contracting authority regarding this procedure.

1. List of statements and documents submitted together with the offer in electronic form:

a) offer form, Annex 1 to this document;

b) document confirming the security ofsecurity deposit (form of the document in accordance with the provisions of Chapter XV);

c) ISO 9001: 2015 certificate for kitchens where meals will be prepared for the customer (if applicable);

d) power of attorney (if required);

e) original commitment entity providing its resources for the needs of the contractor submitting the offer, if applicable;

e) each contractor must include a valid offer for the daysubmitting bids a statement to the extent indicated in Annex 1 to this document in electronic form. The information contained in the statement will constitute a preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings.for a contract. This statement is intended to confirm that the conditions for participation in the procedure have been met, no grounds for exclusion to the extent that each of the contractors demonstrates that they have met the conditions for participation in the procedure, grounds for exclusion and fulfillment to the extent that he refers to themthe resources of the conditions for participating in the proceedings are also submitted by the declaration referred to in chapter XIII. 1.e of this document regarding these entities.

2. Contractors who meet the conditions for participation in the opi procedure may apply for the contractdata in chapter XI of the document.

3. Contractors who are not subject to exclusion from the procedure may apply for the award of the contract. The contracting authority shall exclude from the procedure for awarding a public contract:
