5-year evaluation of hydrotechnical facilities on the area of the Krakow Catchment Board

Scope of development:

Inspection of technical condition should cover:

- body and ground of embankment,

- accompanying buildings: embankments, embankments,

- river or stream bed and protected area,

- condition of embankments and collapses.

- testing of the substrate and shaft body,

- perform calculations of filtration and stusability,

- assess the technical condition of hydrotechnical structures related to embankment.

1. Together with the offer, the Contractor submits his own statement in the form of a uniform European order document according to the standard form introduced by Commission Implementing Regulation (EU) 2016/7 of 5 January 2016. Infthe information contained in the statement constitutes a preliminary confirmation that the Contractor is not subject to exclusion from the procedure and meets the conditions for participation specified in the ToR. Within the scope of Part IV "JEDZ Qualification Criteria", the Contractor may limit himself to completing section α, in which case the Contractor shall not complete any of the other sections in Part IV EAT.

2. In the case of joint application foreach Contractor shall make a declaration referred to above.

3. In the event of reference to the resources of a third party, the Contractor shall make a statement regarding that entity.

4. The Employer informs that at http://ec.europa.eu/growth/espd the European Commission has made available a tool enabling the Employers and Contractors to create, complete and reuse a standardelectronic form of the uniform European procurement document. If the Contractor wants to use the option of creating the EAT / ESPD form himself, all fields of the form are active. They should be completed to the extent appropriate to the requirements specified by the Employer in the proceedings. For all exclusion grounds, the default answer isZac. After selecting the affirmative answer, the Contractor has the opportunity to provide details, as well as to describe any remedial measures taken under the so-called self-cleaning procedures. 1. The Contractor is obliged, before the deadline for submission of bids, to lodge a security deposit in the amount of:

- part: 1: 3 900.00 PLN (in words: three thousand nine hundred zlotys 00/100),

- part: 2: 900.00 PLN (in words : nine hundred zlotys 00/100).

1) Offersand must cover the entire subject of the contract for a given part of the contract and be prepared in accordance with the provisions of the ToR. The Employer recommends using the model forms attached to this ToR when making an offer.

2) The Contractor has the right to submit only one offer for a given part of the contract. Submission of more offers for a given part of the order or offer variantsej will reject all tenders submitted by a given Contractor for a given part of the order.

3) The offer should be made in Polish, in electronic form and signed with a qualified electronic signature. The method of submitting the offer, including encrypting the offer, is described in the regulations of using miniPortal. The offer should be submitted in the original. Purchasermakes the submission of a scan with a qualified electronic signature possible.

4) The Contractor submits the offer to the ePUAP mailbox: / pgwwp-kr / SkrytkaESP via the form for submitting, changing, withdrawing the offer or application available on ePUAP and also available on the miniPortal. The public key necessary for the Contractor to encrypt the offer is available to the Contractors on the miniPortal.In the offer form, the Contractor is required to provide the ePUAP mailbox address at which correspondence related to the procedure will be carried out.

5) The Contractor may, before the deadline for submitting offers, change or withdraw the offer via the form for submitting, changing, withdrawing the offer or application available on ePUAP and also made available on the miniPortal. The way to change and withdraw the offeryou have been described in the user manual available on the miniPortal.

6) Documents or statements referred to in the ToR, in particular regarding the fulfillment of the conditions for participation in the procedure or the lack of grounds for exclusion from the procedure or the fulfillment by the subject of the contract of the requirements specified by the Employer, submitted in the form of an electronic document.

7) Copies of documentsntow or statements referred to in item 6 should be submitted in the form of electronic copies of documents or statements certified as true copies.

8) Certifications as true copies are made by the Contractor, the entity on whose abilities or situation the Contractor, the Contractor together applying for a public contract or a Subcontractor in the scope of docsections or statements that affect each of them. Certification for compliance with the original electronic copy of the document or declaration is made using a qualified electronic signature.

9) The documents making up the offer must be signed by persons authorized to represent the Contractor or authorized by these persons to represent the Contractor on the basis of a separate full
