Przedmiotem zamowIt is necessary to modernize land and building registers, adapt the land and building registry database to conform with the conceptual model specified in the regulation on land and building registry and establish BDOT500 and the initial poviat GESUT database for record bow:

Main Site: Kolbusz-w poviat.

The subject of the order is the modernization of land and building register(EGIB), adaptation of the land and building registry database to compliance with the conceptual model specified in the regulation on land and building registry and establishment of the initial poviat GESUT and BDOT500 base for the risers Toporow, Jagodnik and Trzęs-wka in the commune of Cmolas and Kosowy, Siedlanka and Hucina in the Niwiska commune, located in the Kolbusz-w poviat, in the Podkarpackie voivodship.-The scope of works includes:

1) Adjustment of databases of land and building registers:

Registration boundaries: Toporow, Jagodnik and Trzęs-wka in the municipality of Cmolas.

Registration boundaries: Kosowy, Siedlanka and Hucina in the Niwiska commune.

Kolbusz-w poviat, Podkarpackie voivodship, compliance with the conceptual data model of land and building records, specified in Annex No. 1a to the Regulation of the Ministera Regional Development and Construction from 29.3.2001 on the land and building register (Journal of Laws of 2016, item 1034).

2) Establishment of the initial poviat GESUT base for the ropes: Toporow, Jagodnik and Trzęs-wka in to the municipalities of Cmolas and Kosowy, Siedlanka and Hucina in the Niwiska commune, Kolbusz-w poviat, Podkarpackie voivodship, consistent with the conceptual GESUT data model specified in the RegulationOf the Minister of Administration and Digitization of 21/10/2015 on the Poviat GESUT database and the national GESUT database (Journal of Laws of 2015, item 1938),

3) Establishment of BDOT500, consistent with the BDOT500 data model, specify In the ordinance of the Minister of Administration and Digitization of 2.11.2015 on the database of topographic objects and the master map (Journal of Laws of 2015 item 2028), forcorms: Toporow, Jagodnik and Trzęs-wka in the municipalities of Cmolas and Kos-w, Siedlanka and Hucina in the Niwiska commune, Kolbusz-w poviat, Podkarpackie voivodship

4) Introduction to the teleinformation system of the staroste created by the Contractor EGiB data collection, GESUT district database and BDOT500.

2 . The contracting authority requires the award of the contracted object to at least a 2-year period-3

The contracting authority requires that the activities undertaken by the Contractor or subcontractor related to the implementation of the order, ie: analysis of materials provided for processing the order by the Employer, conducting the determination of property boundaries, related activitiesWith the presentation of the EGiB modernization project, the introduction of data made in the procurement of the Employer's database was carried out by an employee / employee in an employment contract (Article 22 - 1 of the Act of 26.6.1974 Labor Code). is entitled, during the entire period of the order's execution, to verify the employment form of employees referred to in point 4-while doing it. During the performance of the contract, the Contractor, at the request of the Employer, will provide evidence confirming the fulfillment of the obligation referred to in point 4. The Contracting Authority is entitled to request it at a date not shorter than 5 working days, in particular: a) Contractor's or subcontractor's statement regarding employment persons under a contract of employment, or b) certified for compliance with originals. respectively, by the Contractor or sub-contractor a copy of the contract / employment contract for persons performing in the course of the action, the contract / contract should be anonymised in a manner that protects personal data. 4) For non-compliance by the Contractor or sub-contractor with the requirement referred to in point 4, the Employer provides for a sanction in the form of obthe Contractor's obligation to pay a contractual penalty in the amount of PLN 2, 000 for each failure to meet the requirements.

1.O Contractors who are not subject to exclusion in the circumstances referred to in art. 24 sec. 1 and art. 24 sec. 5 point 1 of the Public Procurement Law, meet the conditions for participation in the proceedings.

2. In order to confirm the lack of grounds for exclusion from participation in the proceedings.the speaker requests the following documents ::aa information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting offers or requests to participate in the proceedings;

b) excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separatee the provisions require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the PPL Act;

c) the Contractor's statement that he has not been the subject of a final court judgment or final administrative decision on late payment of taxes, fees or social security or health insurance premiums;or (d) Declaration of the Contractor about the lack of a decision against him as a precautionary measure prohibiting him from applying for public orders. e) declaration of affiliation or not belonging to the same groupsy capital (submitted in term of 3 days from posting on the website information about ktmowa in Article 86 paragraph 5 of the Public Procurement Law.)

3.If Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of documents, referred to in:

1) section 2) lit a). submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent authoritya judicial or administrative body of the country in which the Contractor has his registered office or place of residence or place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 sec. 1 item 13, 14 and 21.

4. Documents referred to in paragraph 3 should be issued not earlier than 6 months before the deadline for submitting offers or applications for admission to participate in the procedure. If in the country in which the Contractor has his registered office or place of residence or place of residence has the person concerned, the documents referred to in paragraph 3 shall not be issued, substituteit is a document containing the Contractor's declaration, indicating the person or persons authorized to represent it, or a statement of the person the document was to be submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority with regard to the seat or place residence of the Contractor or place of residenceania of this person. Record of paragraphs 4 shall apply. 6. The contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which is referred to in the document referred to in para. 2) lit. a), submits the document referred to in section 3 point 1) to the extent specified in art. 24 sec. 1 points 14 and 21. If in the country in which the place is inhabitedand has a person to whom the document was to relate, such documents are not issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-government or economic authority competent for the place of residence of that person. The document should be issued not earlier than 6 m before the deadline splacing offers. Important: continued conditions are included in section III.1.2)

1. The Contracting Authority may annul the procedure for awarding the contract if the funds come from the European Union budget and non-recoverable funds from the assistance provided by the Member States of the European Free Trade Agreement (EFTA), which the Employer intended to allocate to finance all or part of-the orders were not granted to him.

2. The bid security for each part of the order should be submitted before the deadline for the submission of offers, it can be submitted in one or several of the following forms, referred to in art. 45 par. 6 PPL:

e) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of November 9, 2000 on the establishment of the Polish Agency for Enterprise Development.

3. Under the offer binding period, two months should be understood as 60 days.

4. Offershould be prepared in accordance with the Model Offer form (Annex 2 to the Terms of Reference) .In order to demonstrate the lack of grounds for exclusion from the procurement procedure and fulfillment of the conditions for participation in the procedure, the offer must be accompanied by a declaration on the Uniform Form of the European Contractor's Order (EJC) ) (Annex 3 to the ToR), and if it also applies to EAT: Youthe Contractor jointly applying for the order and the entity on the capacity or situation is the Contractor. Power of attorney, if applicable. If the Contractor, demonstrating compliance with the conditions regarding technical or professional capacity of other entities under the principles set out in Article. 22a of the Public Procurement Law, it must prove to the Employer that it will have the necessary equipment when executing the orderresources of these entities, in particular by presenting the obligation of these entities to give them the necessary resources to perform the order.

5. Declarations and documents must meet the requirements set out in the Public Procurement Law and the provisions of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that may be requestedfrom the Contractor in the procurement procedure (Journal of Laws of 2016, item 1126 and with reference to the Single Document, requirements specified in the Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 laying down a standard form for the Single European Of the Order Document (Journal of Laws L 3/16 of 6.1.2016) ..

6 It is not envisaged: to award contracts referred to in Article 67section 1 point 6, 7 of the Public Procurement Law, conclusion of a framework agreement, establishment of a dynamic purchasing system, selection of an offer using an electronic auction.
