1. The subject of the order is the delivery of computer equipment with high computing power and data storage capacity with power supplies as part of the operation of the laser microdissection.

1. In order to confirm the fulfillment by the Contractor of the conditions of participation in the procedure and the lack of grounds for exclusion along with the offer, please submit:

1) a statement valid in the Uniform European form valid as at the day of submitting bidsego of the Order Document (hereinafter referred to as "EAT"), prepared in accordance with the specimen contained in Annex 2 to the ToR. The information contained in the statement will constitute preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings. A single document (EAT) is drawn up, under pain of nullity, in electronic form and is provided with a qualified signature of thetronic. The contractor who refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion against them, also submits uniform documents concerning these subjects on the EAT form, which must take the form of an electronic document signed with a qualified electronic signature by each of them to the extent that each with Contractors shows no grounds for exclusionpose. An analogous requirement applies to EAT submitted in the case of joint application by contractors. These documents confirm the lack of grounds for exclusion and fulfill the conditions for participation in the proceedings.

2. In the event of a joint application for contracting by the Contractors, the statement referred to in paragraph 1 point 1) above, is submitted by each of the contractors jointly applying for the order. ABOUTthis service is to confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of attitudes of exclusion. If the Contractor refers to the resources of other entities, in order to demonstrate the existence of grounds of exclusion and fulfillment to them, to the extentthey are punished for their resources, the conditions for participation in the proceedings are submitted by the statement referred to in Chapter V, para. 1 point 1) of this chapter regarding these entities.

4. The ordering party, prior to the order being awarded, in the scope of demonstrating the fulfillment by the contractor of conditions of participation in the proceedings and the lack of grounds for exclusion, shall call the contractor whose offer has been the highest rated, to submit inmarked, not shorter than 10 days, current time as of the day of submitting the following statements or documents:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

2) excerpt from the relevant register or from central records and information about business activity, if separate provisions require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 of the Public Procurement Law Act,

3) a statement of the contractor about the lack of a final court judgment or final administrative decision against him being in default of taxes, fees or social security or health insurance premiums, or in the case ofand such a decision or decision on documents confirming payment of these receivables together with possible interest or fines or conclusion of a binding agreement on repayment of such receivables;

4) declaration of the contractor about the lack of a ruling against him as a precautionary measure on the prohibition of applying for public orders. The Awarding Entity may first evaluate the offers, followed byand whether the contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the proceedings (Article 24aa paragraph 1 of the Public Procurement Law).

6. The Ordering Party demands from the Contractors to submit a bid bond in the amount of PLN 1, 000.
