Predicted andThe amount of km within the term of the implementation contract is 333, 008 km. • Contractors who are not subject to exclusion pursuant to art. 24 sec. 1 and art. 24 sec. 5 point 1 of the Public Procurement Law. In order to demonstrate the absence of grounds for exclusion, the Contractor will be required to submit the following declarations and documents: a) information from the National Rejescriminal trial in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the PPL, issued not earlier than 6 months before the deadline for submission of bids,

b) excerpt from the relevant register or from central records and business information, if separate regulations require entry in the register or records in order to confirmation of the lack of grounds for exclusion under Art. 24 sec. 5 point 1Pzp,

c) the Contractor's statement that he has not been the subject of a final court judgment or final administrative decision on defaulting taxes, fees or social security or health insurance premiums, or in the event of a verdict or decision confirming the payment of such receivables, including any interest or fines or conclusiona) a declaration of the Contractor about the lack of a decision against him as a precautionary measure prohibiting the application for public orders, - e) within 3 days from the date of posting on the Platform information referred to in Article. 86 par. 5 Pzp, shall submit to the Ordering Party via the Platform, a declaration of belonging or non-affiliation to thisthe capital group referred to in art. 24 sec. 1 point 23 of the PPL (drawn up with the use of the template constituting Annex 3 to the ToR). With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. In the event of a joint application by the Contractors, the above-mentioned Statementse submits each Contractor. f) current statement as at the day of submitting bids, constituting preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings. The Contractor submits this declaration in the form of a Single European Order Document (EIN) to the extent specified by the Contracting Authority in the ToR in electronic form, bearing the qualificationelectronic signature verified by a valid qualified certificate or equivalent means meeting the requirements for this type of signature. Contractors who meet the conditions for participation in proceedings related to competences or entitlements to conduct a given professional activity can prove the award of the contract, i.e. they show that they have valid permitse to pursue the occupation of a road carrier in the field of domestic carriage of goods issued in the form of an administrative decision based on the provisions of the Act of 6.9.2001 on road transport (unified text: Journal of Laws of 2017, item 2200, as amended).

1. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in point III.1.1) 1 of this announcement: - 1) lit. a) submit information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence is the person concerned by the information or document, in the scope specified in art 24 sec. 1 points 13, 14 and 21PPL Acts,

2) lit. b) submits a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming respectively that his liquidation has not been opened or that bankruptcy has been declared. Documents referred to in point 1 item 1 and item 2 of this chapter should be issued not earlier than 6 months before the deadline for submission of tenders. If in kparadise in which the Contractor has its registered office or place of residence is the person concerned, documents are not issued referred to in point 1 of this chapter, are replaced by a document containing a statement from the Contractor, indicating the person or persons entitled to his representation, or the statement of the person to whom the document was to be submitted, submittedbefore a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the place of residence or place of residence of the contractor or place of residence of that person. The provision of point 2 applies.

4. The document referred to in point III. 1) .1) .1.f) of this announcement shall be submitted in electronic form, with a qualified electronic signaturem verified by means of a valid qualified certificate or equivalent measure that meets the requirements for this type of signature in accordance with the principles set out in the ToR.

5. The document referred to in point III. 1) .1) .1.e) of this announcement, shall be submitted within 3 days from the date of posting on the Platform information referred to in art. 86 par. 5 Pzp.

6. Ordering party before granting the order- the contractor, whose offer has been rated the highest, to submit within the prescribed, not shorter than 10 days, the current date as of the day of submitting statements or documents referred to in point III. 1) .1) .1.a) III. 1) .1) .1.d) and III.1.1) 2 of this announcement. Note: The Contractor may confine himself to completing only section α of Part IV of the EED form (general statement oncompliance with the conditions for participation in the proceedings). Appropriate (evidence-based) verification of the fulfillment of specific conditions of participation in the procedure (selection criteria), the awarding entity will do only in relation to the contractor whose offer was rated the highest (in accordance with point II.2.5) of this announcement).
