The economic operator should submit the following documents to prove the personal situation.
— a statement that no final court judgment has been pronounced in the last 5 years for a criminal offense under Article 88 paragraph 1 of the PPL,
— confirmation that no bankruptcy procedure has been initiated by a competent authority,
— confirmation that no liquidation procedure has been opened by a competent authority,
— confirmation of paid taxes, contributions and other public duties by a competent authority of the country where the economic operator is registered,
— confirmation from the register of penalties for the criminal offenses of the legal entities that no secondary penalty has been pronounced forbidding to participate in public procurement procedures, awarding public procurement contracts and public-private partnership contracts,
— confirmation from the register of penalties for the criminal offenses of the legal entities that no temporary or permanent prohibition on performing certain activity has been imposed,
— certificate that no final sanction has been pronounced for him/her — a ban on performing a profession, activity or duty, or a temporary ban on performing a particular activity.
Ability to perform professional activity of economic operator: document of registered activity, as evidence that it is registered as a natural or legal person for performing the activity related to the subject of the public procurement contract or evidence that it belongs to an appropriate professional association in accordance with the regulations of the country where what is registered.
