Wine contractorien show that he has a valid license to conduct insurance business issued by the supervisory body in accordance with the Act of 11.9.2015 on insurance and reinsurance activity (Dz.U.2018.999 jt) or other document confirming the possibility of conducting insurance business in Poland in not less than what is apparent from the subject of this order.

1. Contractors who can not be excluded under the provisions of art. 24 sec. 1 item 12-23 and art. 24 sec. 5 points 1 and 8 of the PPL Act

A / Information from the National Criminal Register, to the extent specified in art. 24 sec. 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the expiry of the terminu submitting offers;

B / Certificate of the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayments these receivables with possible interest or a finein particular, he obtained the legally required exemption, postponement or installment payment of overdue payments or suspension in full of the decision of the competent authority;

C / Certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying-in storagek for social or health insurance, issued not earlier than 3 months before the deadline for submission of bids or other document confirming that the Contractor has concluded an agreement with the competent authority regarding repayment of these receivables with possible interest or fines, in particular obtained the legally required exemption, postponement or spreading overdue payments into installments-or denying in full the implementation of the decision of the competent authority;

D / An excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion based on Art. 24 sec. 5 point 1 of the Act;

E / Contractor's statement on the lack of issuing a final court judgment against him or hera final administrative decision on defaults in payment of taxes, social security or health insurance premiums or, in the event of such a judgment or decision, documents confirming the payment of such claims together with any interest or fines or a binding agreement on repayment of such claims; F / Contractor's statement about the lack orzto him as a preventive measure of a prohibition against applying for public orders;

G / Contractor's statement on non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716);

H / current authorization to conduct insurance activity issued by the supervisory body in accordance with the Act ofia 11.9.2015 on insurance and reinsurance activities (Dz.U.2018.999 j.t) or other document confirming the possibility of conducting insurance activity in Poland in the scope not smaller than it results from the subject of this order.

3. Foreign contractors - If the contractor has a registered office or place of residence outside the territory of the Republic of Poland, instead of a documentaryentow referred to in point 2 (a) a-d) submit respectively the documents indicated in point VI.5.1 of the ToR.

The Ordering Party does not describe, does not set a specific condition in this respect.

2 / The Employer requests that the Contractor submits with the offer:

A / current as of the date of submission of offers, a declaration of not being subject to exclusion from the proceedings on the uniform European Document Formof the Order, also referred to as "EAT" or "single document", the editable template of which constitutes Appendix 2 to the ToR. The information contained in the statement on the single document form is preliminary confirmation that the contractor is not subject to exclusion from the procedure.

In part IV EAT: Qualification criteria The contractor must complete the section and the general statement regarding allh the eligibility criteria and does not have to fulfill any of the other sections in part IV. - A statement on a single document form, made in order to be invalid in electronic form and bearing a qualified electronic signature, must be submitted via the purchasing platform. The electronic communication platform used for submitting EAT by the Contractor is an on-line platformat the address http://e-ProPublico.pl/ (hereinafter referred to as: Platform). The use of the platform is free.

The Employer has also created the ESPD file (eJSD), which is posted on the website: http://www.dzp.agh.edu.pl/ under the call for tender reference number KC-zp.272-460 / 18.

In the case of joint application for contractors, EED made out in an electronic form and signed with qualified-electronic signature by persons authorized to represent a given contractor, submits each of the contractors jointly applying for the order, using means of electronic communication, i.e. by loading the files via the Platform at http: //www.e-ProPublico.pl;

B / The Bid Form prepared and completed in accordance with the template constituting Annex No. 1 to the ToR;

C / OWU of individual insurance lines in the tasks for which the contractor submits an offer;

D / Authorization - power of attorney to sign the offer, unless it results from other documents attached
