Modernization of street lighting in the commune of Kazimierza Wielka to increase its efficiency

Reference Number: II.271.7.2019

The subject of the order is: Modernization of lighting street in the area of the Kazimierza Wielka commune in accordance with the requirements specified by the Orderwith the annexes and the Contractor's offer.

The subject of the order is the performance of the task named:

"Modernization of street lighting in the commune of Kazimierza Wielka in order to increase its efficiency." The subject of the order is: Modernization of lighting street in the commune of Kazimierza Wielka in accordance with the requirements ofThe subject of the order is the delivery of new energy-efficient street lighting fittings in LED technology in the amount of 2321 pieces with accessories and installation materials, including replacement by dismantling existing elements and assembly in their place and meadditionally designated materials supplied, construction of 8 lighthouses supplied from renewable energy sources (RES) solar-wind, installation of 70 pieces of new street lighting control systems, along with elevation of measurement systems outside transformer station cabinets and replacement of uninsulated overhead power cable supplying street lighting for conductors insulated two-corein accordance with the Project Documentation attached to the Terms of Reference for the task: "Modernization of the street lighting system in the Kazimierza Wielka commune in order to increase its efficiency".

Total installed installed luminaires can not be higher than 235 kW and the sum of electricity consumption for the modernized range can not be greater than 801 103.55 kWh in a period of 12 miessence and operation of the lighting system for 4150 hours.

For confirmation, please submit: statement in accordance with the assumptions in point 9.3.

The ordering party does not set any requirements.

The confirmation should be submitted:

a) information from the bank or Cooperative Savings and Credit Union confirming the amount of financial resources held or the creditworthiness of the Contractor during the periodnot earlier than one month before the deadline for submission of tenders, the Contractor shall confirm the fulfillment of the condition if it proves that he has not less than PLN 600, 000.00 or creditworthiness in the same amount. b) Document confirming that the contractor is insured against civil liability in the scope conducted activity related to the subject of the order for the guarantee sum bydeleted by the Ordering Party - for an amount not less than

PLN 2 000 000.00.

The proceedings are conducted based on the provisions of art. 24 aa paragraph 1 of the Act, the Awarding Entity will first evaluate the offers and then examine whether the contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the proceedings.ia lack of grounds for exclusion from the procedure for awarding the contract, the contractor will submit a statement on the absence of grounds for exclusion (uniform document) and submit to the ordering party's request the following documents and statements - a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 item 13, 14 and 21 of the Act;

b) a certificate from the relevant tax office confirming thatthe contractor is not in arrears with the payment of taxes, issued not earlier than 3 months before the deadline for submitting offers;

c) a certificate of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions no earlier than 3 months before the end of termn) submitting offers;

d) a copy from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations require an entry in the register or records, in order to demonstrate the lack of grounds for exclusion pursuant to art. 24 sec. 5 points 1 of the Act;

e) the Contractor's statement about the lack of issuing a final court judgment or final administrative decision on thewith payment of taxes, fees or social security or health insurance fees,

f) the contractor's statement on the lack of a ruling against him as a precautionary measure;

g) the contractor's statement on non-payment of taxes and local charges /

2) If the Contractor is established or place of residence outside the territory of the Republic of Poland, instead of the documents referred to abovein point 1), shall submit, as appropriate: - a) no liquidation has been opened or bankruptcy announced,

b) submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which The contractor has the seat or place of residence or place of residence of the person concerned by the information or the dockment, to the extent specified in art. 24 sec. 1 item 13, 14 and 21 of the Act,

c) is not in arrears with paying taxes, fees, social security or health insurance contributions or that it has entered into an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained by law release, postpone or installment of back payments into installments, orsuspension in full of the implementation of the decision of the competent authority

3) In order to confirm the lack of grounds for exclusion of the contractor from the proceedings referred to in Article 24 paragraph 1 point 23 of the Act, the Contractor, within 3 days from the date of posting on the website, which was provided to the ToR, information on offers submitted in the procedure in accordance with art. 86 par. 5 of the Act, submits pursuant to art. 24 ust. 11 of the Act, declaration on belonging to or lack of belonging to the same capital group
