Purchase of reagents and consumables for immunohistochemistry with apparatus rental, PN-56/20 / JS

The subject of the order is the purchase of reagents and consumables for immunohistochemistry along with the rental of cameras, as described in the subject of the order, Annex 2 and 2a to the Terms of Reference.

Purchase of reagents and consumables for 200, 000 tests / immunohistochemical tests (IHC), 35 tests / EGFR tests and 250 tests / tests of protein PD-L1 (22C3) with equipment rental ( IHC staining system) at the Patolo plantgii and Laboratory Diagnostics for 36 months

1. In order to demonstrate the lack of grounds for exclusion from the procedure pursuant to art. 24 paragraph 1 and paragraph 5 points 1 and 8 of the Act, the contracting authority requires submission with the offer (in accordance with the provisions of Chapter VII of the ToR): Declaration on the form of the "single document" - Annex 3 to the ToR.

2. The contracting authority will call the ContractorThis offer was the highest rated to be submitted by the deadline set, not shorter than 10 days, valid on the date of submission of the following documents confirming the absence of grounds for exclusion, if the Contractor has not indicated in the uniform document the place of collection or use of the document submitted for other proceedings conducted by the Employer:

a) Current excerpt from appropriatethe register or from the central register and information on business activity, if separate provisions require an entry in the register or the register, in order to demonstrate the lack of grounds for exclusion pursuant to art. 24 paragraph 5 points 1

b) Information from the National Criminal Record in the scope specified in art. 24 paragraph 1 items 13-14 and 21 of the Act, issued not earlier than 6 months before the deadlineoffers c.

c) Certificate of the competent head of the Tax Office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids or requests to participate in the procedure, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of these debts togetherwith any interest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of overdue payments or withholding in full the execution of the decision of the competent authority

d) Certification of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that finisheshe is not in arrears with payment of social security or health insurance contributions issued not earlier than 3 months before the deadline for submitting bids or applications for admission to the proceedings, or other document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with with any interest or fines, in particularobtained the exemption provided for by law, deferring or spreading in installments of overdue payments or withholding in full the execution of the decision of the competent authority. / the company has not been ordered as a preventive measure to ban its participation in a public contract (Article 24 paragraph 1 item 22 of the Public Procurement Law),

- that the aforementioned Contractor / company, within the meaning of the Act of 12.1.1991 on local taxes and fees, is not in arrears with the payment of local taxes and fees (Act of 12.1.1991. on local taxes and fees),

- that the aforementioned Contractor / company has not issued a final court judgment or final administrative decision on arrears in payment of taxes, fees or compositionsocial security or health insurance (Article 24 (1) item 15 of the Public Procurement Law).

3. The Contractor, within 3 days from the date of posting on the Employer`s website information referred to in art. 86 section 5 of the Public Procurement Law, i.e. giving names of companies and addresses of contractors who submitted offers on time, is obliged to provide the contracting authority with a declaration of belonging or non-belonging tothe same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law (Annex 7 to the ToR) together with submitting the statement, the contractor may provide evidence that the links with another contractor who submitted an offer in this procedure do not lead to a distortion of competition in the procurement procedure. In addition, the Contractor should submit offer:

a) authorization (original) to signoffers and tender documents, as well as submitting possible explanations, if the signatory is not an authorized person based on an extract from the national court register or a certificate from the central register and information on the economic activity of the Republic of Poland;

b) completed offer form, Annex 1 to the ToR; c) completed limit parameters form, Annex 2 to the ToR;

d)completed price form Annex 2a to the ToR;

e) EAT completed Annex 3 to the ToR;

f) confirmation of payment of the bid security.

To confirm that the offered deliveries meet the requirements specified by the Employer, the Employer will call the Contractor before awarding the contract the offer was rated the highest, to be submitted within a specified time limit of not less than 10 dayseffective on the day of submitting:

a) certificates admitting the subject of the order to trading and use in Poland in accordance with the current provisions of the Act on medical devices for reagents, calibrators, control materials, consumables and analyzers. In the event that the subject of the contract described in the Terms of Reference has not been classified as a medical device and in accordance with the directiveEuropean Union and the Act on medical devices is not covered by declarations of conformity and is not subject to any entry, the Employer requires the submission of an appropriate declaration;

b) materials of the manufacturer or distributor (e.g. folders, technical descriptions, catalogs, etc.) regarding the offered subject of the order, containing information confirming the parameters required in Annex 2 to the ToR. In notedku, if documents are issued in a language other than Polish, please attach a translation.

Other required documents, e.g. from entities submitting a joint bid, subcontractors or foreign entities, have been specified in the ToR. The information clause regarding the protection of individuals with regard to the processing of personal data (GDPR) has been set out in the ToR. Information on the method of cf.contracting the Employer with Contractors and providing declarations and documents, as well as the indication of persons authorized to communicate with Contractors has been specified in the ToR.
