The subject of the order is the supply of chemical reagents and materials and accessories of forensic technology for research for the needs of the Central Forensic Laboratory of the Police. The subject of the order was divided into 22 parts. The detailed description of the subject of the order is specified in the generic-quantity specification constituting Annexes no2.1. 2.22. to the SIWZ (Price forms).

1. Contractors who are not subject to exclusion from the proceedings may take part in the proceedingsproceedings. 2. The orderer shall exclude from the proceedings for awarding the contractor pursuant to art. 24 sec. 1 and par. 5 point 1 of the Public Procurement Law. 2. None of the Economic Operators acting jointly may be subject to exclusion pursuant to art. 24 sec. 1 and art. 24 sec. 5 point 1 of the Public Procurement Law. 3.In order to confirm the fulfillment of the conditions and the lack of grounds for exclusion from participation in the award procedurethe tender, the Contractor shall submit together with the offer valid as of the date of submitting the offers, a statement referred to in art. 25a para. 2 of the Public Procurement Law, in the form of a Uniform European Order Document. 4. Ordering prior to the order being awarded, it will call the Contractor whose offer has been the highest rated, to submit within the prescribed period, not shorter than 10 days, valid as at the date of submission of the pedalconfirmatory declarations or documents: 2.1. no grounds for exclusion, i.e.: 1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law (including for a member of the Contractor`s management or supervisory body, a partner of a company in a registered partnership or partner society or a general partner in a limited partnership or limited joint-stock partnership orproxy, if applicable), issued not earlier than 6 months before the deadline for submission of bids, 2) to confirm the absence of grounds for exclusion under Article. 24 sec. 5 point 1 of the Public Procurement Law - a copy from the relevant register or central records and information on economic activity, if separate regulations require an entry in the register or records; 2.2. that the offered deliveries will meetthe requirements specified by the Ordering Party, ie: 1) information materials (catalogs, prospectuses, technical data, etc.) confirming the compliance of the ordered item with the specific requirements specified by the Employer in Annexes 2.1. 2.22. The Employer allows the submission of catalogs and / or other information materials in English. In the case offeredand the products indicated by the Ordering Party with the name of the producer / model / type, it is not required to attach the above-mentioned documents. 5. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in: 1) point. 2.1. point. 1) submit information from the relevant register or, in the absence of such a register, another equivalent document issued byfrom the competent judicial or administrative body of the country in which the Contractor has his registered office or place of residence or place of residence, the person concerned by information or a document, to the extent specified in art. 24 sec. 1 points 13, 14 and 21. 2) points 2.1. point. 2) submit a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming that they have not been openedliquidation or bankruptcy. 6. If in the country in which the Contractor has a registered office or place of residence or a place of residence, the document concerned does not issue the documents referred to in point 3, it is replaced by a document containing a statement from the Contractor, indicating the person or person entitled to his representation, or a statement of the person whom he isthe document was to concern, filed before a notary or before a judicial, administrative or professional self-government or economic authority competent for the seat or place of residence of the Contractor. Pts. 6 ch. 7 shall apply. 7. The contractor within 3 days from the date of posting on the Purchase Platform and on the website information referred to in art. 86 par. 5 of the Public Procurement Law, porders the Employer to declare that he or she does not belong to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law Act, the model of which is Annex 4 to the ToR. 8. The orderer may apply the procedure of art. 24aa of the Act. 9. The contractor makes a deposit in accordance with Chapter 10 of the Terms of Reference
