a) Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law - issued no earlier than 6 months before the deadline for submitting offers;
b) certificate of the competent head of the tax office confirming that the Contractor is not in arrears with tax payment, issued not earlier than 3 months before the expirythe deadline for submitting bids or applications for admission to participate in the proceedings, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally required exemption, postponement or installment repayment payment or suspension in fullthe competent body of the organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of offers or applications for approvalto participate in the proceedings, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of such receivables, including any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension of the full implementation of the decision competent authority;
d) excerpt from the competent authority;-register or from central records and information on business activities, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 point 1 of the Public Procurement Law Act,
e) Declaration of the Contractor about the lack of a ruling against him as a preventive measure against the prohibition of applying for public orders (see paragraph 19 of this Regulation)section) enclosure 2c to SIWZ,
f) Contractor's declaration on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (see paragraph 19 of this Chapter) Annex No. 2c to SIWZ,
g) Contractor's statement on the lack of issuing a final court decision or final administrative decision on resignation from paymentstaxes, social security or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming the payment of such claims together with any interest or fines or a binding agreement regarding repayment of such claims (see paragraph 19 of this chapter) - attachment No. 2c to SIWZ. - Qualification criteria in accordance withwith order documents
