The scope of the contract includes:

1. property insurance from alllanguage with the extension of the risk of burglary and robbery;

2. civil liability insurance for business operations and property held;

3. insurance of stationary and portable electronic equipment of all risks;

4. insurance of glass and other glass and plastic components against breakage;

5. motor insurance of means of transport (OC, AC, NNW, Assistance);

The detailed description of the subject of the contract was specified in the attachments to the ToR:

Ordering Party in the abovementioned the proceedings use the services of an insurance broker:

Contractors who are not subject to exclusion pursuant to art. 24 paragraph. 1 point 12 23 of the Public Procurement Law in connection from art. 24 paragraph 7 and pursuant to art. 24 paragraph 5 of the Public Procurement Law; meet the conditions for participation in the proceedings regarding: competence or authorization to conduct a specific professional activity, provided that it results from separate provisions:

- the Contractor will meet the condition if he demonstrates that he is authorized to carry out insurance activityin the territory of the Republic of Poland, in accordance with the Act of 11.9.2015 on insurance and reinsurance activity, to the extent identical to the subject of this order or, in the case of conducting business on a basis other than an authorization, another document confirming that the Contractor is entitled to carry out insurance activity in the Republic of Poland in Zakidentical to the subject of this contract;

- current statement as at the date of submission of tenders, constituting a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The contractor submits the statement in the form of the EAT document, to the extent specified in it. In the case of submitting an offer by Contractors jointly applying for the award of the contractthe EAT document is submitted by each member of the consortium;

- offer form Annex 1 to the ToR. If a contractor jointly applies for the contract, this document is signed by a proxy;

- proof of payment of the bid security;

- power of attorney if Contractors jointly apply for the contract, if the Contractor is represented by a proxy (if applicable);

- permission of the competent authoritynu to carry out insurance activity in the territory of the Republic of Poland, in accordance with the Act of 11.9.2015 on insurance and reinsurance activity, to the extent identical to the subject of this order or, in the case of conducting business on a basis other than authorization, another document confirming that the Contractor authorized is to carry out insurance activities on sitee the Republic of Poland to the extent identical to the subject of this order;

- an excerpt from the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;

- information from the KRK in the scope specified in art. 24 paragraph 1 pkt 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the submission deadline;

- general insurance conditions regarding the risks covered by the subject of the contract, constituting the basis of the submitted offer.kite confirming that its liquidation has not been opened or bankruptcy has been issued not earlier than 6 months before the deadline for submission of tenders.

If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland in accordance with the ToR.

The offer must be secured by a bid bond of PLN 120, 000.00. The Awarding Entity informs that pursuant to art. 24aa of the Actstrictly public procurement will first evaluate the bids and then examine whether the Contractor whose bid was assessed as the most advantageous is not excluded from participation in the procedure. In these proceedings, communication between the Employer and Contractors takes place using electronic means of communication within the meaning of the Act of 18.7.2002 on the provision of electronic serviceswith the name (i.e. Journal of Laws of 2017, item 1219, as amended), subject to the requirements set out below. The means of electronic communication in the procedure is the online platform operating at https://e-ProPublico.pl/ (hereinafter referred to as "the platform") and electronic mail. The offer must contain the following statements and documents:

- valid as at the date of submission offers a statement constituting a preliminaryconfirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The contractor submits the statement in the form of the EAT document, to the extent specified in it. In the case of submission of a tender by Contractors jointly applying for the award of the contract, the EAT document is submitted by each member of the consortium; in the event of relying on the resources of third parties documentsEnt EAT is also submitted by these entities. The contractor submits the EAT declaration in the form of an electronic document bearing a qualified electronic signature;

- offer form Annex 1 to the ToR. If the Contractors jointly apply for the contract, this document shall be signed by a proxy;

- proof of payment of the bid security: if the bid security is lodged in a form other than pelectronic document original of the document confirming the payment of the bid security, accompanied by a qualified electronic signature by the issuer of the document;

- power of attorney if Contractors jointly apply for the order, if the Contractor is represented by a proxy (if applicable).

Based on art. 144 section 1 item 1 of the PPL Act, the Employer provides for the possibility of making changes to the concluded contract, in relation to the content of the offer, which is the basis for the selection of the Contractor, in situations described in the relevant provisions of the contract. The bid bond in non-monetary form should be lodged in the original in electronic form. The document is to be signed by a qualified electronic signature by the Guarantee / surety Issuer. Making a deposit in electronic form should include forwarding this documentument in the form in which it was established by the guarantor, i.e. the original document. In accordance with art. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (general regulation on protection dan)) (Journal of Laws EU L 119 of 4.5.2016, p. 1), hereinafter referred to as "GDPR", the Employer informs that: the administrator of personal data provided in the proceedings is the Adam Mickiewicz University in Poznań, H.
