Execution of construction works consisting in replacement of insulation of overhead heating networks and renovation of gantries in the cities of Gdynia and Wejherowo (4 tasks)

1. The subject of the order is the implementation of investment tasks:
Main Site: Gdynia rejon street Kapitańska and ul. J. Wiśniewskiego, Wejherowo district ul. Veteranow, POLAND
2. The subject of the contract includes the replacement of insulation coatings on overhead pipelines, taking place in technologies that take into account the nature of the insulated elements, i.e. for straight sections of the pipeline, insulation should be made using prefabricated insulation shells made of PUR foam;with a galvanized steel sheet jacket as a prefabricated insulation system; for pipe fittings such as elbows and pipe bends as well as T-pieces and fittings, insulation should be made of PUR foam segment elements with foam-bonded contacts, with a galvanized steel sheet cover; for insulation of sliding supports and fixed points, insulation should be used that meets the requirements of mmaterials included in Annex 2 to the ToR.
3. The scope of works planned to be performed under this contract includes, among others execution of works including disassembly and utilization of existing thermal insulation with protective coatings; execution of works including regeneration and anti-corrosion protection of pipelines as well as steel elements and constructions supporting renovated pipespipelines; execution of works including insulation assembly together with all necessary accompanying works; temporary and accompanying works min. pr. all the desired. and play back. after major renovation works; anti-corrosion protection of sliding surfaces, upper parts of concrete supports, anti-corrosion protection of supporting structure elements.
4. As part of the work performed, abouth referred to in point 3.4 of the ToR, accompanying works shall be taken into account, including for: securing construction in terms of health and safety at the site; removal of green interfering with planned works; temporary collection of waste generated during dismantling work on insulation; geodetic works; ensuring the change of traffic organization in the investment area into cwhile building; execution of scaffolding enabling access to pipelines for carrying out works.
5. The contractor is obliged to secure the construction against theft and other negative impacts, taking over the financial consequences in this respect. The value of insurance should include 100% of the value of the work together with materials, including shells for insulation of overhead pipelines transferred to the Contractorzzami of the Employer. Shells for insulation of overhead pipelines, which will be transferred to the Contractor by the Employer have been listed in Annex 1 to the technical specifications for the execution and acceptance of works separate for individual tasks contained in Annex 2 to the ToR, and their value is: PLN 731 296.80 net (details of point 3.6 of the ToR).
6. The scope of works planned for executionand a project was included in the documentation under this order. and STWiOR, constituting Annexes 1 and 2 to this ToR.
7. The Contractor is required to provide the necessary materials to perform the contract, excluding shells for the insulation of overhead pipelines, which will be provided by the Ordering Party, specified in Annexes 1 to STWiOR for specific tasksamending Annex 2 to the ToR.
8. Specifications of shells for insulation of overhead pipelines, which will be provided by the Employer for the purpose of performing the order, are included in Annex 1 to STWiOR, separate for individual tasks. 9. The knee angles indicated in the documentation referred to in item 8 are approximate values.
10. Shells for insulation of overhead pipelinesconstruction site within:
a) in the case of straight sections - up to 14 calendar days from the date of delivery of the written application to the Ordering Party;
B) in the case of knees up to 19 calendar days (details in point 3.11 b of the Terms of Reference). The remaining description of the subject of the order is in point 3.12 3.19 of the Terms of Reference).

1. Contractors who are not subject to the hatch may apply for the contractwith proceedings.
2. The Employer shall exclude from the proceedings the Contractor:
1) who has not demonstrated that the conditions for participation in the procedure have been met or has not demonstrated the absence of grounds for exclusion,
2) subject to exclusion under Article 24 paragraph 1 of the Public Procurement Law (excluding section 1 point 13 letter d) and excluding section 1 point 14 if the person referred to in this provision has been validly convicted of an offensecrime referred to in art. 24 paragraph 1 point 13 lit. d in accordance with art. 133 section 4 of the Public Procurement Law),
3) subject to exclusion under Art. 24 paragraph 5 point 1, point 3, point 8 of the PPL Act
3. In order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings, the Employer (pursuant to the Regulation of the Minister of Development of 26.7.2016 on the type of documents that the Employer may requestfrom the Contractor in the procurement procedure OJ 2016 item 1126) requests the following documents:
1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act issued not earlier than 6 months before the deadline for submitting bids;
2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears withpayment of taxes issued not earlier than 3 months before the deadline for submitting bids or other document confirming that the Contractor has entered into an agreement with the relevant tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferral or distribution provided for by law installments of late payments or wstrcomplete execution of the decision of the competent authority;
3) a certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions issued not earlier than 3 months before the expiry offer submission deadlinet or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferment or installment of outstanding payments as provided for by law, or withholding in full the decision of the competent authority;
4 ) an excerpt from the relevant register or from the central registryand information on economic activity, if separate provisions require an entry in the register or records, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;
5) the Contractor`s statement that he has not issued a final court judgment or a final administrative decision regarding arrears with the payment of taxes, fees or social security contributionshealth or health insurance or in the event of the issuance of such a judgment or decision of documents confirming the payment of these receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables according to the template constituting Annex No. 9 to the ToR;
6) declarations of the Contractor regarding the lack of judgment against him as a preventive measure of the prohibition on applying ppublic procurement according to the model constituting Annex No. 9 to the ToR;
7) the Contractor`s declaration of non-arrears in the payment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2016, item 716) according to the template constituting Annex No. 9 to the ToR;
8) the Contractor`s statement on belonging or not belonging to the same capital group, aboutreferred to in art. 24 paragraph 1 point 23 of the Public Procurement Law (according to the model constituting Annex No. 8 to the ToR), and in the case of belonging to the same capital group, the Contractor may submit together with a statement documents or information confirming that links with another Contractor do not lead to distortion of competition in the proceedings.
