The subject of the order is Purchase of promotional items for the NFEPWM.

The subject of the order ispurchase of the following promotional items: beverage bottle (300 pieces), bamboo cutlery set (200 pieces), set of organic bamboo straws (200 pieces), bags (1, 000 sets), A5 notebook with apples (300 pieces) pcs.), eco coloring book for children (300 pcs.), jute bag (300 pcs.), conference folder (1, 000 pcs.)
A detailed description of the subject of the order is included in Part III of the ToR.
1. ContractorsApplicant for this public contract:
1.1. cannot be excluded pursuant to art. 24 paragraph 1 of the Public Procurement Law (i.e.: Journal of Laws of 2019, item 1843);
1.2. cannot be excluded pursuant to art. 24 paragraph 5 point 1 and 8 of the PPL Act
2. The Employer does not set the conditions for participation in the procedure to be met by Contractors.
3. Contractor`s exclusion takes place in situations specified in pkt 1 above,
with regard to art. 24 paragraph 7 of the Public Procurement Law
4. The Contractor shall attach to the offer a current statement as at the date of submitting the bids to the extent specified by the Employer. The information contained in the statement constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Contractor submits the statement in the form of a Single European DocumentOrders, hereinafter referred to as "single document or EAT" (in accordance with the template of the standard form set out in the European Commission implementing regulation issued on the basis of Article 59 (2) of Directive 2014/24 / EU and Article 80 (3) of Directive 2014/25 / EU ) in electronic form, provided with a qualified electronic signature.
5. To confirm the absence of grounds for exclusion, the Contractorshall be obliged to submit, within the time limit set by the Employer, the following statements and documents:
5.1. information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;
5.2. Contractor`s statement that he has not issued a final or final judgment against himadministrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions (in accordance with Annex 4 to the IDW) or in the event of issuing such a judgment or decision, documents confirming payment of these receivables together with any interest or fines or concluding a binding agreement in regarding the repayment of these claims;
5.3. oświadcContractor`s decision on the lack of a decision as a precautionary measure against him being banned from applying for a public contract (in accordance with Annex No. 5 to IDW);
5.4. an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or a register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph5 point 1 of the Public Procurement Law;
5.5. a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables with any interest or fines, onhe has been provided with an exemption, deferral or installment of overdue payments as provided for by law or withholding in full the execution of the decision of the competent authority;
5.6. certificate of the relevant field organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund (KRUS) or other document confirming that the Contractor is not in arrears with payment of insurance premiumssocial or health security, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained exemption, deferral or installment of overdue payments or withholdsnot in full implementation of the decision of the competent authority;
5.7. the Contractor`s declaration of non-payment of local taxes and fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2018, item 1445, as amended) in accordance with Annex 6 to IDW.
Continued documents confirming the absence of grounds for exclusion:
1. Contractor within 3 days of the residencezenia on the website (www.nfosigw.gov.pl/zamowienia-publiczne/) of the information referred to in art. 86 section 5 of the Public Procurement Law, provides the Employer with a statement on belonging or not belonging to the same capital group, as defined in the Act of February 16, 2007 on competition and consumer protection (i.e., Journal of Laws of 2019, item 369, later as amended), with other Contractors who submittedseparate offers.
2. Employer, in accordance with art. 24aa of the Public Procurement Law, first evaluate the offers and then examine whether the Contractor whose tender has been assessed as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.
3. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland:
3.1. instead of the documents referred to:
1) in pkt III.1.1). 5.1. the notice submits information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or domicile, the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL Act,
2) in point III.1.1). 5.4. III.1.1) .5.6. notices shall be submitted by a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming accordingly that:
a) its liquidation has not been opened or bankruptcy has been declared;
b) it is not in arrears with taxes, social security contributions or health insurance or that he has entered into an agreement with the competent authority ine the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority.
3.2 If in the country in which the Contractor has its registered office or place of residence or place of residence the person to whom the document relates resides, no document appearsgoods referred to in point 3.1 above, are replaced by a document containing, respectively, a statement of the Contractor, with an indication of the person or persons authorized to represent him, or a statement of the person to whom the document was to relate, made before a notary public or before a judicial, administrative or professional self-government body or business relevant for the registered office or citythe residence of the Contractor or the place of residence of that person.
3.3. The Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the document referred to in point III.1.1 applies. 5.1. the notice is submitted by the document referred to in point III.1.2). 3.1. announcements, to the extent specified in art. 24 paragraph 1 point 14 and 21 of the PPL Act. If in the country in which the person to whom the document was supposed to reside there does not appear such documents, it is replaced by a document containing a statement of that person submitted to a notary public or before a judicial, administrative or professional or economic self-government body competent for the place residence of that person.
3.4. Documents, referred to in point 3.1.1) above and 3.1.2) a) above, or the documents replacing them, referred to in point 3.2. and 3.3. above, should be issued not earlier than 6 months before the deadline for submission of tenders, while the document referred to in point 3.1.2) b) above, or the documents replacing it, referred to in point 3.2 above, should be issued not earlier than 3 months beforethe bid submission deadline is
4. In the case of Contractors jointly applying for the award of the contract, there are no grounds for exclusion referred to in point III.1.1) 1. of the announcement, must be shown by each of the Contractors.
1. In these proceedings, communication between the Employer and the Contractors takes place using electronic means of communication, via the miniPortal https: //miniportal.uzp.gov.pl /, ePUAPu https://epuap.gov.pl/wps/portal and e-mail to the following email address: [email protected].
2. The person authorized by the Employer to contact the Contractors is: Rafał Wroblewski from Mon. to Fri at 7:30 15:30; e-mail: [email protected].
3. In all correspondence related to this procedure, the Employer and the Economic Operatorthey use the advertisement number (TED or procedure ID).
4. The contractor intending to take part in the public procurement procedure must have an ePUAP account. The Contractor with an ePUAP account has access to the following forms: submission, modification, withdrawal of an offer or application, as well as to the communication form.
5. Technical and organizational requirements for sending and receiving documents electelectronic copies of documents and statements as well as information provided using them are described in the Regulations of using the miniPortal ("Terms of Service", https://miniportal.uzp.gov.pl/WarunkiUslugi.aspx) and the Regulations of ePUAP (https: // epuap.gov.pl). Additional information is also included in the document "Frequently asked questions to niniPortal". It is assumed that Execawca participating in the proceedings accepts the Regulations available at https://epuap.gov.pl., as well as the Regulations of the miniPortal.
6. The maximum size of files sent via dedicated forms for submitting, changing, withdrawing an offer or application and for communication is 150 MB (the sent files should be compressed into one ZIP archive file).
7. For the date of submitting the offer, inlayers, notifications, electronic documents, statements or electronic copies of documents or statements and other information, the date of their submission to ePUAP is assumed, except for samples for which the deadline for their submission in physical form at the address indicated in point I of IDW (within the time limit indicated on submission of bids).
8. Case identifier and public key for dataProceedings for awarding the contract are available in the List of all proceedings on the miniPortal and in the Annex to the ToR.
9. It is recommended to copy (stick) the Identifier of the procedure, not to rewrite it.
10. The offer together with the attachments required to it, referred to in point XIII.1.6 IDW (subject to point XIII.1.6) lit. d must be submitted via the Form for submission, changes, withdrawal of an offer or application available on ePUAP and also made available on the miniPortal. The public key necessary to encrypt the offer by the Contractor is available to contractors on the miniPortal. In the bid form, the Contractor is required to provide his ePUAP mailbox address, where correspondence related to the procedure may be conducted (it is also recommended to provide an email address).
10.1. The offer should be made in Polish, in electronic form and signed with a qualified electronic signature, excluding samples that must be submitted in physical form to the address given in point I IDW. The offer should be submitted in the original.
10.2. The Purchaser recommends the following format of sent data: .pdf.
Sending data in other formats, e.g.: .doc, .docx, .rtf, .xps, .odt is acceptable, but not recommended due to possible technical difficulties
With the verification of the correctness of the submission of a qualified electronic signature.
Note: it is recommended that the document (offer, EAT, etc.) created using software that creates files in in a format other than .pdf, save in .pdf
