The subject of the contract is a supply of innovative technology, consisting of 2 logical units:

Detailed description of the subject from(hereinafter referred to as "SP"). The public procurement may be awarded only to those who meet the conditions for the personal participation under Section 32 (1) of the Constitution. (a) to h) ZVO.

The applicant or the person proving the fulfillment of the condition stated in the point:

- (a) attested by a record of the criminal record not older than 3 months,

- (b) attested by the health insuranceand (d) attested by a confirmation of the relevant vessel not more than 3 months, - (e) proof of proof of the supply of the goods, which is not more than 3 months, to carry out construction work or to provide a service which corresponds to the subject of the bid, - - f) attested by a solemn declaration.

Uchadzač splneniethe conditions of part time can be established under Section 152 (1) and (3) of the GPA. The entrepreneur entered in the list of entrepreneurs pursuant to the Public Procurement Act in force until 17 April 2016 is entered in the list of economic subjects within the scope of recorded facts. If the applicant has a place of business or place of business or usually resides outside the territory of the Slovak Republic and the status of his place of business, the apartment does not issue some of the documents mentioned in point 2 or does not issue even equivalent documents, it can be replaced by an affidavit in accordance with the regulations in force in the state of establishment, place of business or usual residence. If the right of the applicant applies to a seat, place of business or usual place of residence outside the territory of the Slovak Republic does not regulate an institute of honorary affidavit, he may replace it by declaringthe administration, the notary or other professional body in accordance with the regulations in force in the state of his or her place of business or the place of business or usual residence of the person seeking the service. The fulfillment of the conditions of the applicant may be proving temporarily by submitting the Single European Document pursuant to Article 39 of the GVA. Documents proving the fulfillment of the conditions of part time replaced by the Single European Documentthe applicant according to - 39 par. 6 FVC in time and in part, designated by the contracting authority. A single European document can be downloaded from the UVO web site (www.uvo.gov.sk). The procuring entity requires that the Single European Document be signed by a person authorized to act on behalf of the tenderer. The applicant who represents the group of contractors shall demonstrate that the conditions for the personalbuilding for each member of the group separately. The fulfillment of the condition of part-time according to - 32 par. 1 floor. e) ZVO proves a member of the group only in relation to the part of the subject of the contract, which is to ensure that it is technically feasible or professional. 1 floor. a) Act no. 343/2015 Z.z. - the list of goods or services supplied for the preceding three years from the award of the public procurementprices, delivery times and purchasers; the document is a reference if the customer was a contracting authority or a contractor under Act No. 343/2015 Z.z. - The contracting authority requires the applicant to submit a list of goods delivered for the previous 3 years (to be considered as 3 years prior to the date of submission of the bid as the last day of the deadline for submission of tenders)they are fulfilled in the sense of - 34 par. 1 floor. a) point 1 resp. 2 of Act No. 343/2015 Zz, which demonstrates the realization of at least 2 supplies of equal or similar character than the subject of the order (Part 1 Laser Cutting Machine), at a minimum value of EUR 350 000 excluding VAT for each bid and realization of at least two equipments of equal or similar nature as the subject of the order (2nd part HydraulicCNC folding press) at a minimum of EUR 90, 000 exclusive of VAT for each order. The list of delivered goods must be signed by the applicant's statutory body or an authorized person authorized to act on behalf of the applicant.
