The subject matter of the public contract is to ensure the continuous supply of medicinal preparations with the active ingredient Teriflunomide (hereinafter referred to as "medicated preparations") for the needs of Krajske zdravotni, as as a health service provider sales / releases), including their direct delivery to pharmacies in individual oDoubtful plants of Krajske zdravotni, as, listed in point 5 of the tender documentation.

The subject of the public contract is the provision of continuous deliveries of medicinal preparations with the active substance Teriflunomide (hereinafter referred to as "medicinal preparations") for the needs of Krajske zdravotni, as as health service providers (not for resale / dispatch), including their direct delivery topharmacy in individual branch establishments of the Regional Health Service, as listed in point 5 of the tender documentation.

The participant shall demonstrate the basic eligibility specified in Section 74 of the Public Health Insurance Act by submitting documents pursuant to Section 75 of the Public Procurement Act or otherwise in accordance with the Public Health Insurance Act. The participant shall demonstrate the professional competence under Section 77 (1) by submitting a statement from the Commercial Register or other similar register if anotherClause 45 and Section 86 of the Public Procurement Act governs such records. The Contracting Authority recommends using Section 45 (4) of the Public Procurement Act to prove the qualification and to prove the given part of the qualification by reference to the corresponding information kept in the public administration information system or in a similar system maintained in another member state which allows unlimited access. Such reference must contain internallyaddress and data for logging in and searching for the required information if such data is necessary. The participant is entitled to prove fulfillment of all qualifications to present a declaration of honor (Attachment No. 4).
