1. The subject of the order is the provision of services in the scope of: preparation, delivery of all-day meals for patients of the Employer, cleaning of thermocouples, GN inserts from hospital departments and rental at the headquarters.go to the rooms necessary to complete the task.

1. The subject of the order is the provision of services in the scope of: preparation, delivery of all-day meals for patients of the Employer, cleaning of thermocouples, GN inserts from hospital departments and leasing at the Ordering Party's premisesfor carrying out the task.

2. The scope of the orders includes a full board of patients divided into the diets specified in Appendix No. 5 to the ToR.

3. Meals to the hospital should be delivered at strictly agreed times of the day:

a) breakfast from 07:15 to 07:40

b) lunch from 12:15 to 12:40

c) dinner from 17:00 to 17: 15

The approximate number of patients is around 160 per day, so the Contractor will be required to provide 175 360 meals throughout the duration of the contract. Meals for one patient consist of three meals (breakfast consisting of a milk soup, a second dish and a drink, a dinner consisting of two dishes and a drink and dinner), as well as 5 or 6 meals a day for various special diets, while maintaining the correct withnursing patients in hospitals. Additional meals should be packed collectively for each branch, each portion separately. In the case of a diabetic diet, the Contractor is obliged to deliver dairy-free soups (varied) or substitute products for breakfast. In addition to basic meals, which include breakfast, lunch and dinner. The contractor will also provide a second breakfast, underin the evening, a night meal, the second breakfast and an afternoon snack are issued for diabetic, high-calorie, high-protein diets etc. According to individual diet recommendations, issued in the form of: vegetables, fruit, jelly pudding, , salads, yogurt, buttermilk, kefir, etc. A night meal - issued for a diabetic diet of dependent insulin.

4. Basic diets account for about 50% of the total number of mealsin, easily digestible diet about 30%, and special diets about 20%.

5. The exception to the amount of meals provided is the period of Christmas, New Year and Easter, in which the amount of meals can be reduced by up to 50%.

7. During the holidays and occasional celebrations (eg Christmas Eve, Easter), the Contractor is obliged to prepare traditional dishes, includingconfectionery donations.

6. Percentage of daily energy needs of 3 meals a day should be: - a) breakfast about 30%,

b) lunch about 45%,

c) dinner about 25%.

Contractors who submit an important and properly prepared offer and have a valid decision and a certificate of entry in the register of establishments subject to the official kontrols of the competent local authority (in terms of the location of the food preparation plant) of the State Sanitary Inspector confirming that the facility meets the requirements necessary to ensure hygiene in the production process and in the foodstuffs and to ensure the proper health quality of these foods in the food production, allowed additivesor other ingredients of food and materials and articles intended to come into contact with food or to turn them. The decision of the State Poviat Sanitary Inspector should include a permit for collective catering and the provision of a service in the catering system, as well as have a valid decision of the appropriate State Sanitary Inspector competent for the sake ofthe Contractor's approval of the means of transport used to execute the order as meeting the appropriate sanitary requirements for the purposes of transporting foodstuffs (meals), Contractors who submit a valid and suitably prepared offer and have civil liability insurance in the scope of their related activities can apply for the order. with the prefaceiota orders for the amount of min. 1, 000, 000.00 PLN
