Gammakamera two-head rotary coupled with a spiral multilayer computed tomograph (both devices integrated gantry in one housingwie) allowing to perform diagnostic tests and correction of radioisotope images (correction of absorption, dispersion) and a common patient table for performing independent tests:
1. Scintigraphic:
- planar (static, dynamic and gated),
- SPECT, gated SPECT,
- whole body,
- all other acquisitions performed using one and / or two heads.
2.Hybrydowych SPECT / CT.
3. Diagnostic CT.
Gammakamera brand new, not reconditioned, year of production 2018, not used, is not a prototype, not a demonstration.
Gammakamera complete and will be ready after installation ready to undertake specific medical research without any additional purchases and investments.
Offered medical devices within the meaning of Act on medical devices datedOn 20.5.2010 (ie on 17/01/2017 Journal of Laws of 2017, item 211, as amended), they must have a CE certificate or a CE declaration of conformity.
The contractor is obliged to uninstall Gamechakamera Varicam (Elscint) inventory no. T-802/2529 and its disposal. - Contractor whose tender will be evaluated as the most advantageous, in order to demonstrate the absence of grounds for exclusion from the procurement procedureia will be called on the basis of art. 26 par. 1 PPL to submit the following declarations and documents within the prescribed period:
1. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the PPL, issued not earlier than 6 months before the deadline for submission of tenders;
2. declaration of the Contractor about the lack of a ruling against him as a precautionary measure zakto apply for public orders;
3. excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion pursuant to art. 24 sec. 5 point 1 of the Act;

Contractor whose offer will bee assessed as the most advantageous, in order to demonstrate the absence of grounds for exclusion from the procurement procedure (XIII clause 3 of the Terms of Reference) will be called on the basis of art. 26 par. 1 PPL to submit the following declarations and documents within the prescribed period:
1. certificate of the competent head of the tax office confirming that the Contractor is not in arrears with paying taxes, issued notlater than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment in installments of overdue payments or suspension in full of the implementation of the decision responsibleand the body;
2. a certificate issued by the relevant organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of bids or other document confirming, that the Contractor has entered into an agreement with the competent authority regarding repayment of such receivables, together with any interest or fines, in particular obtaining legally required exemption, postponement or spreading overdue payments or stopping in full the execution of the decision of the competent authority; the Contractor's statement that he has not issued a final court judgment or final judgment against himan administrative complaint about defaulting on taxes, fees or social or health insurance premiums, or in the event of issuing such a judgment or decision - documents confirming payment of such receivables with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables; . the Contractor's declaration of non-payment paystaxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (ie on 15/09/2017 Journal of Laws of 2017, item 1785, as amended); If the Contractor has a registered office or place of residence outside the territory of the Republic of Poland, he shall submit documents in accordance with the Regulation of the Minister of Development of 26.7.2016 on the types of documents that may be requestednotifying the contractor in the procurement procedure (Journal of Laws of 2016, item 1126).

II. The contracting authority provides for the application of the procedure set out in art. 24aa Pzp ("reversed procedure").
III. The offer must be secured by a bid bond in the amount of PLN 55, 000.00. The deadline for submitting the bid bond is the same as the offer submission date. IV. The ordering party does not anticipate orders to supplementh, referred to in art. 67 sec. 1 point 6 and 7 of the Public Procurement Law - V. Documents and statements from this item shall be submitted to the ordering party's request pursuant to art. 26 par1 Pzp
1. Declaration on possession of the CE Certificate or CE declaration of conformity for the offered medical devices within the meaning of the Act of 20.5.2010 on medical devices (ie from 13/12/2018 Journal of Laws of 2018 item 175 with amendments. ).
2. For the offered item, the Ordering Party requires submission of a description containing its specific data, which will enable confirmation of compliance with the requirements set by the Employer and quality / technical assessment, in the form of information leaflets, catalogs or other documents for all offered products subject to the order. Please indicate the pagesy offers and selection of data confirming the fulfillment of the parameters included in the table.
3. The Employer may call for a demonstration (presentation) of the offered freestanding workstation in the same software version by the Contractor at the Employer's headquarters in the same software version as offered in the procedure, in the scope of the requirements set out in the Terms of Reference for the abovementioned
