Gammakamera two-head rotary coupled with a spiral multi-layered computer tomograph (both devicesia integrated gantry in one housing) allowing to perform diagnostic tests and correction of radioisotope images (correction of absorption, dispersion) and a common patient's table for performing independent tests:
1. Scintigraphic:
- planar (static, dynamic and gated),
- SPECT, gated SPECT,
- whole body,
- all other acquisitions carried out with-help of one and / or two heads.
2. Hybrydowych SPECT / CT.
3. Diagnostic CT.
Gammakamera brand new, not reconditioned, production year 2018, not used, is not a prototype, not a demonstration.
The contractor is obliged to uninstall gammakamery Varicam (Elscint) inventory no T-802/2529 and dispose of it., Description in SIWZ
Contractor, whose offer will be evaluatedas the most advantageous, in order to demonstrate the absence of grounds for exclusion from the procurement procedure (XIII clause 3 of the Terms of Reference) will be called on the basis of art. 26 par. 1 PPL to submit the following declarations and documents within the prescribed period:
1. information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the PPL, issued not earlier than 6 months beforeexpiry of the deadline for submitting offers;
2. the Contractor's statement about the lack of a ruling against him as a preventive measure of the prohibition against applying for public orders.
3. excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under art. 24-paragraph. 5 point 1 of the Act;
If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, files documents in accordance with the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procurement procedure (Dz. U. 2016 item 1126)
The detailed description includes the Specification of Essential Order ConditionsI a.
I. The contracting authority provides for the application of the procedure set out in art. 24aa Pzp ("reversed procedure").
II. The offer must be secured with a bid bond in the amount of PLN 37, 000.00.
The deadline for submitting bids is the same as the offer submission date.
III. Initial confirmation of compliance with the conditions of participation and no grounds for exclusion The Contractor sends an electronic form in the formwith a qualified electronic signature valid as of the day of submitting bids, a statement in the form of a Single European Order Document to the extent indicated in the SIWZ to the email address: [email protected]. The ordering party will call the Contractor, whose offer will be the highest rated for submission of documents (based on the content of -5 of the Regulation of the Minister of Development of 26/07/2016 regarding document typesin which the contracting authority may request from the contractor in the proceedings for the award of the contract (Journal of Laws of 27.7.2016, item 1126).
IV. The ordering party does not provide for supplementary orders referred to in art. 67 sec. 1 point 6 and 7 of the Public Procurement Law. Documents and declarations are submitted to the ordering party's request based on art. 26 par. 1 PPL (Article 26 (1)):
1. Declaration on having the CE Certificate or deCE compliance with the offered medical devices within the meaning of the Act of 20.5.2010 on medical devices (i.e., 17.1.2017, Journal of Laws of 2017, item 211, as amended).
2. For the offered item, the Ordering Party requires submission of a description containing its specific data, which will enable confirmation of compliance with the requirements set by the Employer and quality assessment /technical information, in the form of information leaflets, catalogs or other documents for all offered products subject to the order. Please indicate the offer page and check the data confirming that the parameters specified in the table are met.
3. The Employer may call for a demonstration (presentation) offered by the Contractor at the Employer's officej the workstation in the same software version as offered in the procedure, in the scope of requirements specified in the ToR for the above-mentioned
VI station. Detailed information about the conditions of participation are included in the Specification of Essential Order Conditions (SIWZ)
