The subject of the contract is forest management services as defined in art. 6 clause 1 point 1 of the Act of 28 September 1991 on forests (consolidated text: Journal of Laws of 2018, item 2129, as amended) covering works in the field of harvesting, skidding, harvesting by-products, forestry in youm works on the development of the Żarczyn Forestry School, seed production, forest protection, fire protection, tree cover, maintenance of tourist facilities and forest car parks, maintenance of forest roads, maintenance of water and drainage facilities, and preparation of wood for sunbathing of forestry offices, to be carried out. The material scope of the subject of the contract are included in Annexes 1-22 to the ToR. Purchaser of subdivisionThe order was divided into 22 parts. A detailed description of the works (activities) included in the subject of the contract, and a detailed description of the technology for performing individual works (activities) included in the subject of the contract, includes Annex No. 23 to the ToR.

The subject of the contract are services in forest management as defined inArt. 6 clause 1pt. 1 of the Act of 28 September 1991 on forests, covering the following works: Harvesting, logging, forest cultivation, tree cover, forest protection, fire protection, maintenance of tourist facilities and forest car parks, preparation of wood for the office tanning, tree cover
2) Scope the subject matter of the contract is included in Annex 1 to the ToR.
3) Detailed description of the works (activities) included in the subject of the contract, and a detailed description of the technology for performing individual works (activities) included in the subject of the contract, is included in Annex 23 to the ToR., however, not less than 70% of the Value of the Subject of the Agreement
1. They may participate in the proceedingsContractors who are not subject to exclusion from the contract award procedure in the circumstances referred to in art. 24 paragraph 1 point 12-23 PZP, art. 24 paragraph 5 points 1 2 and 4 8 PZP. 2. In order to confirm the lack of grounds for exclusion from the procedure, the Contractor shall be obliged to present: a) the Contractor's statement made on the form of the uniform European order document, b) copyfrom the relevant register or from the central register and information on economic activity, if separate provisions require an entry in the register or the register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the PZP; c) certificate of the competent head of the tax office confirming that the contractor is not in arrears with tax payments, issued not earlier than 3 monthsbefore the deadline for submission of bids or other document confirming that the contractor has entered into an agreement with the competent tax authority on the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full of execution decision of the competent authority; d) certifya request from the appropriate field organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund (Kasa Rolniczego Ubezpieczenia Społecznego) or another document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers or another document confirming that the contractor has concluded agreement withthe competent authority on the repayment of these receivables, together with any interest or fines, in particular, obtained legal exemption, deferment or installment of overdue payments or withholding in full the decision of the competent authority; e) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act and, regarding conviction fora year of imprisonment, to the extent specified by the contracting authority pursuant to art. 24 paragraph 5 points 5 and 6 of the Act, issued not earlier than 6 months before the submission deadline; h) the contractor's statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security contributionsvoting documents or documents confirming the payment of those debts together with any interest or fines, or concluding a binding agreement on the repayment of these debts (in relation to the premise for exclusion described in Article 24 (1) point 15 of the PZP); i) a declaration by the contractor that he has not been awarded a preventive measure against hima ban on competing for public contracts (in relation to the premise of exclusion described in Article 24 paragraph 1 item 22 of the PZP); j) statement of the contractor about the lack of a final judgment of the convicting court for the offense of restriction of liberty or a fine to the extent specified by the awarding entity pursuant to art. 24 paragraph 5 points 5 and 6 of the PZP; k) declaration of the contractor that no osdated administrative decision on violation of obligations arising from labor law, environmental protection law or social security provisions to the extent specified by the contracting authority pursuant to art. 24 paragraph 5 item 7 of the PZP; l) the contractor's declaration of non-payment of local taxes and fees, referred to in the Act of 12 January 1991 on taxes and feeslocal authorities 2. In terms of competence or authorization to conduct business, the Employer does not impose specific requirements. The assessment of compliance with the conditions for participation in the procedure will be made in accordance with the formula meets - does not meet, based on JEDZ, and documents submitted at the request of the contracting authority.
Contractors who meet the conditions for participation in the proceedings for which may participatereferred to in art. 22 paragraph 1 PZP regarding the economic and financial situation. To confirm compliance with the abovementioned of the conditions for participation in the procedure, the Contractor will be required to provide the Employer with the following statements and documents (within the time limits specified in the ToR):the form of the uniform European order document (hereinafter: "EAT"), b) information from the bank or cooperative savings and credit union confirming the amount of financial resources or creditworthiness of the contractor, not earlier than 1 month before the deadline for submitting bids, if for a justified reason the contractor cannot submit documents regarding pfinancial or economic conditions required by the contracting authority, may submit another document that sufficiently confirms that the condition of participation in the procedure described by the contracting authority is met. in EAT, then confirmed in documents or axisstatements submitted by Contractors at the request of the contracting authority. If in the documents submitted to confirm compliance with the conditions for participation in the proceedings any values are given in the currency
Foreign, the Purchaser will convert the value of the currency into PLN according to the average exchange rate of the National Bank of Poland from the day of submitting the contract notice to the Official Journal of the European Union.
Before concluding a contract regardingknows the public contract, the Contractor whose bid was considered the most advantageous is obliged to complete the following formalities:
1) provide the required security for the due performance of the contract;
2) submit to the Ordering Party:
- consortium agreement, if the contract is carried out by contractors jointly applying for the award orders,
- certificate of completion with the result of ppositive specialist training in the field of sawing for persons indicated in JEDZ,
- documents confirming the condition of the education of persons indicated as supervisors in JEDZ,
- certificate of completion of training in working with plant protection products in accordance with the Regulation of the Minister of Agriculture and Rural Development of on May 8, 2013 on training in the field ofplant protection (Journal of Laws of 2013, item 554) for the persons indicated in the JEDZ,
- document confirming the basic skills in the manipulation and classification of wood of the operator of multi-operational machines
- the civil liability policy specified in the model contract agreement public (Annex 29 to the ToR) in the amount of at least PLN 50, 000 per Package,
contractart with a company authorized to collect and utilize oil-absorbed absorbents,
- documents referred to in - 7 para. 4 of the model contract (constituting Annex 29 to the ToR) regarding persons performing activities included in the subject of the contract consisting in obtaining and skidding wood raw material, if the performance of these activities involves performing work in the manner specified in art. 22 - 1 mouthye of June 26, 1974 - Labor Code (consolidated text: Journal of Laws of 2019, item 1040, as amended), i.e.: Certified copy of the contract by the contractor or subcontractor, respectively employment contract of persons to whom the Employment Obligation relates, together with a document regulating the scope of obligations, if it has been drawn up). A copy of the contract / contract should contain information, including personal datathose necessary to verify employment on the basis of an employment contract, in particular the name of the employee employed, date of conclusion of the employment contract, type of employment contract, duration of work and scope of the employee's obligations;
- decision of the Office of Technical Inspection regarding placing in service, and audit book request for machinery and equipment equipped with cranes.
Failure to comply with those indicatedformalities will be treated as the Contractor's evasion of conclusion of the public procurement contract.
