The subject of the contract is forest management services as defined in art. 6 par. 1 point 1 of the Act of 28/09/1991 on forests (consolidated text: Journal of Laws of 2017, item 788 with further changes: "Forest Act") covering works in the scope of: logging and skiddingrenaissance, breeding and protection of the forest, nursery economy, fire protection, tourism management of the forest and seed and selection, to be carried out in the Ostrowiec Świętokrzyski Forest District in 2019

Main Site: Forestry: Czyżow, Stroża, Skałecznica, Piaski, Narożniki

The subject of the orders are Forest management services as defined in art. 6 par. 1 point 1 ustawa of 28/09/1991 on forests (consolidated text: Journal of Laws of 2017, item 788 with further changes: "Forest Act") including works in the scope of: harvesting and skidding of wood, breeding and protection of the forest, nursery economy, fire protection, tourism management of the forest and seed and selection, to be carried out in the Ostrowiec Świętokrzyski Forest District in 2019.

2)description of the subject of the order (activities), their codes, the procedure of receipt, material scope covering the total amount of works in the scope of: logging and skidding, forest breeding and protection, nursery economy, fire protection, tourist forest management and seeding and selection, including a detailed description the technology for the performance of individual works was included in the annexNo. 2.I to SIWZ. The material ranges listed in Annexes 2.I are estimated.

3) I forestry package: Czyżow, Stroża, Skałecznica, Piaski, and Narożniki includes works in the field of forest breeding, seeding and selection, forest protection, tourism management of the forest, harvesting and skidding wood, fire protection.

III) for Package III

a) at least 6 people who finishpositive training for the work with a saw in accordance with - 21 of the Regulation of the Minister of the Environment of 24.8.2006 on occupational health and safety in the performance of certain forest management works (Journal of Laws No. 161, item 1141) or having the corresponding entitlements issued on the basis of previously applicable provisions or the corresponding entitlementsissued in other European Union Member States;

b) at least 1 supervising person with higher or medium forestry education;

c) at least 1 person who completed training in working with chemicals in accordance with the regulation of the Minister of Agriculture and Rural Development from 8.5.2013 training in working with chemicals (Journal of Laws of 2013, item 554) or psettling corresponding entitlements issued on the basis of previously applicable regulations that entitle to work in contact with plant protection products.

IV) for Package IV

a) at least 1 person who successfully completed training permitting to work with a saw according to - 21 Regulation of the Minister of Environment of 24.8.2006 on occupational health and safetyy performing certain forest management works (Journal of Laws No. 161, item 1141) or having corresponding authorizations issued on the basis of previously applicable provisions or equivalent qualifications issued in other EU Member States;

b) at least 1 person supervision with forest or higher forest education;

c) at least 1 person who graduatedy training in working with chemicals in accordance with the regulation of the Minister of Agriculture and Rural Development of 8.5.2013 training in working with chemicals (Journal of Laws of 2013, item 554) or having corresponding qualifications issued on the basis of previous regulations that entitle you to work in contact with plant protection products.

1. condition of participation in pwith respect to the condition regarding economic capability. The condition with respect to the financial situation will be met if the Contractor shows that he has the financial resources or creditworthiness not less than:

I) for Package I

III) for Package III

475, 000.00 PLN

IV) for Package IV

2. The bid bond

The proceedings are proin accordance with the rules provided for the so-called "Reverse procedure" referred to in Article 24aa (1) and (2) of the Public Procurement Law. According to the aforementioned provisions, the Awarding Entity will first evaluate the offers and then examine whether the contractor whose tender was rated as the most advantageous is not subject to exclusion and meets the conditions of participation in the tender - the offer and the statement of EAT shall be submitted byd the deadline for submission of offers, only in electronic form, with a qualified electronic signature. The rules related to the transmission of the offer are described in chapter 8 of the ToR.

In order to confirm the lack of grounds for exclusion from the proceedings referred to in point 6.1. The Contractor will be required to provide the Employer with the following statements and documents (in the dates indicated in thein the SIWZ): a) an excerpt from the relevant register or from central records and information about business activity,

b) certificate from the competent head of the tax office

c) a certificate from the relevant organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund;

d) information from the National Criminal Record in the scope specified in art. 24 sec. 1 point 13, 14 and 21 Pzp and, regarding a conviction for a prison sentence, to the extent specified by the ordering party under art. 24 sec. 5 points 5 and 6 Pzp,

e) the contractor's statement that he has not been the subject of a final court judgment or final administrative decision on late payment of taxes, fees or social security or health insurance premiums or in the event of such a releasedecision or decision confirming the payment of these receivables together with any interest or fines or entering into a binding agreement regarding the repayment of such debts (in relation to the exclusion referred to in Article 24 paragraph 1 item 15 PPL) (according to the model constituting Annex 5 to SIWZ);

f) declaration of the contractor about the lack of a decision against it as a means of protectionthe prohibition of applying for public orders (in relation to the exclusion referred to in Article 24 paragraph 1 item 22 of the PPL) (according to the template attached to the ToR);

g) the contractor's statement on the non-delivery of a final conviction judgment restriction of liberty or a fine to the extent specified by the awarding entity on the basis of art. 24 sec. 5 points 5 and 6 of the PPL (vg) constituting an attachment to the SIWZ);

h) a statement by the contractor about the lack of a final administrative decision against him under breach of labor law, environmental law or social security regulations to the extent specified by the contracting authority under Art. 24 sec. 5 point 7 of the PPL (according to the template constituting an appendix to the ToR);

j) statementcontractors on not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716 with later amendments) (according to the template constituting the attachment to SIWZ);

If the contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to above, he submits the documents indicated in the Regulationof the Minister of Development

26/07/2016 regarding the types of documents that can be requested from the contractor, their validity period and the forms in which these documents can be submitted (Journal of Laws of 2016, item 1126).

In order to confirm the lack of grounds for exclusion from the procurement procedure in the circumstances referred to in art. 24 sec. 1 item 23 of the PPL, the Contractor will be obliged to submit a statementof belonging or not belonging to the same capital group
