The subject of the orders are services in the field of forest management in accordance with the definition in art. 6 par. 1 point 1 of the Act of 28.9.1991 on forests (consolidated text: Journal of Laws of 2017, item 788 with further amendments: "Law on forests") covering works in the field of forest breeding and protection, protection againstfire, harvesting and skidding of wood as well as mechanical soil preparation and grinding of grit residues, to be carried out in the Lipinki Forest District in 2019. The subject of the order is forest management services as defined in art. 6 par. 1 point 1 of the Act of 28.9.1991 on forests (consolidated text: Journal of Laws of 2017, item 788 with further amendments: "Law on forests") covering works in the field of forest breeding and protection, fire protection, harvesting and skidding of timber to be carried out in the area of the Forest DistrictLipinki in 2019. The material scope of the order includes total quantities of works in the field of forest breeding and protection, fire protection, harvesting and skidding of wood resulting from Annex 3 to the Specification of Essential Terms of Order (SIWZ), which consists of Annex No. 3.1. the size of works by activity, attachment no. 3.2. zones of difficulty and distance of skidding, attachment No. 3.3 assortment system for timber harvesting and attachment no. 3.4, protection of deposits and sublots. A detailed description of the works (activities) included in the subject of the order has been included in Annex 3 to the SETC respectively. Property ranges summarized in Annexes 3.1.-3.4. they are estimates. A detailed description of the technology of executing individual forestry works (activities)-the subject of the order has been included in Annex 4 to the ToR. The descriptions of the codes describing the individual works (activities) included in the subject of the order are described in Annex 4 to the ToR. The description of the procedure for the acceptance of works has been included in Annex No. 5 to the ToR.

The Ordering Party does not make any specific requirements in the description of compliance with this condition

In order to confirm the absence of grounds for exclusion from the proceedings and to confirm compliance with the conditions for participation in the proceedings, the Contractor will be required to provide the Employer with the following declarations and documents: a) the Contractor's statement as at the day of submitting bids constituting a preliminary confirmation of fulfillment of the conditions of participation in proceedings specified in the ninthSIWZ and no grounds for exclusion, submitted on the form of a single European order document (hereinafter referred to as "EAT");

b) excerpt from the relevant register or from central records and business information, if separate regulations require entry in the register or records, in order to confirm the absence of grounds for exclusion under Article 24 (5) (1) of the PPL;an appropriate head of the tax office confirming that the contractor is not in arrears with paying tax; d) a certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions; ) information from the National Register of Kain the range specified in art. 24 sec. 1 items 13, 14 and 21 of the PPL and, regarding a conviction for a prison sentence, to the extent specified by the ordering party under art. 24 sec. 5 point 5 and 6 of the PPL;

f) the contractor's statement that the court has not been the subject of a final court decision or a final administrative decision on default in the payment of taxes, fees or social insurance premiums;or in the event of such a verdict or decision, documents confirming the payment of these claims together with any interest or fines or the conclusion of a binding agreement regarding repayment of such debts (with reference to the exclusion referred to in Article 24 paragraph 1 item 15 PPL) ;

g) the contractor's statement about the lack of a ruling against him in the form of a measurea) a preventive prohibition of applying for public orders (in relation to the exclusion referred to in Article 24 paragraph 1 point 22 PPL); h) a statement by the contractor about the lack of a final conviction judgment for a restriction of liberty or a fine within the scope specified by the ordering party on the basis of art. 24 sec. 5 points 5 and 6 PPL;

i) the contractor's statement about the lack of edthe final administrative decision on violating the obligations under labor law, environmental protection law or social security provisions in the scope specified by the awarding entity pursuant to art. 24 sec. 5 point 7 of the PPL;

j) the contractor's statement on the non-payment of taxes and local fees, referred to in the Act of 12 January 1991 ontaxes and local fees.

In order to confirm the lack of grounds for exclusion from the procedure for awarding the order in the circumstances referred to in art. 24 sec. 1 point 23 PPL The Contractor will be required to submit a declaration on belonging to or not belonging to the same capital group. Detailed information on documents submitted by contractors includes graying, including talso information on documents submitted by contractors having their registered office or place of residence outside the territory of the Republic of Poland.

Due to the limited number of signs, detailed information can be found in chapter 7 of this document.

For confirmation of participation in the proceedings regarding financial situation:

1) information on financial resources or creditworthiness of the contractor, which should be provided in Part IV (a) B (point 6) EAT;

2) information of the bank or cooperative savings and credit union confirming the amount of financial resources held or creditworthiness of the contractor in the period not earlier than one month before the deadline for submission of tenders. documents regarding the financial situation or erequired for the contracting authority, may submit another document, which sufficiently confirms that the condition of participation in the procedure described by the contractor is met. Grounds for exclusion from the proceedings: art. 24 sec. 5 p. 1 PPL, art. 24 sec. 5 point 2 of the PPL, art. 24 sec. 5 point 4 of the PPL, art. 24 sec. 5 point 5 PPL, art. 24 sec. 5 point 6 of the PPL, art. 24 sec. 5 item 7 of the PPL, art. 24 sec. 5 point 8-PPL.

2. The Ordering Party requires a deposit to be paid in the amount specified below for the Packages: for Package I in the amount of PLN 87, 000.00; for Package II in the amount of PLN 71, 000.00; for Package III in the amount of PLN 64, 000.00; for Package IV in the amount of PLN 10, 000.00; for Package V in the amount of PLN 44 000.00. The bid security should be submitted before the deadline for submission of offers. Detailed information on the bid securityand ch. 9 siwz.

3. When choosing offers on a given Package, the Ordering Party will be guided by the following criteria of offer evaluation:

a) for Packages I, II, III, IV

- Price 60%,

- Independent implementation of key elements (parts) orders 40%.

B) for Package V

- Price 60%,

- Independent implementation of key elements (parts) orders 20%

- The age of the forest harvester machine 20%.

Szcdetails on the criteria for the evaluation of offers are described in ch. 14 siwz.

4. The proceedings are conducted in accordance with the rules provided for the so-called "reversed procedure" referred to in Article 24aa (1) and (2) of the PPL. Pursuant to the provisions cited, the Awarding Entity will first evaluate the offers, and then examine whether the contractor whose offer has been assessed as the most advantageous, it can not be excludedand meets the conditions for participation in the proceedings.

5. The evaluation of compliance with the conditions of participation in the proceedings will be made in accordance with the formula "meets" / "does not meet", based on information contained in documents and statements submitted by the Contractors referred to in Chapter 7 of Siwz.

6. The ordering party requires a performance bond to be provided by the contractor whose offerconsidered the most advantageous in the given Package. The security for proper performance of the contract will amount to 5% of the total price offered for the Part (Package) contract. Detailed information on the performance bond is provided in Chap. 17 siwz.

7. Deadline for order fulfillment: from the date of conclusion of the contract until 31/12/2019. If the contract is concluded earlier than beforeon January 1, 2019, the order will be processed from January 1, 2019. Deadlines for the completion of individual works (in each Package) will be determined in Orders placed by the Purchaser's Representatives. In the case of contractors jointly applying for the award of the contract, the conditions for participation in the procedure set out in point 6.2. Everyone should meet Siwz togetherthe prisoners, with the reservation that the condition described in item 6.2, subsection 3) lit a) siwz should meet at least one of these contractors. None of the contractors jointly applying for the award may be excluded from the procedure. If the Contractor is applying for a contract for several Packages, he can not demonstrate the fulfillment of the conditions for participation in the proceedings.experience (the same amount of processing resulting from a given contract), the same persons foreseen for the order, subject to the surveyor and supervisor for package no. V (wood acquisition with multi-operational machines and logging), or the same financial potential in more than one package.

10. The Contracting Authority anticipates submitting partial tenders.

11. When orderingy does not provide for the conclusion of a framework agreement.

12. The Contracting Authority does not provide for an electronic auction.

13. Proceedings are not conducted to establish a dynamic purchasing system.

14. The Awarding Entity does not provide for the submission of variant bids
