Subject orders are forest management services, covering the development of four office plan designsforest management, site development and five annexes to the forest management plan.
Main Site: Land of Kędzierzyn Forest District.
The subject of the contract is to develop a forest management plan with an environmental impact forecast for Kędzierzyn Forest District for 2021-2030, as per 1.1. 2021
Contractors who are not subject to exclusion from may apply for the orderproceedings in the circumstances referred to in art. 24 sec. 1 item 12-23 and in art. 24 sec. 5 point 1 of the Public Procurement Law, and meet the conditions in the procedure specified in the ToR. To confirm that the conditions for participation have been fulfilled and no grounds for exclusion for the above reason - the contractor will submit a statement in the form of an EAT. The JEDZ declaration must be submitted before the deadline for submission of offers, only in the form-electronic, with a qualified electronic signature to the address: [email protected] - described in chapter 8 of the ToR.
The Contractors shall submit:
1 as proof of the lack of grounds for exclusion. A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the lack ofthe exclusion pond based on art. 24 sec. 5 point 1 Pzp;
2. Certificate of the competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued no earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has concluded an agreement with the competent tax authority regarding the repayment of these receivables, includingwith interest or fines, in particular obtained legally required exemption, postponement or spreading into installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;
3. Certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears-with payment of social security or health insurance premiums, issued no earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has concluded an agreement with the competent authority regarding repayment of such receivables with possible interest or fines, in particular obtained the legally required exemption, postponement or spreading into installments depending onmajor payments or suspension of the implementation of the decision of the competent authority in full;
4. Information from the National Criminal Record in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;
5. Declaration of the Contractor about the lack of issuing a final court judgment or final administrative decision on his resignation to himwith payment of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision, documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement regarding repayment of such debts (with respect to the exclusion referred to in art 24 section 1 item 15 PPL) - the companyzone on the SIWZ Annex,
6. Declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for public orders (in relation to the exclusion referred to in Article 24 paragraph 1 item 22 PPL) - drawn up in the Annex to the ToR.
7. Declaration of belonging or non-affiliation to a capital group - drawn up on an annex to the ToR - DeclarationsNot all entities belong to the capital group within 3 days of placing the information on the website on opening the offers (Section VII point 6 of the ToR)
The remaining valid statements and documents mentioned above will be required from the Contractor who submitted the most advantageous offer. The Contractor will be obliged to submit it within the deadline specified in the summons, not shorter than10 days. - Foreign contractors submit a document according to provisions contained in Section VII, point 9 of the ToR. Contractors jointly applying for the order - by provisions contained in section VII, point 8 of the ToR. Contractor relying on the resources of other entities - according to the provisions contained in Section VII point 7 of the ToR.
The Contracting Authority will recognize that the Contractor is in a financial position ensuring the implementation of thisorders if it proves that it has financial resources or creditworthiness in the amount not less than:
1) Part I - for the Task No. 1 PLN 100, 000.00,
2) Part II - for the Task No. 2 PLN 100, 000.00,
3) Part III - for Task No. 3 PLN 100, 000.00,
4) Part VI - for the Task No. 4 PLN 100, 000.00,
5) Part V - for the Task No. 5 PLN 50, 000.00,
6) Part VI - for ZadaPLN 6, 500.00,
7) Part VII - for the Task No. 7 PLN 5, 000.00,

Note: The Contractor submitting the bid for the performance of several or all of the Sections must show that he has the financial resources or creditworthiness in the amount ofci being the sum of the amounts indicated above, for the individual Parts for which the offer was made. The ordering party requires a bid bond in the amount specified below for individual Parts:

2. The bid bond should be submitted before the deadline for submitting bids.
3. The form of payment of the bid bond and the terms and conditions of returning the Wadium were written in section IX of the ToR.
Infor clauseconcerning the processing of personal data.
1. According to art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (general regulation on the protection of personal data) (Official Journal of the EU L 119 from 04 May 2016, page 1 hereinafter "RODO") The Contracting Authority informs that the administrator of personal data is PGL LP Regional Directorate of State Forests in Katowice.
2. Personal data will be processed pursuant to Article 6 paragraph 1 lit. c RODO in order to relate to the conduct of this procedure for the award of a public contract and its resolution, as well as if they fail to meetE-mails specified in Article 93 para. 1 PPL in order to conclude a public procurement contract and its implementation, as well as to document the proceedings for the ordering and archiving of the contract.
3. Recipients of personal data will be persons or entities whose documentation of proceedings will be made available on the basis of art. 8 and 96 para. 3 PPL.
4. Personal data obtained in connection with the conductof these proceedings for the public order will be stored, in accordance with art. 97 ust. 1 of the PPL, for a period of 4 years from the date of completion of the public procurement procedure, and if the duration of the contract exceeds 4 years, the retention period covers the entire duration of the contract - On public order.
5. Notwithstanding the provisions of item 4 above, in the case of conclusion-contracts on public order, personal data will be processed until the expiration of the period of limitation of claims under the contract for public order.
6. The personal data obtained in connection with the conduct of this contract award procedure may be transferred to entities providing consulting services, including legal and consulting services,

8. The person concerned with the personal data obtained in connection with the conduct of these proceedings has the right to: 1) access their personal data in accordance with art. 15 RODO,
2) to correct their personal data in accordance with art. 16 RODO,
3) to demand from the Employer as an administrator, processing restrictions-personal data, subject to the cases referred to in art. 18 para. 2 RODO.
4) file a complaint to the President of the Office for Personal Data Protection in the event that it considers that the processing of personal data violates the provisions on the protection of personal data, including the provisions of the GDPR.
9. The obligation to provide personal data is a statutory requirement set out in the provisions of the PPL related to participation in the proceedings-for granting a public order; the consequences of not providing specific data are defined by PZP.
