The subject of the order is the delivery of 2 pieces of the car laboratory for wifrom the radiology-biology groups in accordance with the requirements of Annex 1 to the ToR, detailed description of the subject of the order.
Main Site: Poznań, Warsaw.
The subject of the contract is the delivery of 2 pcs of the car laboratory for the leading radiology-biology groups in accordance with the requirements of Annex 1 to the ToR description of the subject of the order.
The subject of the order should meet the following requirements:
a) correspond to all the features specified in the Specification of Essential Terms of the Order;
b) be new and comply with applicable standards.
Full information on the subject of the order has been described in the Specification of Essential Terms of Order, including in particular Annex 1 and Annex 2 .
The laboratory (vehicle body) should be designed and constructed in accordance with the requirements ofagencies included in the nw. Documents:
1. Regulation of the Minister of Health of 22.4.2005 on harmful biological agents for health in the work environment and health protection of workers professionally exposed to these factors (Journal of Laws of 11.5.2005).
2. Regulation of the Minister of the Environment of November 29, 2002. On the list of pathogenic organisms and their classification, as well asdoses necessary for individual levels of airtightness. OJ 2002 No. 212 item 1798
3. Act of 5 December 2008 on preventing and combating infections and infectious diseases in humans. OJ 2003 No. 169 item 1650

6. Regulation of the Ministera of Labor and Social Policy of 26.9.1997 on general provisions of occupational health and safety. (Journal of Laws No. 169, item 1650, as amended).
The vehicle must be marked with the operating numbers of the State Fire Service in accordance with the ordinance No. 1 of the Chief Commander of the State Fire Service of 24.1.2020 regarding transport management in organizational units of the State Strafirefighting life. Designation data will be provided during the performance of the contract at the Contractor`s request. 24 paragraph 1 and pursuant to art. 24 paragraph 5 point 1 in relation to which liquidation was opened, in the arrangement approved by the court in restructuring proceedingsThe satisfaction of creditors is foreseen by the liquidation of his assets or the court ordered the liquidation of his assets pursuant to art. 332 paragraph 1 of the Act of 15.5.2015 Restructuring Law (Journal of Laws of 2017, item 1508) or whose bankruptcy was announced, with the exception of the Contractor who, after the declaration of bankruptcy, entered into an arrangement approved by a final court decision, if the arrangement does not provide forsatisfaction of creditors by liquidation of the assets of the bankrupt, unless the court ordered the liquidation of his assets under art. 366 section 1 of the Act of 28.2.2003 Bankruptcy Law (Journal of Laws of 2017, item 2 344); point 4 which, for reasons attributable to him, did not perform or improperly performed
To a significant extent the previous public procurement contract or concession contract concluded with Zamawianie, referred to in Article. 3 clause 1 point 1 4, which led to the termination of the contract or the award of damages; point 8, which has violated the obligations regarding taxes, fees or social security or health insurance contributions, which the Awarding Entity is able to demonstrate by means of appropriate evidence, except in the case referred to in art. 24 paragraph 1 point 15, unless the Contractor dhas made payments of taxes, fees or social security contributions due, together with interest or fines, or has entered into a binding agreement on the repayment of those debts.
1. In order to confirm the lack of grounds for exclusion and fulfillment of the conditions for participation in the procedure, the Contractor shall provide a uniform European declaration document as at the date of submitting the offersa, hereinafter referred to as "EAT", constituting Annex 5 to the ToR (template) according to the guidelines indicated in chapter XVI of the said specification. at most to confirm the circumstances referred to in Article 25 paragraph 1 item 1 and 3 of the Public Procurement Lawthe following documents:
1) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act, issued not earlier than 6 months before the deadline for submission of bids;
2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadlinesubmitting bids or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the execution of the decision authority;
3) certificate from the competent bodyerenowy organizational unit of the Social Insurance Institution or Kasa Rolniczego Ubezpieczenia Społecznego or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or another document confirming that the Contractor has concluded an agreement with properauthority regarding the repayment of these claims together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of overdue payments or withholding in full the decision of the competent authority.
The Employer does not require specific conditions in this regard. As a supplement to the point above, i.e.: excerpt from the relevant registertru or from the central register and information on business activity, if separate provisions require an entry in the register or the register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;
5) the Contractor`s statement that he has not issued a final judgment of the court or a final administrative decision regarding arrears with the payment of taxes, fees orsocial security or health insurance burden or in the event of the issuance of such a judgment or a decision of documents confirming the payment of these debts together with any interest or fines or the conclusion of a binding agreement on the repayment of these debts;
6) the Contractor`s statement that no decision has been issued against him as a preventive measure applying for the contractand public;
7) declarations of the Contractor on non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2018, item 1445);
8 ) list of knowledge and experience according to the requirements of Annex 4 to the ToR, together with documents confirming the proper performance of deliveries.
3. If the Contractor has its registered office or place of residence outside the territoryof the Republic of Poland, instead of:
1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,
- submits information from the relevant register or, in the absence of such register, another equivalent document issued by a competent judicial or administrative bodyaju, in which the Contractor has its registered office or place of residence or place of residence, has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act issued not earlier than 6 months before the submission of tenders.
Instead of:
2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxeskow, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferral or distribution provided for by law installments of late payments or withholding in fulland implementation of the decision of the competent authority;
3) a certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social or health insurance contributions, issued not earlier than 3 months before the deadline for submission offers, or other todocument confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferral or installment of overdue payments provided for by law or withholding in full the decision of the competent authority;
4) an extract from the relevant register or from central records and informationon business activity, if separate provisions require an entry in the register or a register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;
- submits a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming respectively that:
List of statements and documents confirming the lack of grounds for exclusionThe terms and conditions of participation in the proceedings have been described in the Specification of Essential Terms of the Contract, in particular in Chapter IX, X and XI. The proceedings are conducted in Polish in electronic form via the purchasing platform at: https://platformazakupowa.pl/pn/straz. Communication between the Employer and Contractors, including all statements, applications, notificationsproperty and information are transferred in electronic form via the platform. The date of submission (receipt) of declarations, applications, notifications and information is the date they are sent via the platform by clicking the "send message" button followed by the message that the message has been sent to the Employer. The Employer with Contractorswill provide information in electronic form via the platform. Information on answering questions, amendments to the ToR, changing the deadline for submitting and opening offers will be posted by the Ordering Party on the platform. Correspondence which, in accordance with applicable regulations, is addressed to a specific Contractor will be forwarded in electronic form via the platform to this endspecific Contractor. Full information on the manner of communication and hardware / network requirements have been indicated in the Specification of Essential Terms of Order, in particular in Chapter XIV.
The offer should be:
a. prepared on the basis of the attachments to this ToR in Polish;
b. submitted in electronic form via the platform;
c. signed qualificationelectronic signature by a person / persons authorized / authorized to represent. Note: The offer should be accompanied by current documents confirming the legal status of the Contractor, e.g. an excerpt from the relevant register or from the central register and information on business activities. The offer does not have to contain these documents if the Contractor indicates in the offer that they are available in the forme electronic at specified internet addresses of publicly available and free databases. The authorization of the persons signing the offer must result directly from the abovementioned documents
If such authorization does not result directly from a document confirming the legal status of the Contractor, the offer should be accompanied by an appropriate power of attorney, established to represent the Contractor (s) applying forpublic procurement.
The exact manner of preparing the offer is described in the Specification of Essential Terms of the Contract, in particular in Chapter XVI.
The Employer provides for the possibility of using the so-called "Reverse procedure", referred to in Article 24aa (1) of the Public Procurement Law.
