Payment of retirement and disability pensions in the form of cash directly to the hands of beneficiaries at their place of residence

The subject of the order is delivery in the form of cash benefits retirement and disability pensions and short-term benefits (including sickness benefits, maternity, one-time compensation, funeral) from social insurance for farmers granted by KRUS organizational units to the benefit of beneficiaries in their place of residence in the form of a remittance, provided that the provision (payment) of the benefit to eligible persons will consist in the delivery of the benefit to the address indicated by the Employer, hereinafter referred to as "transfer".

Subjectthe order is for cash delivery of retirement and disability benefits and short-term benefits (including sickness, maternity, one-off compensation, funeral benefits) from farmers` social insurance granted by KRUS organizational units to the beneficiaries at their place of residence in the form of a remittance, provided that the provision (payment) of the benefitentitled persons will rely on the service being delivered to the address indicated by the Ordering Party, hereinafter referred to as "transfer". The estimated total amount of pension benefits paid in the form of cash to the hands of beneficiaries at their place of residence during the 36 months of the contract is about 18, 556, 100 items of benefits with a total estimated value of pension benefitsabout PLN 24, 122, 976, 800 paid by the Employer. The estimated total number of short-term benefits paid in the form of cash to the hands of beneficiaries at their place of residence during the 36 months of the contract is about 532, 800 items of benefits with a total value of short-term benefits paid about 181.152.000 PLN implemented by the Employer.

Employer ndoes not guarantee a fixed number of transfers during the period of the contract. The benefits will be implemented in the form of an EPE, an electronic retirement transfer, and in the form of a PE, traditional payment transfer, provided to the Contractor by KRUS organizational units as the issuer of the paper transfer form. Collection points for advised benefits should be located throughout the country.In the case of short-term benefits, pickup points from the Employer should be located in the cities where the organizational units of the Employer are located. A detailed description of the subject of the contract is provided in Chapter II of the ToR Model Agreement.

Contractors who can apply for the order:

A. are not subject to exclusion pursuant to art. 24 paragraph 1 mouthyou Pzp; In the case of an offer submitted jointly by several Contractors, the assessment of the above requirements will be made for these Contractors separately.

Required documents confirming non-exclusion:

1. information from the KRK in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Pzp;

1.1. Contractor based in the territory of the Republic of Poland, in relation to a residentand outside the territory of the Republic of Poland, to which the document referred to in item 1 applies, submits the document referred to in item 4, to the extent specified: in art. 24 paragraph 1 points 14 and 21 of the Act. If, in the country in which the person to whom the document was supposed to reside, does not appear such documents, it is replaced by a document containing the person`s declaration made before a notary public orb before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person. 2. the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or - in the event of sucha judgment or decision of documents confirming the payment of these debts, together with any interest or fines, or the conclusion of a binding agreement on the repayment of those debts; 3. the Contractor`s statement that he has not been awarded a preventive measure against his bid for a public contract. 4. If the Contractor has its registered office or place of residence outside the territoryof the Republic of Poland, instead of the documents referred to in item 1, submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence or place of residence. person to whom the information or document relates, to the extent specified inArt. 24 paragraph 1 item 13, 14 and 21 of the Act. 5.If in the country in which the Contractor has its registered office or place of residence or place of residence there is a person to whom the document relates, the documents referred to in item 4 do not appear, they shall be replaced by a document containing the Contractor`s statement, indicating the person or persons authorized to represent him, or a statement by the person dthe document was to apply to a notary public or to a judicial, administrative or professional or economic self-government body competent for the registered office or place of residence of the Contractor or the place of residence of that person. The requirements for the date of issue of the document set out in 4 shall apply accordingly.

The documents referred to in points 1-4 should be issued notmore than 6 months before the submission deadline.

B. meet the conditions regarding competence or entitlements to conduct specific professional activities, provided that it results from separate provisions: The Awarding Entity shall consider the condition fulfilled if the Contractor demonstrates that it has the postal operator`s rights to perform postal orders in accordance with the Postal Law of 23 listadada 2012 (Journal of Laws of 2018, item 2188, as amended) or the right to perform remittances, within the meaning of the Act on Payment Services of 19 August 2011. (Journal of Laws of 2019, item 659, as amended). To this end, the Employer will request a document confirming that the Contractor has the postal operator`s rights to perform postal orders, in accordance with Act Praw letter from November 23, 2012 (Journal of Laws of 2018, item 2188, as amended) or the right to perform remittances within the meaning of the Payment Services Act of 19 August 2011. (Journal of Laws of 2019, item 659, as amended).

When ordering, he will request from the Contractor a document confirming that the contractor is insured against civil liability in the scope ofrelated to the subject of the contract for the guarantee sum specified below.

1. The contractor submits the offer via the Purchaser`s purchasing platform. The offer should be accompanied by a current declaration form in the form of a Single European Procurement Document according to the Commission Implementing Regulation (EU) 2016/7 of January 5, 2016, as at the date of submission of tenders. establishing the standardsource form for the single European order document (Official Journal EU L 3/16), hereinafter referred to as the single document. The information contained in the statement constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with themto the extent that it refers to their resources, the conditions for participation in the proceedings, it also submits uniform documents regarding these entities. The contractor who intends to entrust the performance of part of the contract to subcontractors in order to demonstrate the absence of grounds for exclusion from participation in the proceedings against them, also submits uniform documents regarding these entities and will be required topresent the documents indicated in section III.1.1) related to these entities. In the case of joint application for the Contract by the Contractors, a single document shall be submitted by each of the Contractors jointly applying for the contract. This document must confirm compliance with the conditions for participation in the proceedings and the grounds for exclusion to the extent that each of the Contractors demonstrates compliance with the conditionsow to participate in the proceedings and no grounds for exclusion. With regard to Part IV of the Single European Procurement Document Qualification criteria, Section α - the Contractor may limit himself to submitting a general statement on meeting the conditions for participation in the procedure and not to complete further fields regarding the detailed conditions of participation in the procedure specified by the Employer.

2.The Contractor is obliged to lodge a security deposit: PLN 2, 000, 000 (in words: two million zlotys) before the bid submission deadline.

3. The offer binding period is 90 days.

4. The Contractor, within 3 days from the date of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, will provide the Employer with a declaration of belonging to the same capital groupj within the meaning of the Act of February 16, 2007. on competition and consumer protection. In the case of belonging to the same capital group, the Contractor may submit, together with a statement, documents or information confirming that links with another Contractor do not lead to distortion of competition in the procurement procedure.

5. Requirements for employment by the Contractor or Subsidiaryon the basis of an employment contract referred to in art. 29 paragraph 3a of the Public Procurement Law, persons performing actions indicated by the Employer in the field of order implementation, are specified in the Contract template in Chapter II of the ToR.

6. Employer, in accordance with art. 24aa section 1 of the Act, will first evaluate the offers and then examine whether the Contractor whose bid was assessed as the most advantageous, notis subject to exclusion and meets the conditions for participation in the proceedings.

7. The selected Contractor shall provide a security on due performance of the contract in the amount of 5% of the gross total price stated in the offer, at the latest on the day of signing the contract.

8. The Awarding Entity does not provide for the possibility of awarding contracts referred to in art. 67 section 1 item 6 of the Public Procurement Law
