Deinstitutionalization of ZpS-DD Santovka Intelligent systems and common control platform for control and synchronization of equipment and sensors is based on delivery of intelligent system, delivery of technologyintegration, control platform synchronization, control platform tuning for control and synchronization of device and sensor file, operator training, user and technical documentation development, provision of technical support within the test operation, management, maintenance, technical support and intelligent service delivery .

The subject of the contract is the execution of construction work, which consists of the construction of 2 detached barrier-free family houses incorporated into the housing estate in the village of Santovka, street Ružova č. 26 and Horna no. 18. In a family house on ul. Horna č. 18 is planned to accommodate 12 social service recipients. Family house on the street Ružova no. 26, will occupy 12 clients who are involved in the transformation process and will be moved from the accommodation pavilion and the mansion premises. Family house SO.01-A (Horna ulica) will be installed parallel to the existing building, about 9.5 m away. The family house SO.01-B (Ružova Street) will be located approximately 2.2 m from the southern boundary of the plot, parallel to the boundary of the plot. Farmhouse will be installed parallel to the fence in1.4 m from Horna Street. The main entrance for pedestrians to the family house SO.01-A is from Horna Street through a pavement on the south side of the building. The family house SO.01-B is from the street Ružova through the access pavement also from the southern side. The pub building is accessible from Horna Street. The access to the parcels and the building of the family house SO.01-A and the farm building is through the paved road Horna Street. Do rohouse SO.01-B is the main entrance from the street Ružova. Parking places will be provided individually for each building. In the pub building I will create 3 garage articles for cars up to 3.5 tons. Parking spaces will be 2.5 x 5.0 and 3.5 x 5.0 for people with reduced mobility. The construction of buildings is based on the application of the principle of universal design of buildingsin the natural social environment, which will meet the organizational and cultural conditions of the family type environment. Family houses are designed as SMART-house, which ensures clients safe housing, sufficient privacy and allow them independent individual life by providing innovative and quality social services in the field, outpatient and residential services. The main innovation in the use of smArt technology is to ensure the extension of services, their flexibility, combinability between individual services in order to ensure a more valuable life for the client and his involvement in the events in the community. The project documentation for the construction of 2 family houses and a farm building is prepared in accordance with the valid legislation of the Slovak Republic and the document of the Implementing Agency of the Ministry of Labor and Social Affairs of the Slovak Republicunder the title Universal design of objects of community social services.

A public tender may be attended by a candidate or tenderer who meets the conditions of participation relating to personal status, submits documents or information and fulfills the formalities necessary to evaluate the fulfillment of the conditions for participation. Applicant for part no. 1 and part no. 2 of the subject of the contract must meet the teaching conditionsaccording to - 32 par. The applicant shall prove their fulfillment according to - 32 par. 2, respectively. ods. 4 or para. (5) The Tenderer or candidate shall not be obliged to submit the documents if the contracting authority is entitled to use data from the public administration information systems pursuant to a special regulation. If the tenderer or candidate fails to submit the documentaccording to - 32 par. 1 pism. (a) is obliged to provide the contracting authority with the data necessary for requesting an extract from the criminal record for the purpose of demonstration. The contracting authority shall state in the contract notice that the documents referred to in point 2 are not submitted due to the use of data from the public administration information systems. 91 /2016 Coll. on Criminal Liability of Legal Persons and on the Amendment and Supplementation of Some Conclusions as of 1 July 2016, the applicant is an economic entity with the status of a legal person for the purpose of demonstrating personal status pursuant to Section 32, Par. 1 pism. a) The ZVO is also obliged to submit an extract from the criminal record for a legal person issued by the General Prosecutor`s Office of the Slovak Republic, not older than 3 months, if the document is not registered in the Listand 5. economic operators. The contracting authority shall recognize an equivalent registration or confirmation of registration issued by the competent authority of another Member State, by means of which the tenderer demonstrates that the conditions for participation in the procurement procedure have been met. The contracting authority shall also accept any other equivalent document submitted by the tendererm. If the entry in the list of economic entities does not cover the conditions for participation in personal status laid down in - 32 para. 1 Procurement Act, the tenderer shall prove these facts by means of a separate document proving the required condition of participation issued by the relevant institution.in editable form in PDF format, signed electronically by the issuing institution. If the applicant submits to JED, it shall submit this document in a non-editable form in PDF format. The documents shall be stated in the Cover Sheet of the tender. 3 The Public Procurement Act imposes a fine on the tenderer from EUR 1 000, 00 to EUR 10 000, 00 and prohibits participation inpublic procurement for a period of three years, if it submits information or evidence that is false or altered or falsified, that they do not correspond to the facts and have or could affect the assessment of personal status, financial and economic status or technical or professional competence in the public procurement
