The subject of the contract is the execution of a construction project which also serves as the executive documentation for the construction of gas pipelines in urban areascości Przybyszewo, Lasocice commune Święciechowa along with obtaining all the required opinions, permits and decisions, including obtaining a building permit decision with a last resort clause, taking the bill of quantities, investor cost estimates (Project) and exercising author`s supervision.

The subject of the contract is the execution of a construction project that also serves as the executive documentation for the construction of a / c gas pipeline in the town of Przybyszewo, Lasocice, commune Święciechowa along with obtaining all required opinions, permits and decisions, including obtaining decisions pospreferences for construction with the last resort clause, taking the bill of quantities, investor cost estimates (Project) and exercising author`s supervision.

I. The Contractor, within 3 days of posting on https://zamowienia.psgaz.pl beforeinformation proceedings referred to in art. 86 section 5 of the Act, submits a declaration of belonging or not to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Act (template annex No. 2a to the ToR) in the form described in detail in point 8.27. Terms of Reference.

II. Documents submitted at the request of the Employer by the Contractor whose tender was highest rated, submitted insignificant period (not shorter than 10 days) in accordance with point 8.14 of the Terms of Reference, valid as at the date of their submission, confirming the circumstances referred to in art. 25 paragraph 1 of the Act, it is:

1. Information from the KRK to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids (subject to the content of Article 133 (4) of the Act);

2. certificatethe competent head of the tax office confirming that the Contractor is not in arrears with taxes, issued no earlier than 3 months before the deadline for submitting bids, or another document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these debts together with any interest or fines, in particular he obtained predictionsrelease, postpone or installment of overdue payments or suspend in full the execution of the decision of the competent authority by law;

3. Certificates of the relevant local organizational unit of ZUS or KRUS or another document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadlinesubmitting bids or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the decision of the competent authority;

4. Copy from the relevant register or from CEntral records and information on economic activity, if separate provisions require an entry in the register or the register, in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Act;

5. Contractor`s declarations of non-issue of a final court judgment or final administrative decision regarding late payment of taxes, fees or insurance premiumssocial or health services, or in the event of such a judgment or decision making documents confirming the payment of these debts together with any interest or fines, or the conclusion of a binding agreement on the repayment of those debts;

6. Declarations of non-judgment against him as a precautionary measure forbidding public procurement;

7. the Statementsia about non-payment of taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and fees (Journal of Laws of 2016, item 716). The statement referred to in item 5 7 may be made out on the model acc. annex 2 to the ToR;

8. List of services rendered, and in the case of periodic or continuous services also performed, in the last 3 years before the end of the termthe bid submission deadline, and if the business period is shorter in that period (Annex 5 to the ToR);

9. Evidence (references or other documents) confirming whether the services have been performed and, in the case of periodic or continuous services, also performed within the last 3 years before the deadline for submission of tenders, listed in JEDZ and the list have been performed or are performed onare lying;

10. List of persons directed by the Contractor to carry out the public contract (Annex 4 to the ToR)
