The subject of the order is the preparation of design documentation of a medium pressure gas pipeline dn 90, length about 370 m in Bielsk Podlaski ul. Mickiewicz, province Podlasie.

The object of the contract is to prepare project documentation of a medium pressure gas pipeline dn 90 length about 370 m in the city of Bielsk Podlaski ul. Mickiewicz, province Podlasie.

The subject of the contract includes, in particular:

1 Construction design, detailed design and author`s supervision for the medium pressure DN 90 gas pipeline, length about 370 m, in m. Bielsk Podlaski, ul. Mickiewicz. Shut-off fittings should be designed on the gas pipeline

2 Obtaining all opinions, permits and decisions on behalf of and for the Employer, from the competent authority, obtaining the final building permit decision.

3. Under the contract, the Contractor shall provide complete design documentation, a bill of quantities and an estimate

4. The contractor will be obliged to agreeof the project in the appropriate unit of the Employer.

2. They meet the following conditions for participating in the proceedings.

2.1 They have the competence or authority to conduct specific professional activities, provided thatit results from separate provisions

2.3 They have technical or professional capacity - specified in point III.1.3 of the Advertisement The contractor may, in order to confirm compliance with the conditions for participation in the procedure, in relevant pand, in relation to a specific contract, or part thereof, rely on the technical or professional abilities or financial or economic situation of other entities, regardless of the legal nature of the legal relations with it.

4. Contractors may jointly apply for the contract.

II. The Employer excludes the Contractor from the contract award procedure on the basis of prdeputies referred to in art. 24 paragraph 1 point 12-23 Public Procurement Law and art. 24 paragraph 5 Public Procurement Law, subject to art. 133 section 4 Public Procurement Law and art. 24 paragraph 7-12 of the Public Procurement Law

The assessment of compliance with the above conditions will be made in accordance with the formula "meets does not meet", based on the information contained in the single document and documents to be provided by the Contractor to confirm compliance with the conditions for participation in the procedure.

IIIList of declarations and documents to be provided by the Contractors to confirm the fulfillment of the conditions for participation in the procedure and the absence of grounds for exclusion.

Documents and statements submitted together with the offer by all Contractors

Together with the offer constituting Annex No. 1 to the ToR, the Contractor submits valid as at the date of submission of the offers a statement to the extent indicated in the ToR in the form of a uniform document (Annex 2 to the ToR) in compliance with the requirements set out in points 8.2.1- 8.2.9 of the ToR and documents referred to in points 8.1.2- 8.1.6 of the ToR, i.e.

1. In the case of contractors jointly applying for granting orders (in the form of a consortium, civil partnership), The single document is submitted by each of the contractors jointly applying for the order in compliance with the requirements set out in pointh 8.2.1 - 8.2.9 SIWZ. These documents confirm the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion

To the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion.

2. Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with them in the scope of,

The Employer informs that he will carry out the procedure in accordance with Article. 24 aa PZp, which means that it will first evaluate the offers and then examine whether the Contractor whose offer has been assessed as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.

1. Information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act and, regarding conviction for an offense to imprisonment, to the extent specified by the contracting authority pursuant to Art. 24 paragraph 5 points 5 and 6 of the Act, issued not earlier than 6 months before the offer submission deadline.

Subject to the content of art. 133 section 4 of the Act, itis the contractor is not subject to exclusion in the case referred to in art. 24 paragraph 1 point 13 lit. d, and in the case referred to in art. 24 paragraph 1 point 14, if the person referred to in this provision has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. d.

2 Certificates from the competent head of the tax office confirming that the contractor is not in arrears with payment of taxes, not earlier than 3 months before the deadline for submission of bids, or another document confirming that the contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or payment in installments payments or withholding in fulland a decision of the competent authority;

3 Certificates of the competent organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids, or other document afterclaiming that the contractor has entered into an agreement with the competent authority on the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferral or installment of overdue payments provided for by law or withholding in full the decision of the competent authority;

4 Copy from the relevant register or from central records and information about activitieseconomic establishment, if separate provisions require an entry in the register or a register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Act;

5. Contractor`s statements in accordance with Annex 4 to the ToR.

6. List of persons specified in point 6.1.2.3 lit. and the Terms of Reference, directed by the Contractor to carry out the contract, responsible for the provision of services, together with infoinformation on their professional qualifications, entitlements, experience and education necessary to perform the order, as well as the scope of their activities, and information on the basis for their disposal, according to the template constituting Annex 5 to the ToR.

7. List of services performed, referred to in point 6.1.1.3 lit. b SIWZ, prepared according to the model constituting the Annexand no. 6 to the Terms of Reference.

To the list should be attached evidence that these services have been performed properly, the evidence referred to are references or other documents issued by the entity for whose services they were performed, and if for a justified reason of an objective nature, the contractor is unable to obtain these documents by the contractor`s statement
