The subject of the contract is the supply of new non-refurbished medical technology (laparoscopic tower, ureterorenoscopes, ultrasonic lithotriptor, laserovy generator, etc.) for the University Hospital with Policlinic Skalica, a. s., including delivery-related services, complete installation, operation, operator training, warranty, post-warranty service, removal and disposal of packaging materials for the delivered goods

Brief description of the subject for- a detailed description of the subject of the contract is given in the descriptive documents

1.1. 1 Act No. 343/2015 Coll. on Public Procurement and on the amendment of certain Conclusions as amended (hereinafter referred to as the Conclusions).haszač proves the fulfillment of the conditions of participation concerning personal status according to - 32 par. 1 of the Act, documents pursuant to - 32 par. 2, respectively. according to - 32 par. 4 and 5 of the Act. According to - 32 par. 1 pism. (e) the law requires the tenderer to show that he is entitled to supply the goods covered by this contract. At the same time, the criminal integrity of a legal person is required, according to Act No. 91/2016 Coll. on criminal liability

1.2 The document pursuant to - 32 para. 2 pism. e) it is prohibited by reason of the use of data from public administration information systems pursuant to a special regulation (Act No. 177/2018 Coll. on certain measures to reduce administrative burdens by using public administration information systems and on amending and supplementing certain conclusions (ending against bureaucracy)(i) need not submit. In case of participation conditions pursuant to - 32 para. 2 pism. e) The above rule applies to economic operators (applicants) listed exhaustively in - 2 para. 2 Act No. 272/2015 Coll. on the Register of Legal Persons, Entrepreneurs and Public Organs and on the amendment of certain conclusions. In all other cases, the tenderer is still obliged to present a document proving the fulfillment of the contract1.3 The applicant registered in the list of economic entities under the Act is not obliged to submit documents pursuant to - 32 para.

1.4 The person referred to in - 8 of the Act shall recognize an equivalent entry as entered in the list of economic entities under the Act.or a certificate of registration issued to the competent authorities of another Member State, by which the tenderer proves that the conditions for participation in the procurement procedure have been fulfilled. The person pursuant to Article 8 of the Act shall also accept other equivalent documents submitted by the applicant.Demonstration of the conditions of participation is also in accordance with - 8 of the Act to the person according to - 152 par. 4 of the Act

1.6. Fulfillment of participation requirement according to - 32 par. 1 pism. (e) the law proves the law of a group member only in respect of that part of the subject of the contract which he has

Conditions of participation in public procurement pursuant to - 33 of the Act concerning financial and economic status:

2.2 - 33 para. 1 pism. D) Act - summary of total turnover for the last 3 marketing years, for which they are available depending on the establishment or commencement of the operation of the activity. 1 et seq. Act.

Any person who receives the tender documents without registration without registration under - 8 of the Act does not have records - about the publication of additional information and explanation of the tender documents will not be informed. For the purposes of electronic communication, each candidate / applicant is obliged to registergo to Eranet at URL: https://obstaravanie.eranet.sk. 6. Minimum technical requirements for using the Eranet information system:

8. Explanation of information neededFor the purposes of drawing up the tender and demonstrating compliance with the conditions for participation, the person referred to in Article 8 of the Act shall notify without delay not later than 6 days before the expiry of the time limit for submission of tenders, provided Conditions for participation according to the notification, point III.1.1) Personal status, III.1.2) Economic and financial status and point III.1.3) Technical and professional capacityThe details and method of their demonstration are stated in Annex no. 5 SP. A specimen of the single European document is annexed no. 3 SP.

10. An offer security of EUR 10, 000.00 is required. The manner and conditions of the security shall be determined by law and detailed in point 11 of the CM. I will prove the deposit in the bids to the applicants. The security must be credited to the designated account of the person pursuant to - 8 of the Actor submitted in the form of a bank guarantee in the tenderer`s tender no later than the expiry of the deadline for the submission of tenders. A person pursuant to Section 8 of the Act will classify a failure to comply with this requirement as a failure to meet the conditions set by the person pursuant to Section 8 of the Act and exclude such an applicant. The account number and bank constitution for depositing the security are listed in the relevant part of the SP (point 11.5 (a)). In the case of composition secureWe recommend that bank transfer be accompanied by a document proving the fact as part of the offer. Where a security has been lodged in the form of a bank guarantee / guarantee guarantee, the document must be indicated by the tender as part of the tender and its validity can be demonstrated throughout the tendering period. The costs associated with the participation in this over-limit contract are borne by the applicants in full without any action at all.to the person under - 8 of the Act.

12. Justification for failure to divide this contract into lots: The contract is not divided into lots, given that the subject matter of the contract is homogeneous in terms of function, timing, locality, technical specifications and the type of contract concluded. Social aspects, green public procurement, procurement of innovations were not applied - d) none of the above.
