1. The subject of the order is the supply of lighting sets ZO-2 and ZO-8 in 2020, 2021 and 2022.

1. The subject of the order for task 1 is the delivery of ZO-2 lighting sets in 2020, 2021 and 2022.

As part of task No. 1, the Ordering Party requires the supply of ZO-2 lighting sets in the following quantities:

- in 2020 in the amount of 40 kpl,

- in 2021 in the amount of 40 sets,

- in 2022 in the amount of 40 sets

Ww. the amounts given are the quantities provided for underbasic ordering in individual years of the contract.

The Ordering Party informs that the delivery of the delivery which is the subject of the order in 2021 and 2022 will take place only if the suspending condition, which is the inclusion of the subject of the contract intended for delivery for this calendar year in the Technical Modernization Plan for the given year. Quantity of the subject of the orderplanned for delivery in 2021 and 2022 under the basic order, even if the suspending condition referred to above is fulfilled, may be reduced to the amount of financial resources allocated for this purpose in PMT for a given year.

Lighting sets ordered in scope of task No. 1 must be performed in accordance with the technical requirements (WT) stanobinding document of conduct.

The Employer does not set any conditions which the Contractor is obliged to demonstrate in a special way in terms of ability to conduct professional activity.

1. For the award of the contract mayContractors who are not subject to exclusion pursuant to art. 24 paragraph 1 and art. 24 paragraph 5 item 1 of the Public Procurement Law

2. Along with the offer, each Contractor is required to submit a JEDZ electronic statement with a qualified electronic signature:

- in the case of Contractors jointly applying for the order, JEDZ shall submit each of the Contractors jointly applying fororder,

- if the Contractor, in order to confirm compliance with the conditions for participation in the procedure, relies on the abilities of other entities for each of these entities submits a separate EAT form and a written commitment of these entities to put at his disposal the necessary resources for the needs of implementation orders.

3. Each Contractor within 3 days from the date of publicationon the information website referred to in art. 86 section 5 of the Public Procurement Law, the Employer shall provide a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act

4. In these proceedings, the Employer may apply the procedure provided for in Article 24 aa of the Public Procurement Law

5. The contractor whose offer will be the mosthigher rated for the given task, before awarding the contract, he will be invited to submit the following documents within a designated period, not shorter than 10 days:

A. confirming fulfillment of the conditions for participation in the procedure, listed in part III, point 1.1 of the notice,

B. confirming the lack of grounds for exclusion from the procedure, i.e.: information from the National Criminal Register in the scopee specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law;

2) an excerpt from the relevant register or central register and information on business activity, if separate provisions require an entry in the register or register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law;

3) the Contractor`s statement that he has not issued a final court judgment against himor a final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the event of such a judgment or documentary evidence confirming the payment of such receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of those receivables;

4 ) the Contractor`s statement on bratowards a decision against him as a precautionary measure forbidding public procurement.

4 the documents referred to in item 1 4 are submitted respectively:

- Contractor,

- in the case of Contractors jointly applying for the award of each Contractor jointly applying for the award of the contract,

- in the event that the Contractor, to confirm compliance with the condition of participation in the postProceedings will rely on the abilities of other entities indicated in point 1 5 documents, the Contractor will be required to submit for each of these entities.
