1. The subject of the order is:

Main Site: ul. Wieżowa 8, 02-147 Warszawa

For Part 1 - delivery, extension and installation of electrical distribution switchgears to the Ordering Party's premises (hereinafter "Appliances").-as part of the performance of the Subject of the Contract, the Contractor is obliged to: - Deliveries to the registered office of the Contracting Authority, ie the Polish Air Navigation Services Agency, ul. Wieżowa 8, 02-147 Warszawa;

Conversion of the power supply for precision air conditioning;

Installation and commissioning of the Equipment;

Provision of quality guarantee services and defenses for defects

I. The deadline for completing the order can not be shorter than 4-weeks and longer than 6 weeks for each Part of the order in accordance with Chapter IV of the ToR.

II. Contractors, who are not subject to exclusion from participation in the proceedings under Art. 24 sec. 1 and par. 5 point 1 of the Act

III. List of documents and statements confirming that the Contractor is not subject to exclusion from the proceedings based on art. 24 sec. 1 and5 of the Act. The contractor whose offer has been evaluated at the most, will be requested to submit, within the prescribed period, the following documents:

3.1. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act and issued not earlier than 6 months before the deadline for submitting offers or applications for admission to participate in the procedure;awca having its registered office in the territory of the Republic of Poland, with respect to a person residing outside the territory of the Republic of Poland, submits the document referred to in point 4 sub-point 4.1.1. To the extent specified in art. 24 sec. 1 points 14 and 21 of the Act. If the country in which the person has the place where the document was to reside is located, no such documents are issued, it replacesit is a document containing a statement of that person submitted to a notary or to a judicial, administrative or professional self-regulatory or economic authority competent for the place of residence of that person, issued not earlier than 6 months before the deadline for submission of tenders.

3.2. Write-off from the relevant register or from central records and information about the activityand economic, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act,

3.3. The Contractor's statements that he has not been the subject of a final court judgment or a final administrative decision on his inability to pay taxes, fees or social security or health insurance premiums orin the event of issuing such a verdict or decision documents confirming the payment of these receivables together with any interest or fines or entering into a binding agreement regarding the repayment of such receivables;

3.4. Contractor's statements about the lack of a ruling against him as a preventive measure of the prohibition against applying for public orders;

3.5. In addition, within 3 days of placingby the Ordering Party information on the opening of offers on the website on which the ToR is made available, the Contractors shall submit without a declaration a declaration of belonging or non-membership in the same capital group and, in the case of belonging to the same capital group, evidence that links with another The Contractor does not lead to distortion of competition in the proceedingsiu - prepared according to the print attached to the SIWZ enclosure 5 to SIWZ. - Documents submitted by the Contractor having its registered office or place of residence outside the territory of the Republic of Poland in accordance with the SIWZ Chapter VI point 4.

IV. By entering into this proceeding, each Contractor is obliged to pay a bid bond in the amount of: for lot No. 1 - PLN 16, 000; for part No. 2 - PLN 4, 000. Requiredand regarding the deposit in accordance with Chapter IX of the ToR.

V. The Contractor is bound by the offer for a period of 60 days, counting from the day on which the deadline for submitting bids expires. VI. In the proceedings, the declaration shall be made in writing, however, the EAT shall be sent in an electronic form bearing a qualified electronic signature.
