Providing services in the field of individual speech therapy for children under the comprehensive support program for families "Behind Life"

The subject of the contract is to provide services in the field of individual speech therapy for children as part of a comprehensive family support program "For lifem. "The estimated number of hours of the service in the period from the date of conclusion of the contract to 31/12/2020 was specified in the range of at least 3, 000 h - max. 3, 600 h.

Main Site: Area of the city of Katowice.

1. Priority of the II Comprehensive Support Program "For life" families - early support for the development of the child and his family. The task is financed from state budget funds. The subject of the order is the provision of services fromof individual speech therapy therapy for children as part of a comprehensive support program for families "For life". The estimated number of hours of service in the period from the conclusion of the contract to 31/12/2020 was specified in the range from a minimum of 3, 000 to a maximum of 3, 600 hours.

Therapy services speech therapy will be provided in the form of individual classes with children aged from birth until they take them onschool education. One hour of service is assumed to be a full clock hour (60 minutes). The Contractor`s remuneration shall be the product of the flat hourly gross remuneration rate for one hour of the service proposed in the offer and the number of hours of service actually carried out under the program.

The amount which the Employer intends to allocate to finance the contractenia, is 60 PLN gross for one hour of service provision. Total not more than PLN 216, 000.00 gross for 3, 600 hours.

2. The scope of the duties of a specialist conducting individual speech therapy for children includes:

- after appointing a specialist team for a specific child, determining the directions and schedule of actions to be taken in the field of early support and family supportines,

- establishing cooperation with the institution in which the child is covered by other therapeutic activities to ensure consistency of interactions,

- developing and implementing with the child and his family an individual early development support program in the part intended for a speech therapist,

- assessing progress and difficulties in the functioning of the child, including identifying and eliminating barriersand restrictions in the environment hindering his activity and participation in social life,

- analyzing the effectiveness of assistance provided on the basis of the program to the child and his family,

- introducing changes in the program according to the current needs of the child and his family,

- planning further activities in the field of early assistance.

Employment requirement under an employment contract. Employer requiresthat the Contractor or Subcontractor throughout the entire period of performance of the subject of the contract employs on the basis of an employment contract within the meaning of the Act of 26.6.1974 Labor Code (i.e. Journal of Laws 2019 item 1040, as amended) persons performing activities indicated in point 2. The time dimension must correspond to the time necessary for employees to perform all activities covered by the service of providing inindividual speech therapy therapy.

Criteria for the evaluation of the offers:

Detailed criteria for the evaluation of the offers are described in point15 Terms of Reference

Price (C) = weight 58%, service availability (D) = weight 42%

The score under the service availability criterion (D) will be awarded as follows:

Declaration of service in 3 selected locations from the list of the offer form ( minimum required by the Employer) = 0 points

For declaring the provision of services in 1 selected location above the required minimum = 3 points Maksymalfor the number of points to be obtained under this criterion is 42 points

Note: If the Contractor does not declare a minimum of 3 places of service from the list of the offer form, the offer will be rejected.

The total number of P points in the evaluation of each offer is the sum of calculated points in its assessment according to each criterion: P = CD

Place and date of submission and opening of bids:

1. The encrypted offer (together with the required attachments) should be submitted to the Employer`s electronic mailbox (Specialist Psychological and Pedagogical Counseling Center) by 3.12.2019 by 10: 00

2. Public opening of the offers will take place at the seat of the Employer`s Representative, Katowice, ul. Wita Stwosza 7, on 3/12/2019 at 11:00, in room 111.

3. Offers are opened byusing the application to encrypt offers available on the miniPortal and is made by decrypting and opening offers using a private key.

4. The opening of bids is public, Contractors may participate in the bid opening session.
