1. The object of the order is the supply of consumables and consumables to-sterilization. The subject of the order consists of the following 30 packages.

1. ABOUTthe contract may be awarded to Contractors who:

- are not subject to exclusion from the proceedings due to one of the circumstances indicated in art. 24 sec. 1 of the Public Procurement Law. The Contracting Authority does not provide for the exclusion of the Contractor based on art. 24 sec. 5 Public Procurement Law Acts,

- They meet the conditions for participation in the proceedings, referred to in point III.1) of this announcement - the purchaser did not specify any specific termsunkow.

2. Declarations or documents confirming compliance with the conditions for participation in the procedure, no grounds for exclusion and that the offered deliveries meet the requirements specified by the awarding entity:

2.1. EAT - a statement submitted electronically (using the online shopping platform).

2.2. Declaration on a capital group - submitted within 3 days from the date of posting to-the website of the Ordering Party, information referred to in art. 86 par. 5 Public Procurement Laws

2.3. Statements or documents confirming fulfillment of the conditions for participation in the proceedings and no grounds for exclusion to which the Ordering Party will call the Contractor whose offer was the highest rated within the prescribed period, not shorter than 10 days:

a) Information from the National Criminal Register inthe scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law issued not earlier than 6 months before the deadline for submission of bids,

b) Contractor's statements on the lack of issuing a final court decision or final administrative decision on default of taxes, fees or social security contributions or health or in the event of such a judgment-or decisions of documents confirming the payment of these receivables together with any interest or fines or conclusion of a binding agreement on repayment of these amounts, Annex No. 4 to the ToR,

c) Contractor's statements about the absence of a decision against him as a preventive measure for the prohibition of applying for public orders No. 5 to SIWZ,

d) Statements of the Contractor regarding-that the offered products meet the conditions for the introduction to medical trade and use specified in the Act of 20.5.2010 on medical devices (ie, Journal of Laws of 2017, item 211, as amended), attachment No. 7 to SIWZ,

e) In case of doubts whether the offered devices meet the requirements specified by the Ordering Party, the Contracting Authority may call the Contractor whose offer has been the highest rated, to the1) after 1 free sample of the offered tool - applies to the assortment offered in package No. 4. The samples should be delivered in a transparent, openable packaging, described with the full product name, catalog designation and item number and package, 2) Technical cards or catalog sheets.

f) Documents submitted by Foreign Contractors - as described in the ToR,

3. The condition-participation in the order award procedure is the bid bond in the correct amount for the given package for which the Contractor submits the offer.
