The subject of this part of the contract is the delivery of new unrefined radiological imagingused for diagnostics, namely delivery of 1 USG portable suitable for the needs of the OAIM department and Urgent Receive including 3 types of probes. The subject of this part of the contract is the delivery of the above mentioned equipment including the guarantee for 24 months, transport to the place of delivery, installation and installation at the place of delivery, execution of tests / trial operation and putting the equipment into operation, first time

Applicant must comply with the conditions of participation pursuant to - 32 of the Public Procurement Act (hereinafter referred to as ZVO) in full. This fact is manifested in the way stated in the GPO.

In the case of a group of suppliers, the procedure according to - 37 of the FTA is followed.

The contracting authority does not have access to the public administration information systems according to a special regulation (against the bureaucracy);according to - 32 par. 3 is not possible

Not required.

1. The price must include all costs associated with the realization of the object of the contract, including all related services and fees, including communications for communication in accordance with the subject matter specification. An interested person is obliged to take into account, before submitting his tender, all that is strictly necessary for the full and proper performance of the contract, whiletheir prices shall include all costs associated with the performance of the subject matter of the contract. In the event that the applicant applies a single European document (JED), he / she is required to complete it in accordance with the ban on public procurement. The contracting authority allows to fill in section 4 - global data. Documents and documents temporarily replaced by the JED must show the valid status of the tender at the date of submission of tenders. 5. The submission of the bid is only possible by an authenticated bidder. Authentication can be accomplished through the following methods:

- in the Josephine system by registering and logging in using an ID card with an electronic chip and a security personality code (eID). The system is authenticated by a company using eID registerstatute of the company. Authentication is carried out by the system provider Josephine on weekdays from 8 am to 4 pm, - by uploading a qualified electronic signature (for example, signing the eID) of the company statutory person to the user card after registration and logging into the Josephine system. 7. Bid security is only required for the following parts of the subject matter:
