The subject of the order is a comprehensive service of telephone registration and information of patients of the Oncology Center of the Institute, including, among others: The subject matter of the order is comprehensivenessIn the telephone service of registration and information of patients at the Institute's Oncology Center, including, among others:

1. Patient records for visits to the surgery offices of the Oncology Center at ul. Roentgen and Wawelska;

2. Coordinating cancellations of visits and / or changing appointments by patients;

3. Full telephone information in the field of health care services and organization of the clinic's operationoncology on ul. Roentgen and Wawelska;

4. Full telephone information regarding contact details, including phone numbers for all CO-I organizational units based on data provided by the Employer,

5. Reminding patients about the appointment (SMS, telephone, e-mail);

6. Support for telephone registration between 8:00 AM and 6:00 PM from Monday to FridayATKU.

1. In order to demonstrate the absence of grounds for exclusion from the proceedings under Art. 24 sec. 1 and par. 5 points 1 and 8 uPzp, the Ordering Party requires sending a Statement on the "EAT" form - in accordance with the rules described in Chapter IX - ordering the contract, the contractor will call the Contractor whose offer has been evaluated, to submit in the shortened period, not shorter than 10 days, aktuaas of the date of submitting the following documents confirming the lack of grounds for exclusion, if the Contractor did not indicate in the single document the place where the document was collected and used or the document submitted for another proceedings carried out by the Employer. a) Current excerpt from the relevant register or central records and information about the activity economic, -if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion based on Art. 24 sec. 5 points 1 uPzp;

b) Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13-14 and 21 uPzp, issued not earlier than 6 months before the deadline for submitting bids;

c) A statement confirming no grounds for exclusion fromContractor's proceedings (Annex No. 6 to the Terms of Reference), ie: the contractor / company has not been certified as a preventive measure to prohibit applying for a public order (Article 22 paragraph 1 point 22 of the Public Procurement Law) - that the above-mentioned the contractor / company within the meaning of the Act of 12.1.1991 on local taxes and fees is not in default of paying taxes and local fees (Act oflocal)

- that in view of the above the contractor / company has not been issued with a final court judgment or a final administrative decision on default of taxes, fees or social security or health insurance fees .... (Article 22 paragraph 1 point 15 of the RPA)

d) Certificates of the competent head of the Office Of the Tax Office confirming that the Contractor is not in arrears with paying taxes, issued not earlierthan 3 months before the deadline for submitting bids or applications for admission to participate in the proceedings, or other document confirming that the contractor has concluded an agreement with the competent authority regarding repayment of such receivables together with any interest or fines, in particular obtained the legally required exemption, postponing or spreading overdue payments in installments or wmaintaining in full the implementation of the decision of the competent authority; e) Certificates of the local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the expiry deadline for submitting bytt either requests to participate in the procedure, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of such receivables together with any interest or fines, in particular obtained legally permissible, postponement or installment payment of installments or suspension in full implementation of the decision of the competent authorityu;

2. Contractor, within 3 days from the date of posting information on the website of the Employer, referred to in art. 86 par. 5 uPzp, i.e. providing names of companies and addresses of Contractors who submitted bids on time, is obliged to provide the Ordering Party with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 uPzp (attachik No. 7 to the ToR). Along with the submission of the statement, the Contractor may provide evidence that links with another Contractor who submitted an offer in this proceeding do not lead to distortion of competition in the procurement procedure. In order to demonstrate the absence of grounds for exclusion from the proceedings, the Ordering Party must submit a Statement on the form. EAT "

I. The contractor should also submit in wraz with the offer ::

a) Authorization to sign the offer and tender documents, as well as to submit possible explanations, if the signer is not a person authorized on the basis of extract from the national court register or certificate from the business register (authorization is required in the original or a copy certified by a notary public),

b) confirmation is lodgeda bid bond,

c) completed price form (attachment No. 1 to SIWZ),

d) completed offer form (Annex 2 to the Terms of Reference)

II. If the Contractor has a registered office or place of residence outside the territory of the Republic of Poland, he files documents in accordance with the Regulation of the Minister of Development of 26.7.2016 regarding documents that the Employer may request from the Contractor, their validity period and forms, in-which these documents can be submitted.
