IAAS and PAAS service in the Dedicated Private Cloud model

Reference Number: 108 / EZP / 18. The subject of the order is a service consisting in the provision and implementation of SCCS for an optimal and safe work environment for hospital operations in real time in IT, along with the resources needed for this purpose to process and collect dataand providing the necessary network infrastructure - Providing and implementing SCCS for an optimal and safe work environment for real-time hospital operations in the IT area, including resources for processing and data collection needed for this purpose, and necessary network infrastructure - The contractor calls the contractor, which offer was rated the highest, to submit indesignated for a period of not less than 10 days as of the date of submitting statements or documents:

1) A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion based on Article. 24 sec. 5 point 1 of the Act;

2) Contractor's statements about the lack of itego of final judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of issuing such a judgment or decision - documents confirming the payment of these receivables of these receivables or conclusion of a binding agreement on repayment of these debts. 3) Current National Informationego Register of Criminal Records in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law - issued not earlier than 6 months before the offer expires.

4) Contractor's statements about the lack of a ruling against him as a precautionary measure prohibiting to apply for a public order.

If the contractor is established or resides outside territory of the Republic of Poland, instead ofokumentow, referred to in Chapter VI, paragraph 1 point 1.1), 1.3) SIWZ shall consist of: document or documents issued in the country in which he has his registered office or place of residence, confirming accordingly that: there are no exclusion circumstances referred to in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law - submit an extract from another relevant register or in the absence of such a register in the country in whichthe contractor has a registered office or place of residence, another equivalent document issued by the competent judicial or administrative body of the state in which the contractor has its registered office or place of residence, its liquidation has not been opened or its bankruptcy declared. The documents referred to should be issued not earlier than 6 months before the offer submission has expired. - Document confirming that Wthe contractor is insured against civil liability in the scope of conducted activity related to the subject of the order - the Contractor is obliged to pay a bid bond in the amount of PLN 20, 000. To confirm the fulfillment of conditions for contractors in the scope of the service the Contracting Authority calls the contractor whose offer has been rated the most, to submit designated, not shorter than 10 days, date valid as at the day of submitting statements or documents:

1. Certificate confirming the Contractor's application of a quality management system compliant with the requirements of the PN-EN IS0-9001,

2 standard. Certificate (certificate) stating that the Contractor meets the requirements of the TIA 942 standard at the level of at least Tier 3 / Rated 3,

3. Certificate confirming the Applicant's use of PN-EN ISO 27001 in the field of data processing security or another certificate (certificate) confirming the Contractor's ability to process and protect data in Data Center facilities where the Employer's data will be processed.

4. Declaration of employment for the service of min. 70% persons employed under a contract of employment (full-time),

5. The orderer reserves the righta local vision of the Contractor whose offer will be selected as the most advantageous in order to check whether the Contractor meets the requirements of TIA 942 standard at the level of at least Tier 3 / Rated 3, the Contractor will be notified of such necessity and will set the date of the visit not later than 10 days after receiving the notification. Failure to meet the requirements of the standard will result in the offer being rejected. The offer must include the followingsubmitting documents and materials:

1) Completed and signed by the person authorized to represent the contractor. Form of the offer constituting Annex No. 1,

2) Filled and signed Annex No. 2 (Package).

3) Filled and signed Table No. 1 (Annex No. 5)

4) Completed and signed Table 2 (Annex No. 5)

5) Completed and signed Table 3 (Annex No. 5)

6) Completed and signed Tabela No. 4 (Annex No. 5)

7) Completed and signed Table No. 5 (Appendix No. 5) - The contracting authority may, in proceedings conducted under an open tender, first evaluate the offers and then examine whether the contractor whose offer was evaluated as the most advantageous, it is not subject to exclusion and meets the conditions for participation in the proceedings of Art. 24aa.
