Implementation of the system for monitoring and managing the vehicle fleet using GPS technology

1. The subject of the Order is to carry out the work of providing and starting at the Employer`s system for monitoring and managing a fleet of 2, 000 vehicles with the use ofthe existence of GPS technology, hereinafter referred to as GPS system.

1. The subject of the Order is to carry out the work of providing and starting at the Employer`s system for monitoring and managing a fleet of 2, 000 vehicles using GPS technology, hereinafter referred to as GPS System.

2. The Contractor shall perform the Contract in the following scope:

a) will deliver, install and launchon the Employer`s official vehicles:

and. GPS devices 2, 000,

ii. additional device sensors 500 pcs,

iii. fuel probes 200 pcs,

iv. two-state switches (private / business travel) 80 pcs;

b) provide and implement GPS System software on the Ordering Party`s IT infrastructure and provide the required licenses for GPS System software;

c) provide, along with the required licensesme, a mobile application for business smartphones of the Employer`s employees, enabling communication with the GPS System;

d) ensures integration of the GPS System with the existing systems of the Employer:

and. PasCom FLOTA II fleet management system,

ii. Central Map Server,

iii. Central Access Control System,

iv. The SIEM (Security Information and Event Management) system;

e) will be carried out by the Employerfor training for 10 (ten) administrators and 100 (one hundred) end users of the GPS System;

f) will prepare and provide the Employer with project documentation and as-built documentation;

g) provide the Employer with warranty service within 24 months from the date of receipt of the final implementation of the GPS System;

h) provide the Employer with additional support services within 24 months from the date of collection horseimplementation of the GPS System in the amount of 400 man-hours.

3. A detailed description of the subject of the Order is included in Annex 1 to the ToR. 24 paragraph 1 point 13) point 23) of the Public Procurement Law Act (referred to as the Act announcement), with the proviso that the Contractor is not subject to exclusion in the case referred to in art. 24 paragraph1 point 13 lit. d) the Act, and in the case referred to in art. 24 paragraph 1 item 14 of the Act, if the person referred to in this provision has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. d of the Act. The contracting authority will exclude the contractor also in the cases referred to in art. 24 paragraph 5 points 1), 2), 4) and 8) of the Act.

In the case of Contractors jointly applying for the award of the Contractand the conditions described above must be met by each of the Contractors separately.

1. Contractors who have creditworthiness or financial resources on a bank account for an amount of not less than PLN 3, 000, 000 (say: three million zlotys 00/100) may apply for the Contract.

2. In the case of Contractors jointly applying for the Contract, the conditions described in point III.1.2)1., III.1.3) 1 and III.1.3) 2 contract notices may be met by at least one of the Contractors individually or jointly a group of Contractors jointly applying for the award of the Contract.

3. The Contractor demonstrating compliance with the conditions referred to in points III.1.2) and III.1.3) of the contract notice, may rely on the resources of other entities on the principles set out in Art. 22a paragraph 1 of the Act.

4. Assessment of compliance with the conditions required of Contractors referred to in points III.1.1), III.1.2) and III.1.3) of the contract notice will be made according to the formula "0 1", where the value 0 means "does not meet" and the value 1 "meets", on the basis of statements contained in the Single European Order Document JEDZ (Annex 2 to the ToR) attached to the offer submitted in the said progresscrossing.

1. The offer must be accompanied by a preliminary statement in the form of JEDZ (Annex 2 to the ToR), valid as at the date of submitting the offer, confirming that the Contractor is not subject to exclusion and meets the conditions for participation in the Procedure.

2. To confirm compliance with the conditions for participation and no grounds for exclusion. Zam. I will call the Contractor whose highest bid is to be submitted on timeie, not less than 10 days, valid as at the date of deposit:

1) Info. from the National Reg. Criminal within the scope specified in art. 24 paragraph 1 item 13), item 14) and 21) of the Act issued not earlier than 6 months before the submission deadline.

2) the competent head of the tax office confirming that the copy is not in arrears with taxes, issued not earlier than 3 months beforem the deadline for submitting bids, or other document confirming that the Wyk has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full decision of the competent authority.

3) proper field jorganizational unit of ZUS or KRUS or other document confirming that the Wyk is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the Wyk has entered into an agreement with the competent authority on the repayment of these claims together with any interest or fines, in particularhe obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the execution of the decision of the competent authority.

4) Copy from the relevant register or from the central register and information on business activity, if separate provisions require an entry in the register or register (art. 24 item 5 point 1) of the Act).

5) Statement Execution of non-issue to nego of a final court judgment or a final administrative decision on arrears in payment of taxes, fees or social security or health insurance contributions, or in the event of such judgment or decision of documents confirming payment of these claims together with any interest or fines, or a binding agreement on the repayment of these receivables (art. 24 paragraph 1 point 15 of the Act) - Annex No. 3 to the ToR.

6) Statement Execution of the lack of a judgment against him as a preventive measure prohibiting him from applying for a contract (Article 24 paragraph 1 item 22 of the Act) - Annex 4 to the SETC.

7) Statement Execution of non-payment of taxes and local fees, referred to in the Act of 12.1.1991, on taxes and local fees - Annex 5 to SIWZ.

8) Educationd. Execution of belonging or not belonging to the same capital group as referred to in Article 24 para. 1 point 23) of the Act - App. 7 to SIWZ.

9) Information from the bank or cooperative savings and credit union confirming the amount of financial resources or creditworthiness issued for an amount not less than PLN 3, 000, 000.00 issued not earlier than 1 month beforethe date of submitting offers in the Procedure,

10) List of services rendered, and in the case of periodic or continuous services also performed, in the last three years before the deadline for submitting offers, and if the period of conducting activity is shorter in this period, together with their value, subject, dates of performance and entities for whom the services were performed, and attachmentsm of evidence specifying whether these services have been performed or are being performed properly (Annex 8 to the ToR),

11) List of persons referred by the Execution for the implementation of the Order. in particular those responsible for providing services (Annex 6 to the ToR).
