The contracting authority imposes the transmission of the application in the form of forms DC1 and DC2 but accepts that the candidate submits his application in the form of a Single European Market Document (DUME). Presentation in the form of DC1 andDC2: candidates must use forms DC1 (letter of application) and DC2 (declaration of the candidate) to submit their application. These documents are available free of charge on the website www.economie.gouv.fr - they will contain the elements indicated below: for companies, mutuals and insurance organizations: information concerning the legal situation of the companyise:

- the letter of application (DC1),

- declaration on honor to justify that it does not enter in any of the cases mentioned in articles L. 2141-1 to L. 2141-5 and L. 2141- 7 to L. 2141-10 of the public procurement code and that it is in good standing with regard to articles L. 5212-1 to L. 5212-11 of the labor code concerning the employment of disabled workers, in accordance with article R. 2143-3 of the code ofpublic procurement,

- certificate of approval to cover this risk issued by the ACP or copy of the order published in the Official Journal authorizing the coverage of said risk by the company / mutual.

Declaration concerning the overall turnover and the turnover relating to the services covered by the contract, relating to a maximum of the last 3 financial years available depending on the availabilityat the creation of the business or the start of activity of the economic operator, insofar as the information on these turnover is available.

It is reminded that an insurer (company or mutual) cannot present several offers through multiple intermediaries a general agent or broker may not be the holder of the contract. This open invitation to tender issubject to the provisions of articles L. 2124-2, R. 2124-2, R. 2161-2 to R. 2161-5 of the public procurement code. For the intermediary, in addition to documents sent by companies, mutuals and insurance organizations: information concerning the legal situation of the company:

- the letter of application (DC1),

- declaration on honor for justify that it does not fit into any of the cas mentioned in articles L. 2141-1 to L. 2141-5 and L. 2141-7 to L. 2141-10 of the public procurement code and that it is in good standing with regard to articles L. 5212-1 a L. 5212-11 of the labor code concerning the employment of disabled workers, in accordance with article R. 2143-3 of the public procurement code,

- certificate of registration with ORIAS - art. L. 512-1 of the insurance code,

If the offer is proposed by an insurance company and an intermediary, it must imperatively be signed by the company. If the offer is only signed by the intermediary, the latter must have submitted in the documents concerning the application, a certificate from the company mandating him expressly to reply on his behalf and sign the documents.of the offer. Information concerning the economic and financial capacity of the company (article R. 2142-6 of the public procurement code):

- declaration concerning the overall turnover and the turnover concerning the services subject to the contract, relating to a maximum of the last 3 financial years available depending on the date of creation of the company or the start of activityfrom the economic operator, to the extent that information on these turnover figures is available,

- certificates of financial guarantees and professional civil liability insurance.

Information concerning professional references and the technical capacity of the `company as provided for in articles R. 2142-13 and R. 2142-14 of the public procurement code:

-presentation of a list of the main controllable references for the insurance of works equivalent to those covered by this contract during the past 3 years, indicating in particular the amount, the date and the public or private recipient.

The services are proven by certificates from the addressee or, failing this, by a declaration by the economic operator. In accordance witharticle R. 2143-14 of the public procurement code, candidates are not required to provide the documents and information mentioned above, already sent to the city of Antibes as part of a previous consultation and which remain valid . No transmission by paper or electronic physical medium (Cd-Rom / Dvd-Rom / Usb key) is authorized for this consultation except for:

-any backup copy; the contracting authority imposes the transmission of documents electronically on the buyer profile of the contracting authority at the following address: https://e-marches06.fr - no other mode of transmission is authorized except for the exceptions mentioned above above.
