"Construction of sewerage and water supply systems in Słupno and Sierak-w Żołta FIDIC"

The subject of the orders includes development of project documentation, obtaining relevant decisions and execution of construction works falling within the scope of Contract No. 6, part 2: "Construction of sewage networkThe project is part of the key project "Construction of sanitary sewage system and water supply network in the Radzymin commune Stage II" and is one of the parts of the order for the design and construction of sewage and water supply networks. : Town of Słupno and Sierakow in the Radzymin commune

The subject of the order is elaboratednot the design documentation, obtaining relevant decisions (including decisions on building permits / applications for construction works), construction together with submitting a notification of completion of construction works or application for a decision on use to the appropriate building supervision authority and obtaining no objections to use or legally valid decision on the use of the constructed infrastructurein Słupno and Sierak-w, including:

• construction of gravity channel DN 315 PVC with a total length of approx. L = 2.44 km

• construction of a gravity sanitary duct DN 200 PVC with a total length of approx. L = 9.49 km

• construction of 748 sections of DN160-DN300 sewerage network (hereinafter referred to as sewage branch) in street road lanes, including: DN300 1 piece (from the main channel to ul.Neighborhood); DN200 45 items (from the main channel to side roads, plots for roads and premises); DN160 702 pieces (from the main canal to the property boundary). Sewerage branches are planned for all properties with built-in ducts. In the case of built-up plots, the scope of design includes a section of the sewerage pipe from the sanitary canal in the street to the cityca inclusion of sewage from buildings located on private premises. In the case of undeveloped plots, the design includes a sewerage conduit terminated with a sewage well on the plot. The construction of designed sewage branches covers the section from the sanitary canal in the street to the boundaries of private premises.

The final number of sewage offshoots will result from the developmentand design guidelines adopted based on current geodetic divisions of real estate preceded by a field vision. In justified cases (eg a large amount of sewage generated within the property, for which the DN160 diameter connection will be insufficient) The Employer allows increasing the branching diameter DN160 to DN200.

Construction of 8 sewage pumping stations;u discharge DN90-160 mm PEHD with a total length of approx. L = 1.03 km;

• construction of a DN105 PEHD water pipeline with a total length of approx. L = 0.8km, including switching to a built-up network of two existing water supply connections via a welded post tractor elektrooporowo;

construction of 138 sections of the DN40-DN160 water supply network (hereinafter referred to as water supply branches) in the road lane of Żeromskiego w Słupnie and Sienkiewicza in Sierak-w from the main network, including: DN160 1 pcs and DN110 21 items (from the main water supply to side roads and plots designated for roads); DN40-50 116 pieces (from the main water supply system to the property boundary, choice of diameter depending on the water demand, the destination of the plot) If the road is separated on the plot during the design works or planoThe branch is to be provided for all undeveloped and developed properties without a water supply connection, as well as side roads and plots intended for roads in ul. H. Sienkiewicz and S. Żeromski.

Scope of the projectbuilding and construction of water supply branches in the case of built-up and undeveloped plots includes a section of the water main from the aqueduct in the street to the boundaries of private premises. In the case of side roads / plots intended for roads, water supply branches should be made to the border of the road lane ul. H. Sienkiewicz in Sierak-w and S. Żeromski in Słupno.

The final number of water branchesdraft and assumed diameters will result from design solutions adopted on the basis of current geodetic divisions of real estate preceded by field vision.

The ordering party leavesfrom the determination of this condition. Contractors must meet the conditions for participation in the proceedings regarding the economic or financial situation. The contractor shall attach to the offer a statement in the form of a single document (hereinafter referred to as "EAT"), valid as per the day of submitting bids, prepared in accordance with the standard form provided for in the Regulation of the European Commission issued on the basis of Article 59paragraph. 2 of Directive 2014/24 / EU. The information contained in the statement will constitute preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure.

2. In order to confirm the fulfillment of the condition regarding the economic or financial situation, the contractor requests from the contractor:

a) declaration by the contractor of the contractor's annual turnover in the area covered by the order, for the lastnot 3 years, and if the period of business is shorter - for this period;

b) documents confirming that the contractor is insured against civil liability in the scope of conducted activity related to the subject of the order for a guarantee sum determined by the Employer.

3. In the case of joint application for contractors, EAT submits each of the contractsjointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion. Contractor who refers to the resources of other entities in order to demonstrate the lack of grounds for exclusion against them and spto the extent to which they refer to their resources, the conditions for participation in the proceedings are also submitted by EEDs regarding these entities.

5. The Contractor, which relies on the financial or economic situation of other entities, regardless of the legal nature of the legal relationship that binds him with it, submits a written commitment of the entity to put the Contractor at the disposal of the necessary resources for the needs ofof ordering.

6. Contractors jointly applying for the award of the contract shall submit the documents indicated in point 2 in such a way as to show that the conditions for participation in the Contractor's proceedings meet jointly (ie they submit at least one of these contractors or all of those contractors in common).

7. If for justified reasons, the contractor can not submit documents regarding the financial situation orb) it can submit another document, which sufficiently confirms fulfillment of the condition of participation in the procedure described by the Employer.

1.In order to confirm the absence of grounds for exclusion of the Contractor, the ordering party requests: 1) information from the National Criminal Register in the scope specified in art 24 sec. 1 point 13 lit. a-c, 14 and 21 Pzp issued toe) earlier than 6 months before the deadline for submitting offers;

2) excerpt from the relevant register or from the Central Register and Information on Economic Activities, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 Pzp;

3) the contractor's statement that he has not been the subject of a final court judgment against him orb the final administrative decision on default of taxes, fees or social security or health insurance premiums or, in the event of such a judgment or decision being issued, documents confirming payment of such claims together with any interest or fines or a binding agreement on repayment of such debts;

4) the contractor's statement about the lackruling against him as a precautionary measure prohibiting to apply for public orders; - 5) the contractor's statement on whether or not to belong to the same capital group as referred to in art. 24 sec. 1 point 23 Pzp. Along with the submission of a statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the proceedingssplitting orders. The contractor submits this declaration to the awarding entity within 3 days of posting on the website information on the amount he intends to use to finance the order, companies and addresses of contractors who submitted offers within the time, price, time of order execution, warranty period and payment terms included in the offers .

2. The ordering party demands from youa con-player who relies on the abilities or situation of other entities on the principles set out in art. 22a Pzp, presentations in respect of these entities of documents mentioned in point 1 subpoints 1-4.

3.If Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point 1:

1) subpoint 1 - submit information from the relevant register orin the absence of such a register, another equivalent document issued by the competent judicial or administrative body of the country in which the contractor has its registered office or place of residence is the person concerned by the information or document, to the extent specified in art. 24 sec. 1 item 13 letter a-c, 14 and 21;

2) item 2 - submits a document or documents issued in the country in which the contractorhas its registered office or place of residence, confirming respectively that its liquidation has not been opened or bankruptcy has been announced. 4.If in the country in which the Contractor has its registered office or place of residence has the person concerned, documents are not issued, referred to in point 3, shall be replaced by a document containing the contractor's statement, as appropriate, to indicatem persons or persons authorized to represent it, or a statement of the person to whom the document was to be applied, submitted to a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the Contractor's seat or place of residence or place of residence. .The contractor having its registered office in the territory of the Republic of Poland in relation to themay reside outside the territory of the Republic of Poland, to which the document referred to in point 1.1 applies, submit the document referred to in point 3.1) to the extent specified in art. 24 sec. 1 point 14 and 21 of the Public Procurement Law.
