The subject of the task is the construction of bicycle paths and pedestrian-bicycle paths in the scope of:
Main Site: City of Opole
The subject of the contract is the execution of construction and assembly works related to the construction of the pedestrian - bicycle route along ul. Father Edward Frankiewicz on the episode from ul. Skip to street Credit along withconstruction of conventions, reconstruction of technical infrastructure and construction of storm water drainage and street lighting.
Location of construction: The investment will be implemented in the Opolskie Voivodeship in the city of Opole, on ul. Frankiewicza in Opole on the section from ul. Jumping to railway crossing.
Length of planned pedestrian-bicycle route: approx. 275 linear meters.
Road works:
Earthworkswill be on the construction of the trough and laying a new structure of the pedestrian cycle route, descents and sewage. The pedestrian cycle route will be made of 3.0 m wide AC8S asphalt concrete surface (with local narrowing to 2.6 m). The pedestrian cycle route will be limited from the road by a concrete curb 15x30x100 raised 6 cm above the road surface along with the sewage made of concrete cubes. From theserhenium greenery and on the border of the plot, the cycling route will be limited by a concrete rim 8x30x100 on a concrete bench. Conventions along the width of the pedestrian - bicycle route are designed with asphalt concrete surface AC8S. In addition to the walking and cycling route, descents are planned with a concrete block surface 8 cm in graphite. At the entry and exit of the cycling route on foota concrete inrun curb is constructed on a flange with a road surface. Conventions to arable land are designed with a surface from a mixture of aggregate. The green area should be humbled and sown with grass. Introduction of target traffic organization.
Sanitary industry works (storm water drainage / road drainage):
New street inlets and drainage are designed for storm water drainagewater that drains rainwater from drains to rainwater drains. The planned storm water drainage system, apart from the drainage water, will also take over water from drainage of pavements and exits. There is an active rainwater sewage network in the street, which is used to drain the street by building new rain drains, drains and inspection chambers for their inclusion. Przykanalirain pipes made of PVC SN8 pipes with a diameter of 160 mm will be included in the gravitational sewage system.
Electricity robot: Reconstruction of low voltage power line pole stands colliding with the construction of a bicycle pedestrian route. Construction of street lighting in the number of 25 stands (luminaires and columns) LED light with the necessary electrical network.
Material scope, among others:
b) Implementation of descents for cycling routes.
c) Cutting of colliding greenery.
d) Construction of storm water drainage.
e) Removal of collision with the low-voltage power line.
f) Construction of street lighting.
g) Introduction of the target organization The detailed description of the subject of the contract is specified in the design documentation, technical specifications of the execution and acceptance of the construction workch, bill of quantities developed by Pracownia Projektowa SEWI Sebastian Raudzis Sebastian Wilisowski s.j. ul. Oleska 117, 45-231 Opole: Construction of a pedestrian and bicycle route along ul. Ojca Edward Frankiewicza STAGE II, and: Project for the reconstruction of two posts of low voltage power lines at ul. Edward Frankiewicz developed by Teodor Mateja, as well as: Documentation of the Futurea Project for the construction of street lighting on Ojca Edwarda Frankiewicza Street in Opole, including pedestrian and bicycle routes, developed by Teodor Mateja. exclusion from the procurement procedure in the circumstances referred toreferred to in art. 24 paragraph 1 point 12-23 of the PPA and art. 24 paragraph 5 item 1 8 of the PPA, the Employer requests the submission of the following documents:
1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act on Public Proceedings and, regarding conviction for an offense to imprisonment, to the extent specified by the contracting authority pursuant to art. 24 paragraph 5 points 5 and 6 of the Act on Public Health, issuedj not earlier than 6 months before the deadline for the submission of tenders or requests to participate in the proceedings of the Employer in accordance with the provisions of Article. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document within a specified time limit not shorter than 10 days;
2) a certificate from the competent head of the tax office confirming thate the contractor is not in arrears with payment of taxes issued no earlier than 3 months before the deadline for submitting bids or requests to participate, or another document confirming that the contractor has entered into an agreement with the relevant tax authority regarding the repayment of these debts together with possible interest or fines, in particular he obtainedreleasing, postponing or dividing in installments of overdue payments or withholding in full the decision of the competent authority of the Employer in accordance with the provisions of art. 26 section 1 tax law will call the Contractor whose tender has been highest rated to submit this document within a specified time limit not shorter than 10 days;
3) a certificate from the relevant local organizational unit of ZaklSocial Insurance or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders or requests to participate in the procedure, or another confirmation document that the contractor has entered into poroagreement with the competent authority on the repayment of these receivables, together with any interest or fines, in particular, obtained by law exemption, deferment or distribution in installments of overdue payments or withholding in full the decision of the competent authority of the Employer in accordance with the provisions of art. 26 section 1 tax law will call the Contractor whose bid was highest rated tosubmit this document within a specified time limit not shorter than 10 days;
4) In order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law, the contracting authority requests a copy of an appropriate register or from the central register and information on business activity, if separate provisions require an entry in the register or register - the Awarding Entity in accordance with the provisions ofrt. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document within a specified time limit of not less than 10 days
5) the contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding late payment of taxes, fees or insurance premiums social or health or - in the case ofissuing such a judgment or decision - documents confirming the payment of these receivables together with any interest or fines or concluding a binding agreement on the repayment of these receivables in accordance with the provisions of art. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document within a designated period of not less than 10 daysand date;

The Awarding Entity refrains from specifying this condition of participation in the said procedure. Completion of point III.1.1) 6) declaration of the contractor about the lack of a ruling against him as a preventive measure forbidding public procurement The contracting authority in accordance with the provisions of art. 26 section 1 tax law will call the Contractor whose bid was highest rated tosubmitting this document within a specified time limit, not shorter than 10 days;
7) the contractor`s statement on belonging or not belonging to the same capital group; in the case of belonging to the same capital group, the contractor may submit, together with a statement, documents or information confirming that links with another contractor do not lead to distortion of competition in postsubmitted in accordance with the provisions of art. 24 paragraph 11 of the PPA;
8) In order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 5 and 6 of the Public Procurement Law, the contracting authority requires a submission of a contractor`s statement that no final judgment of the convicting court has been issued for the offense of restriction of liberty or a fine to the extent specified by the contracting authority pursuant to art. 24 paragraph 5points 5 and 6 of the PPA - Employer in accordance with the provisions of Art. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document within a specified time limit not shorter than 10 days,
9) In order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 7 of the Act on Public Procurement, the contracting authority requires the Contractor to submit a declaration that he has not issued an ostate to himof an administrative decision on breach of the obligations arising from the provisions of labor law, environmental protection law or social security provisions to the extent specified by the contracting authority pursuant to art. 24 paragraph 5 point 7 of the PPA - Employer in accordance with the provisions of Art. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document inmarked period, not shorter than 10 days,
10) In order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 8 of the Public Procurement Law, the Awarding Entity requests the Contractor`s statement on the Contractor`s statement on non-payment of local taxes and fees, referred to in the Act of 12 January 1991 on local taxes and fees (Journal of Laws of 2016, item 716 ) - Purchaserin accordance with the provisions of art. 26 section 1 tax law will call the Contractor whose bid was highest rated to submit this document within a time limit of not less than 10 days.
3. If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in Chapter 10, item 2 above:
a) item 1) - he shall submit information from the relevant regester or, in the absence of such a register, another equivalent document issued by the competent judicial or administrative authority of the country in which the contractor has its registered office or place of residence or place of residence, has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 and par. 5 points 5 and 6 of the PPA;
b) items 2, 3, 4) - submits a document or documentyou issued in the country in which the contractor has its registered office or place of residence, confirming accordingly that:
- he is not in arrears with the payment of taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding the repayment of these debts together with with any interest or fines, in particular, he has been granted an exemption, deferral or legal stipulationspreading overdue payments into installments or suspending in full the execution of the decision of the competent authority,
- its liquidation has not been opened and no bankruptcy has been announced.
These proceedings are conducted in the form of an open tender.
