The subject of the investment is the reconstruction of the surface structure of the voivodship road No. 156 on sections in km 48 220 50 000. The length of the voivodship road covered by the reconstruction: 1 780 m.

The subject of the investment is the reconstruction of the voivodeship road construction No. 156 on sections in km 48 220 50 000. Length of the voivodship road covered by reconstruction: 1 780 m.

Scope of investment includes: - reconstruction of the existing roadway and regulation of its width up to 6.0 (lanes 3.0 m)

- passagesthighs of side road intakes

- extension and renovation of existing constructions

- construction of new bus platforms

- rebuilding of roadsides

- improvement of marking elements,

- cleaning and profiling of existing road trenches

Detailed description of the order and technical requirements are set out in Chapter III of the Terms of Reference.

Statements and documents to be provided to the contractor for the purpose-demonstrating the lack of grounds for exclusion and confirming compliance with the conditions of participation in the proceedings - 1. The Contractor is obliged to attach to the offer a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor:

a) is not subject to exclusion;

b) meets the conditions for participation in the procedure.

2. The declaration referred to in point 1. The Contractor is obliged to submit in the form ofa single European order document (EED) prepared in accordance with the standard form provided for in the European Commission Implementing Regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as the "single document" (attachment No. 1 to the offer form) under pain of nullity, in electronic form, bearing a qualified electronic signaturem, in accordance with the principles of point 8.2 of the SIWZ.

3. The Employer requires that the Contractor, in the scope of the qualification criteria described in Part IV, EED only fill in the ALFA section in Part IV and not fill in any of the other sections in Part IV.

4. Contractor, within 3 days from the date of posting on the website information referred to in art. 86 par. 5 of the Public Procurement Law, will provide the orderer with a statementabout belonging or not belonging to the same capital group, referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. With the submission of a statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure. At the request of the contracting party, the contractor is obliged to submit the following statements or a documentyou:

5.1 To confirm that the contractor meets the conditions of participation in the proceedings: 1) documents confirming that the contractor is insured against civil liability in the scope of activities related to the subject of the order for a guarantee sum not less than PLN 2, 000, 000.00. ) list of construction works carried out not earlier than in the last 8 years before the end of the summerthe time limit for the submission of offers, and if the period of business is shorter - during this period, along with the type, value, date, place of performance and entities for which these works were carried out,

With the inclusion of evidence determining whether these works were duly completed, in particular information about whether the works were carried out in accordance with the provisions of the Budo lawwell-founded and properly completed, whereby the evidence referred to are references or other documents issued by the entity for which the construction works were carried out, and if for a justified reason of an objective nature the contractor is unable to obtain these documents - other documents,

3) list of persons referred by the contractor to perform the public order, in particularresponsible for managing construction works, along with information on their professional qualifications, entitlements, experience and education necessary to perform a public order, as well as the scope of their activities and information on the basis of to dispose of these persons.

If for a justified reason the contractor can not submit required by the ordering partyego documents referred to in point 5.1.1), the contracting authority allows the contractor to submit other documents, okay, mentioned in art. 26 par. 2c of the Public Procurement Law - the Contractor will fulfill the condition if he shows the minimum level of ability, i.e.: 1) he is insured against civil liability in the scope of conducted activity related to the subject of the order for a guarantee sum not less than 2, 000, 000.00 PLN.

In the case of submitting an offer by entities acting in co-operation, the condition can be met in total.

I. The Contracting Authority provides for the application of the procedure referred to in art. 24 aa section 1 of the Public Procurement Law

II. Premises for exclusion of contractors: 1. The contractor is excluded from the contract award procedure in respect of any of the circumstances referred to in Article 24 paragraph 1 point 12-23 Public Procurement Laws. 2. In addition, the orderer will exclude the contractor based on Art. 24 sec. 5 points 1, 5, 6, 7, 8 of the Public Procurement Law - III. At the request of the ordering party, the contractor is obliged to submit the following declarations or documents: In order to confirm the absence of grounds for exclusion of the contractor from participation in the proceedings, the contractor is obliged to submit documents in accordance with the Regulation of the Minister of Development from 26.07.2016 in-on the types of documents that the contracting authority may request from the Contractor in the proceedings for the award of the contract (Journal of Laws of 2016, item 126), ie pairs 5. point: 1), 2), 3), 4) 5), 6), 7), 8), 9).

If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, and also if the person to whom the given document relates is residing outside the Territory of the Republic of Poland.paragraph 7 of the abovementioned Regulation on the types of documents - Contractor established in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which document refers to - applies paragraph 8 of the abovementioned Regulation on types of documents - Contractor established in the territory of the Republic of Polandin respect of a person residing outside the territory of the Republic of Poland, the document of which applies - paragraph 8 of the abovementioned Regulation on types of documents

IV. Deadline for ordering: Required deadline for processing the order by 29.7.2019.

V. The Contractor is bound by the offer 60 days from the deadline for submission of offers. I start the run of the offer binding period-along with the deadline for submission of offers. (Article 85 (5) of the Public Procurement Law).

VI. Requirements for the bid bond:

1. The Contractor is obliged to submit a bid bond in the amount of PLN 70, 000.00 (in words: seventy thousand PLN 00/100) before the deadline for submission of tenders. The bid bond can be contributed in:

1) money,

2) bank guarantees, or social and savings bank sureties, except thate surety is always a cash guarantee;

3) bank guarantees;

4) insurance guarantees;

5) sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2016, item 359).

3. The bid bond in the form of money should be paid by bank transfer to the bank account of Getin Noble Bank SA-O / Zielona Gora account number 38 1560 0013 2263 6869 1000 0011, with a note on the transfer: "Wadium in proceedings ZDW-ZG-WZA-3310-71 / 2018 on" Reconstruction of the road province. VIII. The offer should include:

1) completed offer form,

2) commitment to provide resources - Annex No. 2 to the offer form (if the contractor relies on the resources of another entity)

3) proof of payment of the bid bond,

4) completed price form. 5) power of attorney to sign the offer in the original or a copy certified by a notary public - in the case of a signaturean offer and authentication of copies of documents by a person not mentioned in an excerpt from the relevant register or central register and information about the economic activity of the contractor.
